2.Effects of Power Output and Energy Density on the Histologic Change of Guinea pig Skin Following Copper Vapor Laser Irradiation.

BACKGROUND: Recentliy copper vapor laser(CVL) has been applied with encouraging results in the treatment of port wine stain. The CVL emits yellow light(578nm) in a sequence of about 10,000 to 15,000 pulses/second(10-15kHz) with a pulse duration of 15-50 cases. One of the basic methods of evaluationg the effects of laser lights is to study histologic changes after laser irradiation. OBJECTIVE: The effects of power output and energy density on the histologic change of guinea pig skin following CVL irradiation was evaluted to suggest a way of deciding parameters of CVL to achieve the best clinical results. METHODS: After setting the power output for one of 0.8W, 1.6W and 2.4W, the laser was irradiated with an energy density of 10, 12, 14, 16, 18, 20J/cm. A biopsy was done 15 minutes after irradiation and the biopsy specimens was stained with H&E and NBTC histocberr ical method. RESULTS: 1. At the power of 0.8W, specimens irradiated with an energy density above 14J/cm showed diffuse nonspecific coagulation necrosis of the epidermis and upper dermis. 2. At the power of 1.6W, specimens irradiated with energy density of 14, 16J/cm showed coagulation necrosis of dermal vessel and perivascular collagen. With energe density of 18, 20J/cm, diffuse nonspecific coagulat,ion necrosis of epidermis and upper dermis were observed. 3. At the power of 2.4W, specimens irradiated with an anergy density of 12, 14, 16, 18J/cm showed coagulation necrosis of the dermal vessel and perivascular collagen. With an energy density of 20J/cm, diffuse nonspecific coagulation necrosis of the epidermis and upper dermis was observed. CONCLUSIONS : With the above results it was found that t,he higher power output of CVL enlarged the range of energy density confining thermal damage to the dermal v-ssels and perivascular collagen. Additionally the higher power raised the upper limit of energy density with which t,he epidermis was viable.

4.UVB Induced Minimal Erythema Dose ( MED ) and Minimal Melanogenic Dose ( MMD ) in Young Adult Koreans.

BACKGROUND: The erythemal response and delayed tanning of the skin to UV radiations which are used as diagnostic phototest and guideline of phototherapy are different according to the races and light sources. OBJECTIVE: The MED and MMD induced by UVB radiation were measured in 130 normal young adult Koreans. METHODS: In this study, a high pressure mercury are lamp(Burdick UV-800) and a sunlight fluorescent lamp(Waldmann UV 800) were used as UVB light sources. Multiple sites of the lower baek or buttock skin were irradiated with an increasing dose by a constant, anount. The minimal doses of erythema response and delayed tanning of the skin were assessed visaually at 24 hours and 7 days after irradiation, respectively. RESULTS: MED and MMD of Burdick UV-800 are 18.0+/-9.8mJ/cm2 (mean+/-S.D.), 29.7+/-12.5mJ/cm2, respectively. MED and MMD of Waldmann UV 800 are 70.8+/-28.5mJ/cm2 and 91.2+/-33.1mJ/cm2, respecively. The most frequent MED by Burdick UV-800 is 10mJ/cm2 and those by Waldmann UV 800 are 50m J/cm2 and 60m J/cm2. CONCLUSION: In this study MED and MMD of young adult Koreans by two kinds of UVB light sources were assessed. The RESULTS: how that MED is less than MMD and the levels of MED under which the photosensitivity is suggested are 5.6mJ/cm2 by Burdick UV-800 anr 32.4mJ/cm2 by Waldmann UV 800.

5.Comparison of Clinical Outcome of Excision versus Osteosynthesis in Type II Accessory Navicular.

PURPOSE: The purpose of this study is to compare the clinical outcome of excision versus osteosynthesis of type II accessory navicular performed by a single surgeon. MATERIALS AND METHODS: Cases of 14 feet treated with excision and 13 feet by osteosynthesis for type II accessory navicular of 25 patients from 2002 to 2009 were included in this study. Radiological measurements and American Orthopaedic Foot and Ankle Society (AOFAS) midfoot scale was evaluated. RESULTS: AOFAS midfoot scale of both excision and osteosynthesis groups at last follow-up showed improvement from pre-operation. However, there was no statistical difference in AOFAS midfoot scale and subjective satisfaction between the two groups at last follow-up. In detail of AOFAS midfoot scale, pain and footwear requirements showed statistically favorable results for the excision group, while activity limitation and support showed statistically favorable results for the osteosynthesis group. Subjective recovery time returning to daily activities and starting rehabilitation exercise were 14.6 weeks in the excision group and 13.7 weeks in the osteosynthesis group (p=0.025, Mann-Whitney). Suture anchor loosening was observed in one case in the excision group and non-union in two cases in the osteosynthesis group. CONCLUSION: Both excision and osteosynthesis are favorable surgical methods, but each method has advantages and possible complications such as suture anchor loosening or non-union. Surgeon's preference, patient's chief complaint, specific needs of patient after the operation and consideration of the size of accessory navicular can be a criteria to consider when selecting a surgical method.

7.Three Cases of Membranous Reticular Inflammatory Polyp of the Colon.

Inflammatory polyposis of the colon is rare portion of the colon polyp entity. We experienced three patieats who suffered from colitis of unknown'etiology and discovered membranous retkular with/ without filiform polyp in the distal sigmoid colon of the them. Case 1, 56-year-old female, combined with basaloid cercinoma of the rectal canal that confirmed by pathology after abdominoperineal resection. So we obtained specimen of inflammtory polyp end tried to evaluate the mechanism of it. We found healed ulcer under the mucosal bridge. Case 2. 55-year-old male, had been suffered from irritable bowel syndrome. Prior to visit outpatient clinic, he complained tenesmus with diarrhea for a week. Inflammatory polyp was discovered by colonoacopy but his symptoms relieved by tranqulizers only. Case 3. 57-year-old male, has been a good health. He undertook colonoscopy for routine check thst revealed membranous reticular with filiform inflammatory polyp from distal sigmoid colon to hepatic flexure. Such shape of inflammatory polyp was not documented in Korea, so we reported three cases of it with review of the literature.

10.The Effect of Radial Keratotomy (RK) Combined with Double Ruiz Procedure on the Corneal Curvature.

Under the postulation that double Ruiz procedure could increase the effect of radial keratotomy(RK), RK combined with horizontal and vertical Ruiz procedure was performed in three groups of 22 rabbit eyes. In the group A of six eyes, RK with vertical Ruiz procedure was performed, and eight weeks later the horizontal Ruiz procedure was added. In the group B of eight eyes, RK with horizontal Ruiz procedure was performed, and four weeks later the vertical Ruiz procedure was added. In the group C of eight eyes, RK with horizontal and vertical Ruiz procedure were performed simultaneously. As a control, in the group D of eight eyes, RK without Ruiz procedure was performed. Changes in keratometry for four weeks and eight weeks postoperatively were analyzed. The results were as follows: 1. At eight weeks postoperatively, mean changes in keratometry were 6.434D, 3.663D, 4.030D and 1.585D in groups A,B,C and D, respectively. 2. Mean changes in keratometry of groups A,B and C were significantly higher than that of group D. 3. Mean change in keratometry of 6.434D in group A was significantly the highest of the three groups in which the double Ruiz procedure was performed. From the above results, it was concluded that the double Ruiz procedure increased the effect of RK and that the procedure was more effective when secondary Ruiz procedure was added eight weeks later.
