1.Experimental Study on Serial Changes of Tc-99m-MDP Scintigraphy after Tibia Fracture

The purpose of this study was to investigate that there was any differences of uptake ratio of Tc99-m-MDP during the healing process related to fracture level in tibia. Thirty mature rabbits were devided into three groups and each group consisted of ten rabbits. The osteotomy was performed at the proximal third of tibia in group I, at the middle third in group II and at the distal third in group III, respectively. After fracture was reduced, the fractured tibia was fixed with K-wire and immobilized with long leg cast. Serial Tc-99m-MDP scintigraphy with pixel counting method was performed in thirty cases of tibial shaft fracture from one day to eight weeks after fracture. The results obtained were as follows: l. In group I, the mean uptake ratio of Tc-99m-MDP at fracture site was 1.40±0.45 at two days after fracture, 3.34±1.21 at two weeks and increased rapidly to a maximal value of 7.51%1.01 at three weeks and decreased thereafter to 4.530.83 at eight weeks. 2. In group II, the mean uptake ratio was 1.10±0.10 at three days, 3.931.±10 at three weeks and increased rapidly to a maximal value of 7.37±2.21 at four weeks and decreased thereafter to 4.54±0.96 at eight weeks. 3. In group III, the mean uptake ratio was 1.13 ± 0.11 at three days, 3.95±0.93 at three weeks and increased rapidly to a maximal value of 7.43±1.72 at four weeks and decreased thereafter to 4.69±0.93 at eight weeks. These experiments suggest that the changes of Tc-99m-MDP scintigraphy in the proximal third fractures of tibia was earlier than middle or distal third fractures and there was no differences between middle and distal third fractures of the tibia.

2.Job Stress, Job Satisfaction, and Organizational Commitment according to Life Organization (LIFO) Behavior Type of Novice Nurses.

PURPOSE: This study was done to provide data that would contribute to effective task performance in clinical nursing and development of nursing professional by understanding job stress, job satisfaction and organizational commitment according to Life Organization (LIFO) behavior types of novice nurses. METHODS: This study was a cross-sectional survey and the participants were 313 novice nurses who worked in one university hospital located in the city of Gwangju. Data were collected through the structured questionnaires that included general characteristics, job stress, job satisfaction, and organizational commitment scales. Collected data were analyzed using descriptive statistics, and ANOVA with SPSS/WIN 19.0. RESULTS: Regarding job stress according to LIFO behavior types of novice nurses, there were statistically significant differences between the favorable situation and the unfavorable situation. There were also statistically significant differences regarding the organizational commitment according to LIFO behavior types. CONCLUSION: The results indicate a difference in behavior patterns of novice nurses that suggest a need for education in order to reduce stress from the job and enhance organizational commitment by developing each nurse's preferred pattern and compensating for unfavorable patterns, as well. There also needs to be nursing resources that take into consideration behavior patterns of novice nurses.

3.Tibia Fracture on Removal of Interlocking Medullary Nail: A report of 3 cases.

Interlocking medullary nails have been widely used in the treatment of long bone fractures especially in tibia and femur. But with increasing use of nails, many preoperative, intra-operative and post-operative complications have been reported. We have experienced 3 cases of very rare complication, fracture of tibia shaft which occurred during removal of interlocking medullary nail after complete healing of original tibial fracture. We report these rare complications with literature review.

4.Impairment Grading in Orthopedic Certificates.

But many orthopedic surgeons are not accustomed to issue such medical certificates. So we want to review the way of evaluation and grading of disabilities for such different kinds of medical certificates(for examples: disability grading certificate for use in traffic accident, industrial accident, liability of national compensation, private insurance, the handicapped, military service and others) and compare the differences between them.

5.Buerger's Disease of Paratesticular Tissue: A case report.

We report a case of Buerger's disease manifested by a scrotal mass, in view of its rarity. A tender bean sized mass was palpated in the left scrotum of a 34 year-old male heavy smoker, who had a history of right pain two years ago which subsided spontaneously. The left testis with the paratesticular tissue was excised. Vessels of the spermatic cord and epididymis showed microscopic changes of Buerger's disease. The involved arteries and veins revealed a predominantly subacute pattern with granulomatous inflammation and Langhans' type giant cells within the thrombi.

7.Sertoli-Leydig Cell Tumor of Hemangiopericytoma Pattern: A case report.

The Sertoli-Leydig cell tumor is a gonadal tumor of sex-cord stromal type, similar to that seen in of the various phases of testicular development in the male. This tumor is exceedingly rare, accounting for only 0.1% to 0.5% of all primary ovarian neoplasms. It occurs predominantly in the second and third decades(mean age about 25 years), less than 10% after menopause. We investigated a case of poorly differentiated Sertoli-Leydig cell tumor of right ovary, occured in a 76-year-old woman. Grossly, the tumor measured 2, 100 gm in weight and 25 x 19 x 8 cm in dimensions. The outer surface was smooth and glistening without rupture of the capsule. Cut sections revealed a multilobulated brown solid mass with multiple cystic change. Microscopically, it showed the typical findings o a Sertoli-Leydig cell tumor. The characteristic feature is hemangiopericytoma paftem of sarcomatoid spindle cells. Therefore, we present it with a brief review of the literature.

8.Malignant Small Cell Tumor of the Thoracopulmonary Region (Askin Tumor): Report of a case.

Malignant small cell tumor(MSCT) of the thoracopulmonary region(Askin tumor) is extremely rare and is seen predominantly in children and adolescents. This tumor represents a distinct clinicopathologic entity of neuroectodertnal origin, arising from the soft tissues of the chest wall or peripheral lung. This tumor tends to recur locally, but does not seem to disseminate widely. The overall survival is poor. Recently, we experienced a case of the MSCT of the thoracopulmonary region of a 12-year-old female. She was admitted because of a chest wall mass on radiographic examination, and a complaint of intermittent chest pain. Grossly, the mass was lobulated, round and had a solid appearance with focal necrosis and hemorrhage on the cut surface. Histologically, small round to oval cells were arranged in compact sheets, nests and lobular patterns with intervening fibrovascular stroma. Ultrastructurally, the presence of loose-fitting membrane-bound neurosecretory granules was noted.

9.Foreign Bodies in the Chest: How Come They Are Seen in Adults?.

The radiologic and clinical findings of foreign bodies in the chest of children are well recognized. Foreign bodies in adults are infrequent, however, and the radiologic findings of these unusual circumstances have rarely been described. We classified various thoracic foreign bodies into three types according to their cause: Type I, Aspiration, Type II, Trauma or Accident; Type III, Iatrogenic. This pictorial essay will illustrate the radiologic findings and consequences of thoracic foreign bodies in adults, which have rarely been described in the radiologic literature. The clinical significance of thoracic foreign bodies will be also be discussed.

10.A Study of Bcl-2 Oncoprotein Expression in Endometrial Carcinoma Correlated with Hormone Receptor Status.

Bcl-2 is a proto-oncogene initially described in follicular lymphoma, associated with chromosomal translocation(14;18). Recent studies have shown the presence of Bcl-2 in nonhematolymphoid tissue, especially in hormonally responsive tissue. The endometrium is an attractive model for studying the hormone dependent regulation of Bcl-2 expression. We have studied the immunoreactivity of Bcl-2 oncoprotein in relation to the immunoreactivity of estrogen receptors(ER) and progesterone receptors(PR) by immunohistochemistry in 52 human endometrial carcinomas, according to nuclear grade. The results obtained are summarized as followings, 1) Immunohistochemical grade of Bcl-2 showed a significant inverse correlation with nuclear grade. 2) Immunohistochemical grades of ER and PR also showed a significant inverse correlation with nuclear grade, and were well correlated with each other. 3) Immunohistochemical grades of Bcl-2 and hormone receptors showed a strongly significant correlation. On the basis of the above results, we suggest that Bcl-2 expression may be under hormone dependent control and that it can be used in prognosis and choice of hormonal therapy in the presence of hormone receptor.
