1.Mechanism of pulmonary arterial smooth muscle cell apoptosis induced by nitric oxide

AIM: To explore the mechanism of apoptosis in pulmonary arterial vascular smooth muscle cells (PASMCs) induced by nitric oxide (NO). METHODS: Wistar rat PASMCs were isolated from explants from the intrapulmonary, incubated with NO donor, sodium nitroprussid (SNP) under 12 or 24 hour normoxic and hypoxic conditions. The cell cycle progression and sub-G 1 of PASMC were analyzed via flow cytometric staining of propidium iodide, and the expression of nuclear transcription factor NF-?B and pro-apoptotic factors caspase-3 were detected using immunochemistry staining. 2.Pregnancy after renal transplantation:report of 3 cases and review of the literature

Objective To evaluate the effects of pregnancy and delivery on allograft function and newborns in renal transplant recipients.Methods The effects of pregnancy and delivery on allograft function and newborns in 3 renal transplant recipients were observed and analyzed.The age of the 3 recipients was 32,31 and 31 years,respectively,at pregnancy.They all received triple immunosuppressive therapy(CSA+AZA+prednisone) for the prevention of acute rejection after renal transplantation.Results No acute rejection occurred during pregnancy,and the liver and renal functions were normal in the 3 patients.Preeclampsia occurred in 2 of them.Caesarean section was performed successfully on them at 34,38 and 37 gestational weeks,respectively.The weight of the neonates was 2350 g,3800 g and 3800 g,respectively;and the Apgar scores of the newborns were all 10.(Apgar scores include appearance,pulse,grimace,activity,and respiration.Apgar score ≥8 means newborns in healthy status).After follow-up for 14-46 months,there was no abnormity of the function of transplanted renal and the upgrowth of the newborns.Conclusions Successful pregnancy and delivery are possible in renal transplant recipients with normal renal function.

Protein tyrosine phosphatase (PTP) 1B is a potential target for the treatment of diabetes and obesity. We have previously identified the benzoyl sulfathiazole derivative II as a non-competitive PTP1B inhibitor with in vivo insulin sensitizing effects. Preliminary SAR study on this compound series has been carried out herein, and thirteen new compounds have been designed and synthesized. Among them, compound 10 exhibited potent inhibition against human recombinant PTP1B with the IC50 value of 3.97 micromol x L(-1), and is comparable to that of compound II.

4.Effects of both high glucose and high insulin on proliferations and migrations of human vascular smooth muscle cells as well as miR-145 level

Objective To identify the effect of both high glucose and high insulin on miR-145 level.Methods The human vascular smooth muscle cells ( VSMCs) were cultured and the proliferation of VSMCs was induced by high glucose and high insulin medium.The samples were divided into 4 groups: normal group, high glu-cose group (25 mmol/L), high insulin group (300 mU/L), high glucose and high insulin group.The ex-pression of miR-145 in VSMCs was assayed by real-time PCR.Proliferation of VSMCs was determined by MTT method After 72 h cultivation.The migration of VSMCs was analyzed by cell scratch test .VSMCs in each group was transfected by miR-145 virus ( lentiviral vector ) .Proliferation and migration were assayed after 48 h transfection.Results The expression of miR-145 in VSMCs of other three groups was decreased ( P<0.05 ) , especially the expression in the high glucose and insulin group was the lowest ( P<0.01 ) .Prolifera-tion and migration of VSMCs was promoted by high glucose and/or high insulin medium.Under fluorescent, transfection rate of VSMCs was about 80%after 48 h transfection.Proliferation and migration of VSMCs in each group after transfection were significantly lower than before ( P<0.05 ) .Conclusions High glucose and high in-sulin could decrease the expression of miR-145 in VSMCs, The overexpression of miR-145 may inhibit the prolif-eration and migration of VSMCs .
5.Effect of hydrogen sulfide on oxidative stress in hypoxic pulmonary hypertension

Objective:To study the modulatory effect of hydrogen sulfide (H2S) on oxidative stress in the development of pulmonary hypertension induced by hypoxia. Methods:Twenty male Wistar rats were randomly divided into control group (n=6), hypoxic group (n=6) and hypoxia+NaHS group (n=8). Hypoxic challenge was performed everyday for 21 days. NaHS solution was injected intra-peritoneally everyday before hypoxic challenge for rats in the hypoxia+NaHS group. After 21 days of hypoxia, the mean pulmonary artery pressure(mPAP) was measured by cardiac catheterization. The weight ratio of right ventricle to left ventricle+septum [RV/(LV+SP) ] was also measured. The lung homogenates were assayed for total antioxidant capacity(T-AOC), superoxide dismutase (SOD), oxidized glutathione (GSSG), reduced glutathione (GSH), malondiadehyde(MDA) and hydroxy radical(?OH), and the SOD mRNA levels were assayed by real time polymerse chain reaction. 6.Preliminary application of 3D printing to manufacture a customized guide plate for clinical percutaneous needling at the hip joint

Objective To investigate the safety and efficacy of a customized guide plate which is designed by digital medical technology and manufactured by 3D printing in clinical percutaneous needling at the hip joint.Methods Enrolled in this study were 9 patients with hip disorder who required hip joint needling during the period from April 2015 to August 2015.They were 7 males and 2 females,21 to 53 years of age (average,42 years).Laterality:5 left sides and 4 right sides.Conditions:4 cases of traumatic arthritis and 5 cases of osteoarthritis.All of them underwent lamellar CT scans whose data were imported into the software of Mimics 10.01 for 3D reconstruction.Then the software of 3-matic was used to design a customized guide plate on the reconstructed models.Finally,3D printing was used to manufacture the guide plate.The needle angle,direction and depth were adjusted in vitro and the operation was simulated before actual needling in clinic.After percutaneous hip joint needling was performed in the 9 patients separately,the accuracy of needling was checked using C-arm fluoroscopy.Results The percutaneous hip joint needling was performed successfully in the 9 patients in whom the customized guide plate was used.The results were consistent with what had been designed on the reconstructed models.The operation time in the 9 patients averaged 417.7 s (range,from 387 s to 590 s).Little intraoperative bleeding was observed at the site of needling,fluoroscopy was applied only once for check.No neurovascular injury or other operation-related complications occurred.Conclusion For patients who need repeated percutaneous needling at the hip joint,the customized guide plate designed by us can improve accuracy and ensure safety and efficacy of needling.
7.Rifampicin and isoniazid resistance among pulmonary tuberculosis patients in Luohu District from 2012 to 2022

Objective:
To investigate the resistance to rifampicin and isoniazid and the changing trends among patients with pulmonary tuberculosis in Luohu District, Shenzhen City, Guangdong Province from 2012 to 2022, so as to provide insights into improving drug-resistant pulmonary tuberculosis control and prevention strategies.
Methods:
Basic information, treatment classification and drug resistance data of patients with pulmonary tuberculosis and positive pathogenic detection in Luohu District from 2012 to 2022 were collected through the Tuberculosis Surveillance System of Chinese Disease Prevention and Control Information System, and resistance rates of rifampicin and isoniazid and the changing trends were analyzed.
Results:
A total of 2 126 patients with pulmonary tuberculosis were collected and had a median age of 34 (interquartile range, 25) years, including 1 334 males (62.75%) and 792 females (37.25%). There were 302 patients with drug-resistance in Luohu District from 2012 to 2022, with a resistance rate of 14.21%. Among them, 60 patients were monoresistant to rifampicin (2.82%), 113 patients were monoresistant to isoniazid (5.32%), and 129 patients were multidrug resistant (6.07%). The rate of rifampicin monoresistance showed a downward trend from 2012 to 2022, while the rate of multidrug resistance showed an upward trend (both P<0.05). There was no significant tendency in the rate of isoniazid monoresistance (P>0.05). The rate of multidrug resistance among patients without Shenzhen residence was higher than that among patients with Shenzhen residence; the rates of rifampicin resistance and multidrug resistance among retreated patients were higher than those among treatment-naïve patients (all P<0.05).
Conclusions
The rate of rifampicin monoresistance appeared a downward trend and the rate of multidrug resistance appeared an upward trend among patients with pulmonary tuberculosis in Luohu District from 2012 to 2022. Attention should be given to non-Shenzhen residence and retreated patients.
8.Biomechanical comparison of three methods of internal fixation for distal femoral fractures.

OBJECTIVETo compare the biomechanics of three internal fixations for distal femoral fractures,and to choose suitable clinical internal fixation according to experimental date.
METHODSThe fracture models of femoral condydle were made on 12 fresh cadaver femurs (C1 type of AO classification). The fractures were fixed by supracondylar intramedullary interlocking nails (A), L-shaped condyle plates (B) and compressed plates (C) respectively. The straining, displacement, rigidity and intensity of femoral supracondyla of the three internal fixations of distal femoral fractures were observed and analyzed by biomechanics. These three different internal fixation methods were compared by experimental stress analysis. These results were further confirmed by theoretical analysis using finite element method (FFM).
RESULTSUnder the same load condition, supracondylar intramedullary interlocking nail fixation was the best on the intensity and rigidity,while for compressed plate and L-shaped condyle plate, the femoral condyle intensity and rigidity were worse (P < 0.05). There were distinct differences in stress and displacement among three methods (P < 0.01).
CONCLUSIONSupracondylar intermedullary interlocking nail is the best method among the three internal fixation methods. It has advantage such as screw location, solid fixation and less complication. It is a good internal fixation method for distal femoral fracture.

9.Application df/dt(max) index for determinations of skeletal contractility.

AIMTo analyze the most force rising ratio (+ dF/dt(max)) and the most force descending ratio (- dF/dt(max)) for more sensitive index of skeletal contractility.
METHODSMouse model of swimming training was used, most force (f(max)), + dF/dt(max) and - dF/ dt(max) of gastrocnemius contracting were measured and changes of gastrocnemius supermicro-structure was observed.
RESULTSMitochondria increased and myoneme was regular in general training load, gastrocnemius F(max) - dF/dt(max) significantly increased than in control (P < 0.05), + dF/dt(max) increased more significantly (P < 0.01). Mitochondria was rare and myoneme was unusual in overtraining load, gastrocnemius F(max), - dF/dt(max) reduced significantly than in control (P < 0.05), + dF/dt(max) reduced more significantly (P < 0.01).
CONCLUSION+ dF/dt(max) is better index than F(max) in judging skeletal contractility.

10.Research on long-year-program medical students' pediatric clinical skill training methods

To study the clinical skills training methods and formulate a training objective for pediatric long-year-program students, to let the students, through three years of training program, reach the clinical ability as a chief resident, with better laboratory research skills, teaching awareness and responsibility, and also with good comprehensive quality and excellent English, and finally become clinical talents with great potential and innovation, this report summarized our study results of training two terms of long-year-program pediatric graduates, who fully meeted the training objectives in the department's practice.
