1.Relationship Between Self-reported Symptoms of Work-related Musculoskeletal Disorders and Health Related Quality of Life.

OBJECTIVES: To explore the relationship between symptoms of work-related musculoskeletal disorders (WMSDs) and health-related, quality of life (QoL). METHODS: WMSDs symptoms were measured by a self-assessed questionnaire on musculoskeletal disorders symptoms and health-related QoL was measured by SF-36. Analysis of variance and logistic regression were used to analyze the relationship between WMSDs and health-related QoL. RESULTS: The prevalence of WMSDs was 64.6% in the upper limbs, 44.0% in the low back, and 39.6% in the lower limbs. Subjects with musculoskeletal symptoms had significantly lower scores on all SF-36 dimensions than those without symptoms among all body parts. Compared with the control, the case's odds ratio was higher in all SF-36 dimensions, especially for physical functioning. CONCLUSIONS: WMSDs have a negative effect on health-related QoL. The essential prevention of WMSDs requires intervention for health promotion in manufacturing workers.

3.Health Inequity among Waged Workers by Employment Status.

OBJECTIVES: The purpose of this study was to examine the relationship between the differences in employment status and self assessed health in Korea. METHODS: We analyzed 4 year follow-up data generated by the Korean Labor and Income Panel Study(KLIPS), which was conducted on 1,207 men and 582 women who had undergone a change in employment status. The study subjects were placed into 1 of the following 4 groups based on their employment history; Non-precarious workers, Precarious to non-precarious workers, Non-precarious to precarious workers and Precarious workers. Logistic regression was then used to examine the relationship between the changes in employment status and self assessed health. RESULTS: When males were considered, self assessed health was better among the precarious to non-precarious workers (OR 1.58, 95% CI=1.57-1.60) and the precarious workers (OR 1.29, 95% CI=1.28-1.30) than in the non-precarious workers, after adjusting for age, socioeconomic status (education level, occupational class, marital status, average equivalent household income and average number of hours worked per week), health behavior (smoking, drinking and exercise) and medical service access (regular medical examination, have chronic disease or hospitalized within 1 year). When female workers were considered, the precarious to non-precarious workers (OR 1.89, 95% CI=1.86-1.92), non-precarious to precarious workers(OR 1.24, 95% CI=1.23-1.26) and precarious workers (OR 1.27, 95% CI=1.25-1.28) all reported poorer health than the non-precarious workers after adjusting for the aforementioned factors. CONCLUSIONS: This study showed that changes in employment status were associated with differences in self assessed health among men and women. Specifically, the results of this study showed that a corresponding positive outcome based on self assessed health was greater for employees that changed from precarious to non-precarious jobs and for male employees with precarious jobs, whereas female employees with non-precarious jobs had higher self assessed health. However, additional longitudinal studies on the health effects of employment status should be conducted.

4.An experimental model of ischemia in rabbit hindlimb.

This study was performed to establish an experimental model of ischemia for the investigation of new treatment modality of limb-threatening ischemia. We produced ischemia in the hindlimbs of 8 New Zealand white rabbits. Under general anesthesia, the left femoral artery was exposed, freed, and excised from distal external iliac artery to proximal popliteal and saphenous arteries. And then both hindlimbs were serially examined to assess the ischemia according to the time table until postoperative 6 weeks. We assessed clinical observation, blood pressure, radioisotopic perfusion scan, and angiography. Clinical ischemic changes of the operated feet were observed in 63%. The blood pressure of left calves was measurable on postoperative day 3 (p<0.05, vs preoperative day 2) and then gradually increased to reach a plateau in postoperative week 6. Radioisotopic arterial perfusion showed similar profiles as in blood pressure. Angiography of ischemic hindlimbs demonstrated a few collateral vessels arising from the internal iliac artery with the reconstitution of the posterior tibial artery in postoperative week 2. In postoperative week 6, collaterals remained the same in number. However, these became dilated and tortuous and showed reconstitution in distal hindleg. In conclusion, this is a reproducible, measurable, and economical animal model of hind limb ischemia.

5.Detection of the Expression of GnRH and GnRH Receptor mRNAs and Effect of GnRH on Prostate Cancer Cell Proliferation.

Prostatic cancer is the typical hormone dependant cancer and several kinds of hormones are used for the treatment of prostate cancer. Since Harris had proposed the hypothalamo-pituitary gonadal axis, the hypothalamus is believed the exclusive organ producing GnRH. According to the recent researches, several organs were proved to be the extrahypothalamic GnRH sources in human and animal. In this research, the expression of the GnRH and GnRH receptor mRNA, detection of the GnRH which prostate cancer cell produced and effect of the GnRH on the prostate cancer cell proliferation using three human prostate cancer cell lines, ALVA 41, ALVA 101 and DU-145 were studied. In Situ Hybridization method was used for the detection of the expression of the GnRH and GnRH receptor mRNA. The charcoal stripped serum and high performance liquid chromatography were used for the detection of the GnRH produced from prostate cancer cells. Thymidine incorporation assay was used for the evaluation of the effect of the GnRH on the prostate cancer cell proliferation. The GnRH mRNA were detected in 96.7% of ALVA 41, 91.5% of ALVA 101 and 95.3% of DU-145 and GnRH receptor mRNA expression signals were detected in almost all of the examined prostate cancer cells, more than 95%, in three cell lines. The number of signals of the GnRH receptor mRNA were more than GnRH mRNA. The GnRH produced from the rostate cancer cell was detected at culture medium with retention time 19.40 minutes. The cancer cells cultured with peptide hormone deficient medium using charcoal stripped serum showed more than 20% growth retardation to the cancer cells grown at the medium used normal serum. The treatment of the GnRH on the cancer cells growing at the peptide hormone deficient medium showed statistically insignificant dose dependant growth retardation. The RESULTS of our research showed that the human prostate cancer cells, including two hormone refractory prostate cancer cell lines, produce the GnRH and the GnRH receptor in the same cell which could be suggest that the role of GnRH produced from the prostate cancer cell would be autocrine action. And the prostate cancer cell growth was down regulated by unknown complex of various peptide hormones and the GnRH does not has the significant effect on the proliferations of the prostate cancer cells. With those RESULTS we obtained in this research and other's data, it seems that there is a system that contains production of GnRH and GnRH receptor and metabolic mechinary within prostate cancer cell. And there should be the some changes in the hypothalamo-pituitary gonadal axis and the mechanism using GnRH analogues for the treatment of prostate cancer aside from central mechanism.

6.Clinical and Angiographic Characteristics and Long-term Follow-up in Patients with Variant Angina Who Presented as Acute Myocardial Infarction.

BACKGROUNG AND OBJECTIVES: There were numerous reports for clinical characteristics and prognosis of patients with variant angina (VA) but little information is available for patients with VA who presented as acute myocardial infarction (AMI). The purpose of this study is to determine the clinical and angiographic predictors for initial development of AMI in patients with VA and prognosis of patients with VA who presented as AMI. MATERIALS AND METHODS: The study group comprised 166 patients with VA:forty one (25%) of whom presented as AMI (Group A;Male 32, mean age 50 years) and 125 presented as typical VA or unstable angina (Group B;Male 73, mean age 54 years). The diagnosis of VA was made by spontaneous spasm and ergonovine or acetylcholine (only Group B) provocation. RESULTS: 1)Male gender (78% vs. 58%, p<0.05), smoking (74% vs. 53%, p<0.05), and disease duration (18+/-5 vs. 7+/-1 month, p<0.0001), and ST-segment elevation during chest pain (71% vs. 23%, p<0.05) were significantly higher in group A than in Group B. 2)Prevalence of fixed stenosis of 50% or greater was higher in Group A than in group B (12% vs. 2%, p<0.05) and the percent stenosis after nitroglycerin injection was also greater in group A than in group B (43+/-5% vs. 28+/-2, p<0.01), but the disease activity such as frequency of resting angina, spontaneous spasm, and multivessel spasm were not different between two groups. 3)During clinical follow-up at a mean duration of 2.7 years, three patient (2%) in group B died of a cardiac cause. Non-fatal MI occurred 1 (2%) and 3 patients (2%) in group A and B, respectively. CONCLUSIONS: Our data show that male gender, smoking, duration of disease, ST-segment elevation during chest pain, and a fixed stenosis of 50% or greater are predictors for initial development of AMI in patients with VA. The prognosis in group A is excellent and this may be associated with less severe atherosclerotic disease and a high rate of medication with calcium channel blocker or nitrate compared with those in previous studies.

This study aimed to characterize the seasonal abundance of hard ticks that transmit severe fever with thrombocytopenia syndrome virus from April to November 2019 and 2020 on Ganghwa-do, Incheon Metropolitan City, Korea. The ticks were collected at grassland, grave site, copse and mountain road using a collection trap method. The ixodid hard ticks comprising three species (Haemaphysalis longicornis, H. flava, and Ixodes nipponensis) collected were 6,622 in 2019 and 3,811 in 2020. H. longicornis was the most frequent (97.9% in 2019 and 96.0% in 2020), followed by H. flava (2.0% and 3.0% in 2019 and 2020, respectively) and I. nipponensis (less than 0.1%). Our study demonstrated that seasonal patterns of the tick populations examined for two years were totally unsimilar. The hard ticks tested using RT-qPCR were all negative for severe fever with thrombocytopenia syndrome virus.

9.Rescue Utilization of Abciximab (ReoPro) for the Thrombus.

The presence of pre-existing intracoronary thrombus has consistently been shown to be among the strongest predictors of unsuccessful angioplasty and abrupt vessel closure. Abciximab, platelet glycoprotein IIb/IIIa receptor antagonist, through prevention platelet aggregation and coronary thrombosis, has shown promise in helping to decrease the incidence of complications of PTCA when prophylatically administered in patients presenting with unstable angina or complex lesion morphology for PTCA and in lower risk patients as well. However, the cost of abciximab and its associated increased risk of bleeding may limit its use as a prophylactic treatment. This study was performed to evaluate the effect of the rescue administration of abciximab in seven patients with thrombus containing lesion during angioplasty. Thrombus was disappeared in 4 patients and decreased in 2 patients, and the follow-up angiogram showed normal brisk flow in all 6 patients. There were no death or myocardial infarction on clinical follow-up at a mean of 7 months except one which was developed restenosis at the angioplasty lesion. Dissolution of thrombus and restoration or maintenance of TIMI grade 3 flow were achieved without complications after administration of abciximab when delivered in a rescue manner on thrombus containing lesion during angioplasty. These results showed that failure to give preprocedural proph-ylactic abciximab did not appear to exclude the possibility of a beneficial effect of abciximab, given therapeutically during the early stage of thrombus formation in patients with complicated lesion during angioplasty.

10.A case of intestinal non-rotation incidentally detected on DISIDA scan.

The most common developmental anomaly of midgut rotation and fixation is non-rotation. Symptomatic intestinal malrotation is relatively common in infants and children but uncommon in later ages. Most adult cases are silent throughout life and are not discovered unless they cause acute or chronic abdominal pain. Many such patients have ill-defined abdominal complaints and are labeled as having a "functional" disorder since no definite clinical abnormalities are found other than the subjective complaints. The most frequent symptomatic presentation in the adult is midgut volvulus the symptom of which is usually self-limited although often recurrent and sometimes leading to an abdominal catastrophe. The diagnostic means for the adulthood malrotation include simple and contrast radiographic studies and CT scan. Malrotation first detected by cholescintigraphy is rare. We report a case of intestinal non-rotation incidentally discovered on DISIDA hepatobiliary scintiscan.
