2.A Case Report of Colorectoanal Intussusception.

Colorectoanal intussusception is a rare and distinct entity that differs from the more common rectal prolapse. Typically the intussusception occurrs with tumor at the apex of the intussuscepted segment acting as lead point. Here we present a case and review the literature of colorectoanal intussusception. The case presented here is that of an elderly woman with a proximal sigmoid colon cancer at its apex. Anterior resection was electively performed after reduction of the intussusception. It is important to differentiate a colorectoanal intussusception from the more common rectal prolapse because treatment may differ. The anorectum remains in its normal anatomic position in colorectoanal intussusception, whereas the anal canal is effaced with the prolapsed segment of bowel in rectal prolapse. Identification of a tumor at the apex of the intussuscepted bowel should also arouse suspicion that the condition is not a rectal prolapse.

3.Unusual Type of Talar Body Fracture: A Case Report

Fractures of the body of the talus are extremely rare, constituting only about 0.1 percent of all fractures (Coltart 1952). Consequently, the prognostic problems have so far been elucidated only on the basis of few reports in the literature. The authors experienced an unusual type of the shearing fracture of the talar body with total disocation that the talar body was lying upside down in the ankle joint. Satisfactory results were obtained after follow up period of 20 months and some are reporting this case with bibliographic reviews.

4.The Effects of 2% Dorzolamide(Trusopt) and 0.12% UF-021(Rescula)on Intraocular Pressure and Peripapillary Retinal and Optic Nerve Head Microcirculation .

We investigated the effect of 2% dorzolamide and 0.12% UF-021 on intraocular pressure and peripapillary and optic nerve head microcirclation with Heidelberg Retina Flowmeter. The 40 eyes of 40 normal subjects were divided 2 groups. 2% Borzolamide and 0.12% UF-021 were instilled in each group. The IOP and microcirculation of peripillary retina and optic nerve head were measured before and 90min, 4, 8, 12, 24 hours after instillation. The IOP was decreased at 90 minutes, 4, 8 hours after instillation of 2% dorzolamide and maximal reduction of IOP was observed at 90 minutes (P<0.05). The IOP was decreased at 90min, 4, 8, 12 hours after instillation of 0.12% UF-021 and maximal reduction of IOP was observed at 4 hours(P<0.05). But, there was no statistically significant changes of microciculation in both groups(P>0.05). We suggest that 2% dorzolamide and 0.12% UF-021 reduce the IOP, but do not alter the mictocirculation of peripapillary retina and optic nerve head.

5.A STUDY ON THE DIMENSIONAL ACCURACY OF MODELS USING 3-DIMENSIONAL COMPUTER TOMOGRAPHY AND 2 RAPID PROTOTYPING METHODS.

Statement of problem. Relatively low success rate of root analogue implant system was supposed to be due to the time duration between extraction and implant installation. The use of three-dimensional computer tomography and the reconstruction of objects using rapid prototyping methods would be helpful to shorten this time. Purpose. This aim of this study was to evaluate the application possibility of the 3-dimensional computer tomography and the rapid prototyping to root analogue implants. Material and methods. Ten single rooted teeth were prepared. Width and height of the teeth were measured by the marking points. This was followed by CT scanning, data conversion and rapid prototyping model fabrication. 2 methods were used; fused deposition modelling and stereolithography. Same width and height of this models were measured and compared to the original tooth. Results. Fused deposition modeling showed an enlarged width and reduced height. The stereolithography showed more exact data compared with the fused deposition modelling. Smaller standard deviation were recorded in the stereolithographic method. Overall width error from tooth to rapid prototyping was 7.15% in fused depostion modelling and 0.2% in stereolithography. Overall height showed the tendency of reducing dimensions. Conclusion. From the results of this study, stereolithography seems to be very predictable method of fabricating root analogue implant.

6.Clinical Evaluation of the Traumatic Optic Neuropathy.

Traumatic optic neuropathy is one of true ophthalmic emergencies and there is no proven form of treatment for traumatic optic neuropathy. 82 cases of traumatic optic neuropathy were investigated to evaluate the effectiveness of high dose corticosteroid for the visual improvement. Age, sex, initial visual acuity, final visual acuity, interval to treatment, the type of trauma and the affected region were studied retrospectively.250 mg of Methylprednisolone was administered intravenously every 6 hours for 3 days, and then followed by tapering using oral prednisone The vision was improved in 36 of 82 cases[43.9%]. It was difficult to interpret the relation-ships between the affected region and visual improvement, the interval for each treatment and final visual acuity. However, the vision was improved in 45 of 50 cases who had an initial visual acuity of above light perception, but in the two of 32 cases with no light perception. If indicated, fifteen cases were treated with a combination of high dose corticosteroid and optic nerve decompression. In initial treatment of traumatic optic neuropathy, high dose corticosteroid was effecive. Whether or not initial visual acuity was better than light perception was a key risk factor in the outcome.

7.Analysis of Angiographic Outcome by Thrombolysis in Myocardial Infarction(TIMI) Frame Count for Primary Stenting in Patients with Acute Myocardial Infarction.

BACKGROUND AND OBJECTIVES: Primary intervention by stent implantation during acute myocardial infarction is a novel strategy to provide better myocardial perfusion compared to thrombolysis or baloon angioplasty. We aimed to assess the reperfusion achieved by primary stenting, employing TIMI frame count for more objective and quantitative measurement. MATERIALS AND METHOD: Measurements for number of frames required to opacify standardized angiographic landmark branch(TIMI frame count) were determined for the coronary arteries of 77 normal controls and 65 patients with acute myocardial infarction who underwent primary stenting within 12 hours of symptom onset. RESULTS: In normal subjects, TIMI frame count for left anterior descending artery(LAD) was 1.3 times of mean count of right coronary artery(RCA) and left circumflex artery(LCx), and significant less than that of TIMI study(22.3+/-4.9 vs 36.2+/-2.6, p<0.05). TIMI frame count for RCA and LCx was similar to results of TIMI study. In infarct related arteries(IRA), corrected TIMI frame count(CTFC) after primary stenting was similar to those of normal control. Frame counts of RCA was larger compared to that of normal control, but statistically insignificant(23.0+/-7.5 vs 17.6+/-3.5, p>0.05). There was no difference of CTFC of non-infarct related arteries between patients and normal controls. CONCLUSION: The TIMI frame count of LAD artery in normal Korean subjects was significantly less than that of American counterpart. In patients with acute myocardial infarction, primary stenting appeared to provide improved coronary flow similar to that observed in normal subjects, as measured by TIMI frame counting.

8.A Case Report of Pleuropulmonary Blastoma in Childhood.

Classic pulmonary blastoma is a variant of carcinosarcoma which is seen almost exclusively in adults. By contrast, most cases of pulmonary blastoma in children have been described as having an exclusive mesenchymal composition, which was proposed as pleuropulmonary blastoma. Recently we experienced a case of pleuropulmonary blastoma, type 1. This 27-month-old male baby was transferred to our hospital due to the left tension pneumothorax. Chest CT revealed a subpleural pulmonary cystic lesion on the left upper lung and an open cystectomy was performed. Histologically the lesion was composed of variable-sized cystic structures lined with a single layer of respiratory-type epithelium. the underlying stroma was composed of sheets of small, round to oval, primitive tumor cells. Some of them had eccentric, eosinophilic cytoplasm, suggestive of rhabdomyoblastic differentiation. These rhabdomyoblastic cells were fuchsinophilic and positive with desmin and vimentin on immunohistochemistry.
