1.A statistical study of dermatoses in the Kangwondo.

17,966 new outpatients who visited the Department of Dermatology of Wonju Christian Hospital from 1985 to 1989 were analyzed statistically. The study results are summarized as follws : 1. Among the 17,966 outpatients, the total number of male patients were 8358 (46.5%) and female patients were 9608 (53.5%). 2. In age distribution, the most frequent age groups were lst decade (24.0), 2nd decade (16.5), and 3 rd decade (25.5) amounting to 66% of the total outpatients. 3. The 15 most common dermatoses were dermatophytosis, atopic dermatitis, urticaria, acne, seborrheic dermatitis, allergic contact dermatitis, other eczema, hypopigmentary disorder, bacterial infection, psoriasis, insect bites, and herpes zoster. 4. Those dermatoses which showed increasing tendency were other eczema, hypopigmentary disorder, herpes zoster, and those of decreasing tendency were scabies, bacterial infection. psoriasis, insect bites. 5. Seasonal distribution. showed those diseases occurring most frequently during the summer season were dermatophytosis, insect bites. urticaria, acne, eczema, contact dermatitis. hypopigmentary disorder, bacterial infection. and herpes zoster. The incidence of atopic deratitis. and psoriasis were incfreased during the winter season. 6. In sexual distribution sebourrheic dermatitis. and scabies were more frequent in the males. whereas with urticaria. acne, contact dermatitis, and other eczema the females were more commonly affected. 7. In age distribution, atopic dermatitis, bacterial infection, and insect bites were the most frequent in the Ist decade. Dermatophytosis. urticaria, acne, seborrheic dermatitis, contact dermatitis, other eczema, hypopigmentary disorder, drug eruption, and psoriasis were most frequent in the 3 rd decade, with scabies in the 2nd decade, and herpes zoster in the 6th decade. 8. The distributions of dermatoses as disease groups, were eczema (23.8%), dermatophytosis (13.9%), urticaria-drug eruption (12.2%), skin appendage disorder (11.2%), and viral infection (8.1%), which constituted 69.2% of the total.

2.A case of Maffucci's syndrome.

Maffucci's syndrome is a congenital, non-hereditary mesodermal d splasia manifested by multiple enchondromas and hemangiomas. Several case reports and reviews indicate that there is a tendency toward malignant transformation of the enchondroma. The incidence of malignancy may be as high as 30%. We report a case of 10-year-old female with multiple hemangiomas on the face, palms, and soles and skeletal chondromatoses of the multiple bones.

4.A Case of Cervical Pregnancy Treated by both Intraamniotic and Systemic Methotrexate Injection.

Cervical pregnancy is a rare form of ectopic pregnancy and it can cause life-threatening bleeding. Until recently abdominal hysterectomy has been considered the standard therapy for fear of profuse hemorrhage. Methotrexate, however, can be useful agent to conserve further fertility and it is used in various routes of administration. We report a case of cervical pregnancy that was successfully treated by intraamniotic methotrexate injection under the guidance of transvaginal ultrasonography and subsequent four systemic doses thereafter.

5.Peripheral neuroblastoma.

A case of peripheral neuroblastoma of a 33-year-old male, which was located on the left buttock, is presented. Histologcally, this tumor demonstrated smaliound cell infiltrations which were arranged in a characteristic rosette pattern and the tumor cells were positively stained with neuron specific enolase. We review the clinical, histopathological ultrastructural and immunohisto chemical characteristics of this rare tumor, as well as the differential diagnosis with other small round cell tumors.

6.Sequential Surgical Treatment of Hemoptysis Caused by: A case report.

We experienced a case of hemoptysis caused by bilateral upper lobe aspergilloma. He was 66 years old and had a history of old unclear pulmonary tuberculosis with irregular medication 20 years ago and intermittent hemoptysis for several years. In x-ray study, there was a bilateral upper lobe aspergilloma with cavity. He received bilateral wedge resection through thoracotomy with some interval to reduce postoperative complications. We selected the priorty of operation through the bronchoscope in the operation room. Both sides had the same pathology of aspergilloma and he was discharged after an uneventful postoperative course.

7.The Relationship of Positive Antimicrosomal Antibody and Postpartum Thyroiditis.

Postpartum thyroiditis is a common but frequently unrecognized disorder, affecting approximately 5% of women during the first 12 months after delivery. It is characterized by transient hyperthyroidism occurring about 14 weeks postpartum followed by transient hypothyroidism at 19 weeks postpartum. Our aim is to investigate the prevalence of positive antimicrosomal antibody in women 3 months postpartum and an association of antimicrosomal antibody with symptomatic and biochemical thyroid disorders. We used data collected from 205 women who visited Dankook University Hospital in 3 months postpartum, Our results showed that the rate of positive antithyroid microsomal antibody was 12.7% in women 3 months postpartum and the prevalence of biochemical hyperthyroidism and biochemical hypothyroidism with positive antithyroid microsomal antibody women 3 months postpartum were 26.9% and 19.2%, respectively. The prevalence of postpartum biochemical thyroid dysfuncion 3 months postpartum was 13.7%. There was no relationship between any of the following factors and thyroid antibody status: fetal distress, birth weight and infant sex, matemal age, experiences and mode of delivery, previous medical disease(such as pstrointestinal and psychotic diseases), experiences of previous abortions, gestational age and past history of thyroid diseases.

8.Esophageal Duplication Complicated with Perforation.

Esophageal duplication cysts are a very rare form of benign esophageal tumors and are frequently asymptomatic. Esophageal duplication cysts result from errors in coalescence of vacuoles during the normal secretion and vacuolization process in the 10-mm embryo. Recently, we experienced a case of long tubular esophageal duplication complicated with perforation into esophageal lumen. We report it with a review of literatures.

9.Infradiaphragmatic Retroperitoneal Pulmonary Sequestration: A Case Report.

Pulmonary sequestration is a very rare congenital malfomation, especially when it is located in the infradiaphragmatic retroperitoneal space. The patient was a 56-year-old man with no respiratory symptom but had some postprandial epigastric discomfort for several days. Plain chest x-ray revealed mild elevation of left diaphragm and abdominal CT scan showed large cystic retroperitoneal mass with some calcification. We performed the operation through the thorachoabdominal incision and the mass was proven to be a pulmonary sequestration. Therefore, we report the case with review of literatures.

10.Expression of E-cadherin according to the Presence of High Risk Prognostic Factors, Clinical Stages and Pathologic Types in Cervical Cancer Patients Treated by Radical Hysterectomy.

OBJECTIVE: To analyze the relationship between the degree of expression of E-cadherin and presence of high risk prognostic factors (lymph node metastasis or parametrium involvement), clinical stages and pathologic types in invasive cervical cancer. METHODS: An immunohistochemical technique has been applied to formalin-fixed, paraffin- embedded samples from 20 radical hysterectomy without risk factors and 16 radical hysterectomy with risk factors. The degree of expressions of E-cadherin immunostaining was compared with the International Federation of Gynecology and Obstetrics (FIGO) stage, presence of high risk prognostic factors, and pathologic types. RESULTS: The difference of the degree of expression of E-cadherin was not statistically significant between high-risk group (lymph node metastasis or parametrium invasion) and non-risk group. The difference of the degree of expression of E-cadherin was not statistically significant according to clinical stages and variable pathologic types either. CONCLUSIONS: These results suggest that the degree of the expression of E-cadherin has no relationship with known high risk prognostic factors, clinical stages and pathologic types in invasive cervical cancer.
