1.Change in Plasma Homocysteine Concentration during the Recovery Phase of Renal Transplantation.

A prospective study of vitamin B6, B12, folate, and homocysteine levels was carried out in 7 kidney transplant(KT) recipients. The first sample for the basal level was drawn on the morning of the KT day before the start of cyclosporine injection. Thereafter, serial blood samples were taken every day until the serum creatinine level decreased below 1.5 mg/dl and then every 2 or 3days until discharge. The serum creatinine level decreased to below 1.5 mg/dl within 3days except for cases 4 and 6. The homocysteine levels decreased markedly in synchronisation with serum creatinine levels increased during the first 7days and then declined together with serum creatinine levels but went up again after a few days. The levels of B6, and vitamin B12 do not continue to decrease after the levels of serum creatinine have decreased to 1.5mg/dl, while homocysteine levels go up progressively. In conclusion, contrary to our expectation, the level of homocysteine rebounds a few days after KT following a transient decline. A deficiency of folic acid, vitamin B6 and vitamin B12 does not seem to cause hyperhomocysteinemia in KT recipients. It is necessary to pay attention to hyperhomocysteinemia in KT recipients, especially when the recipients have an atherosclerosis-related complication.

2.Histological Changes of Innervated Muscles after neourolysis: An Experimental Study on Rabbit Sciatic Nerve

The normal vascularization of peripheral nervesand the vascular factor in peripheral nerve les ons have regained increasing interest among surgeons. So, several attempts have been made to assess the relative importance of the vasa nervorum and intrinsic longitudinal vascular plexuses of nerve in maintaining the blood supply.of a segment of nerve trunk. The purpose of our experiment was to determine in laboratory animals the maximum extent to which a nerve can be mobilized without impairing its vascular supply so much that nerve function is jeopardized. All our studies were carried out on both sides of the sciatic-tibial nerve of thirty-two rabbit which were anesthetized intraperitoneally with urethane. The experimental procedure differed in three groups, Group I in which the sciatic nerve was mobilized 3 cm in length, Group II was mob lized 7 cm length and Group III was mobilized 10cm long. The tibialis post. muscles of each group were analyzed at intervals ot 1,2,4,6 and 8 weeks after neurolysis. Each muscles were examined grossly and histologically after hematoxylin and eosin staining. Experimental studies showed that a peripheral nerve is a well vascularized structure with a considerable reserve capacity in its microirculation. The intrinsic collateral system is well developeed and experimental deta supported the view that peripheral nerves may be mobilized over a cons derable length with or the only minium interference with their microvascular flow. The results were as follows: 1. The first evidence of histologic change in the muscle fibers was in the sarcolemmal neclei. 2. Localized atrophy of muscle fibers were observed at the six weeks after neurolysis. 3. With increasing length of neurolysis, abnormal finding were developed in early stage. 4. Massive atrophy of muscle fibers were noted in the muscle fibers which neurolysed more 7cm.

3.Studies on the VP4 and VP7 Genes of Bovine Rotaviruses from Field Samples Using RT-PCR and RFLP Analysis.

Characterizations of the VP4 (P type) and VP7 (G type) genes of Korean isolates of bovine rotavirus were performed using RT-PCR/RFLP and nucleotide sequencing analysis. After RT-PCR amplification of partial length (1094bp) of the VP4 and full length (1062bp) of the VP7 genes, amplified PCR products were digested with restriction endonucleases and digestion patterns were compared with those of reference rotaviruses. With the VP4 genes, four RFLP (AD) profiles were observed; three (A, B and C) were the same as those of bovine rotavirus NCDV (P[1]), IND (P[5]) and B223 (P[11]), respectively, Profile D was the same as that of porcine rotavirus OSU (p[7]). With the VP7 genes, five RFLP profiles (I-V) were observed; three of them (1, II and III) were the same as those of bovine rotavirus NCDV (G6), Cody I-801 (G8), and B223 (G10), respectively, Profile IV and V were atypical to those of reference bovine rotaviruses used in this study. These two profiles were identified as G6 and G5, respectively, after analyzing and comparing the nucleotide sequences. The G typing analysis revealed that 61.9% (26/42) were G6, which included G6 subtype; 28.6% (12/42) were G5; 7.1% (3/42) were G10; 2.4% (1/42) were G8. The P typing analysis revealed that 54.8% (23/42) were P(5); 28.6% (12/42) were P(7); 11.8% (5/42) were P(11); 4.8% (2/42) were P(1). Our results showed that G6/P(5) were the most prevalent rotaviruses in diarrheic calves in Korea. Also, this is the first report that G5P(7) rotaviruses were identified from cattle with diarrhea.

7.Clinical Features and Outcomes from Diagnostic Work-up in Definite Multiple Sclerosis.

BACKGROUND: To observe the clinical features and the outcomes from diagnostic work-up in definite multiple sclero-sis (MS) which was diagnosed by using a Poser's criteria. METHODS: We retrospectively evaluated 43 cases who were diagnosed as multiple sclerosis in Seoul National University Hospital by using Poser's criteria from January 1st, 1991 to December 30th , 1995. Among the 43 cases, 35 patients were classified to definite MS group, clinically or laboratory supported. We analyzed age, sex, initial symptom, age of onset, duration between initial symptom and definite MS diagnosis, total numbers of recurrence and symptoms, CSF profile, evoked potential study, follow-up duration and the MRI findings of the definite MS patients. RESULTS: Of 35 cases who were diagnosed as definite MS, 15 patients were male and 20 patients were female. The mean age of onset was 34.6 years, the mean duration between initial symptom onset and diagnosis of definite MS was 16.9 months, and the mean follow-up duration was 34.8 months. The most common site of initial involvement was spinal cord (24 cases), and then optic nerve (5 cases), brain stem (4 cases), and cerebrum (2 cases) were followed. Thoracic spinal cord (24 lesions) was more frequently involved than cervical cord (18 lesions). Moreover 32 cases (91.4%) had at least one time of spinal cord involvement and there is a tendency that the proportion of double spinal cord lesion is increasing with the number of spinal cord recurrence. CONCLUSIONS: Spinal cord involvement in multiple sclerosis is the most common symptom and double spinal cord involvement in MRI is frequently observed. We think that it would be the characteristics of the multiple sclerosis in Korea.

9.Reading and Understanding of Food & Nutrition Labels and Dietary Behaviors of Female Middle and High School Students.

This study investigated the use and understanding of food and nutrition labels in 498 female adolescents (middle school students: MS 248, high school students: HS 250) in Gyeonggi-do. Fifty percent of the MS and 47.2% of the HS read food labels, and the most common reasons for reading labels were to find a product's expiration date and price. The food label information considered most important by the subjects was the name of the manufacturing company and expiration date. Over 80% of the subjects read nutrition labels. The MS read nutrition labels to find nutrients and their amounts in foods, while the HS read labels mostly in an effort to control body weight. These subjects gave more attention to calories and fat, the nutrients related to body weight, than to other nutrients. The subjects were highly aware of the necessity and positive effects of nutrition labels, because they believed labels could make it easier for them to choose healthy foods. However, scores for understanding nutrition labels showed the subjects failed to understand label information accurately. Generally, the MS showed better dietary behaviors than HS. But the HS had significantly higher scores than MS for the item "know relative weight with height." Those that read food labels had significantly better BMIs, dietary behaviors, and awareness and understanding of nutrition labels. There were significant positive relationships among awareness & understanding of nutrition labels and subjects' dietary behaviors. The study findings can be utilized to better plan nutrition education programs aiming to improve use and awareness of food and nutrition labels among adolescents.
