2.A Case of Angiocentric Lymphoma.

Angiocentric lymphoma is a lymphoma like lesion which has been known as a lethal midline granuloma, lymphomatoid granulomatosis and polymorphic reticulosis. It involves predominantly lungs, and other extranodal sites such as upper respiratory tract, skin, kidneys, peripheral nervous system and CNS. The authors experienced a case of angiocentric lymphoma which was misdiagnosed as orbital cellulitis because there were persistent epiphora, erythematous swelling in dacryocyst area and diffuse edema in upper and lower eyelids which occurred after functional endoscopic sinus surgery. This case was diagnosised as angiocentric lymphoma through multiple tissue biopsies. This disease is rare, especially in orbit and eyelid. It leads to the destruction and inflammation of tissue. To avoid misdiagnosis to orbital cellulitis, accurate biopsy is needed with the interchange of clinical informations with pathologist. We report this case with the review of literatures.

3.Influences of Heat on Function and Morphology of Peripheral Blood Cells.

BACKGROUND: Heat-treated erythrocytes have been reported to be undergone fragmentation and microspherocytes transformation in vitro. However, the changes for enzyme activity of WBCs is not well known. The purpose of this study is to investigate the effects of temperature on function and morphology of WBCs and platelets. And we also defined the relationship between temperature-induced changes in morphology of the erythrocytes and cholesterol level. METHODS: Peripheral blood specimens collected from 55 normal healthy subjects were assayed. CBC and biochemical test for heat-treated specimens were performed. Osmotic fragility was tested and echinocytes were counted using phase contrast microscope. DNA was extracted from heated WBCs with Wizard genomic DNA extraction kit(Promega Corp, USA) and applied on 1.5% agarose gel. After heating, plasma proteins were fractionated by sodium dodecyl sulfate-poly-acrylamide gel electrophoresis (SDS-PAGE) and compared according to changes of temperature. For the phagocytic function of WBCs, 1 x10(7) CFU/mL of gram-negative bacilli were incubated with specimens for 4 hours. Enzymatic activities for myeloperoxidase(MPO), alkaline phosphatase (ALP), and esterase (EST) were confirmed with cytochemical reaction. RESULTS: After incubation at 50 degrees C for 5 min, platelets were counted as 2,978.2 x 10(9)/L and the number of eosinophil was 5.58 x 10(9)/L. And blood glucose level was decreased from 92.4 mg/dL to 59.1 mg/dL, whereas the value of cholesterol was changed as follows; 201.3 mg/dL at 0 sec, 174.6 mg/dL at 60 sec, 151.2 mg/dL at 90 sec, and 197.6 mg/dL at 5 min. In the same temperature, the proportion of echinocytes was 94.1% at 90 sec and decreased to 2.3% at 5 min. In osmotic fragility test, the hemolysis of RBCs began at 0.74% of NaCl and completed at 0.35% of NaCl. Phagocytic function of leukocytes was lost at 52 degrees C, and enzyme activity was lost at following temperature ; MPO at 70t, EST at 70 degrees C, and ALP at 56 degrees C, respectively. SDS-PAGE patterns reveal individual differences of protein at same temperature condition. Aggregating function of platelets were lost, after incubation at 43 degrees C for 5 min. In EDTA-anticoagulated blood, temperature-induced platelelet aggregation was intensified, while in heparinized blood, platelet aggregation was not induced by heat. CONCLUSION: For morphological transformation of erythrocytes in vitro, 50 degrees C is critical point of temperature to induce fragmentation and microspherocytes. Plasma cholesterol appears to play some role to formation of echinocytes induced by heat. MPO is more stable for high temperature than EST and ALP. And sensitivity of RBCs to temperature is considered to be different individually.

4.A Clinical Study on Trauamtic Hyphema in Chunbuk area.

A retrospective study was made of 334 consecutive cases of traumatic hyphema in the rural Chonbuk population. The average age of these patients was 26.2 years with the majority being male(91.9%). The major causes of traumatic hyphema were blow injuries(166 patients, 49.7%). The only 13 aptients(3.9%) had agriculture related injuries. Secondary hemorrhage occured in 23(6.9%) of all patient, and mainly between the second and fourth day following admission. A separate group of 88 patients(26.3%) who were admitted to hospital 24 hours or more after sustaining injury were found to have a secondary hemorrhage rate of 10.2%(nine of 88 patients). No secondary hemorrhage occured in Grade 0 hyphema. 22 eyes(6.5%) had a final visual acuity of 0.3 or worse. Retinal pathology,not hemorrhage, was most often responsible among those patients suffering poor visual outcome. In this study on the population of Chunbuk province, some patients were treated late, and had a high incidence of secondary hemorrhge.

5.Second-tier Instruction of Cardiopulmonary Resuscitation by CPR Anytime(R) Trainees.

PURPOSE: CPR Anytime(R), a self-instructional video program, has gained popularity amongst CPR instructors for training non-healthcare providers. This instructional kit enables second-tier instruction. The purpose of this study is to determine the status of second-tier instruction CPR by CPR Anytime(R) trainees. METHODS: Questionnaires were sent to 606 CPR Anytime(R) trainees from 12 BLS training sites. The training period was from October 2006 to July 2007. Questionnaires included provider's basic information, post-course self exercise, amount of second-tier instruction, and multiplier status. RESULTS: The response rate of the questionnaire was 53.6%(325). The mean age was 20.4+/-10.3 years old. The professions of respondents were as follows: students (76.3%), office workers(13.2%), teachers(4.4%), and service providers(1.9%). The post-course self exercise rate was 49% with three fourths of those completing the exercises using both the DVD and MiniAnne(R). Second-tier instruction tools use rates(48%) were as follows: DVD and manikin(35%), manikin only(9%), verbal only(2%), and DVD only(1%). The total multiplier effect was 1.77(575/ 325) with the multiplier effect of teachers significantly higher than others. CONCLUSION: This study found that 48% of CPR Anytime(R) providers perform second-tier instruction to family and friends. Amongst them, the total multiplier effect was 1.77 (575/325).

6.Treatment Patterns and Changes in Quality of Life during First-Line Palliative Chemotherapy in Korean Patients with Advanced Gastric Cancer.

PURPOSE: The purpose of this study was to evaluate chemotherapy patterns and changes in quality of life (QOL) during first-line palliative chemotherapy for Korean patients with unresectable or metastatic/recurrent gastric cancer (GC). MATERIALS AND METHODS: Thiswas a non-interventional, multi-center, prospective, observational study of 527 patients in Korea. QOL assessments were conducted using the European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaires (QLQ)-C30 and QLQ-STO22 every 3 months over a 12-month period during first-line palliative chemotherapy. The specific chemotherapy regimens were selected by individual clinicians. RESULTS: Most patients (93.2%) received combination chemotherapy (mainly fluoropyrimidine plus platinum) as their first-line palliative chemotherapy. The median progression-free survival and overall survival were 8.2 and 14.8 months, respectively. Overall, “a little” changes (differences of 5-10 points from baseline)were observed in some of the functioning or symptom scales; none of the QOL scales showed either “moderate” or “very much” change (i.e., ≥ 11 point difference from baseline). When examining the best change in each QOL domain from baseline, scales related to some aspects of functioning, global health status/QOL, and most symptoms revealed significant improvements (p < 0.05). Throughout the course of first-line palliative chemotherapy, most patients' QOL was maintained to a similar degree, regardless of their actual response to chemotherapy. CONCLUSION: This observational study provides important information on the chemotherapy patterns and QOL changes in Korean patientswith advanced GC. Overall, first-line palliative chemotherapy was found to maintain QOL, and most parameters showed an improvement compared with the baseline at some point during the course.
