1.Proximal Metatarsal Chevron Osteotomy Combined with Modified McBride Procedures for Hallux Valgus Patients..

Seventy cases of moderate to severe hallux valgus deformities were operated by proximal metatarsal chevron osteotomy combined with modified McBride procedures from September 1994 to September 1996. The hallux valgus angle improved from an average of 35.1 degrees to 12.1 degrees postoperatively. The first intermetatarsal angle improved from an average of 15.4 degrees to 7.9 degrees. The position of sesamoids was normalized and the metatarsal bone shortened minimally. Union occurred in 6 weeks. Subjectively, pain was relieved in 95% of the patients, while walking and standing abilities improved in 90%. The shoewear improved in only 60% of the patients, showing less satisfactory results compared to other factors. In terms of complications, two recurrences occured but were treated without repeat surgery. Two malunions and four superficial wound infections occurred. In conclusion, proximal chevron metatarsal osteotomy combined with modified McBride procedures is a reliable operation as regards stability, technical ease, low complications and satisfactory results for moderate to severe Hallux valgus deformities.

3.A Study on Paranoid Ideation & Obsession in College Freshmen.

The authors studied paranoid ideation and obsession, using Derogatis' SCL-90, in the subjects of 2,564 male and 1,252 female college freshmen of Yeungnam University. The authors collected the reports of Self-Rating Paranoid ideation and Obsession Scale during the periods from January to February, 1987, and applied ANOVA and t-test on paranoid ideation and obsession scores in order to compare them between various psychosocial factors, and sexes. The results are as follows: 1. There was not significant difference in the mean averages of total paranoid ideation scores between male and female students: male students scored 4.16±3.09, female students scored 4.16±2.91. 2. High scored items were delusion and suspiciousness. 3. Fifty-nine male students (2.3%) showed high paranoid ideation scores of 12 or higher, while twenty-six female students (2.1%) showed the same scores. 4. There was strong tendency toward higher paranoid ideation scores in the students who were dissatisfied with their home atmosphere, college, department and familiarity of parents and those who had pessimistic view of self in the past, present or future in both group (respectively P<0.001). The male students who believed protestantism showed higher level of paranoid ideation scores (P<0.01). The male students who attended the department of fine art showed higher level of paranoid ideation scores (P<0.05). 5. There was significant difference in the mean averages of total obsession scores between male and female students (P<0.05): male students scored 10.40±5.43, female students scored 10.75±5.02 6. High scored items were perfectionism, indecisiveness, inattention & obsessive thoughts. 7. Thirty-eight male students (1.6%) showed high obsession scores of 25 or higher, while fifteen female students (1.2%) showed the same scores. 8. There was strong tendency toward higher obsession scores in the students who were dissatisfied with their home atmosphere, college, department and familiarity of parents and those who had pessimistic view of self in the past, present, or future in both group (respectively P<0.001). The female students who attended the department of home economic & whose educational fees were paid by her brothers & sister, showed higher level of obsession scores (respectively P<0.01).

4.A Case of Twenty-Nail Dystrophy.

A 38-year-old female patient presented dystrophic nail changes on her all finger and toe nails for 10 years. On examination, nails were uniformly affected with longitudinal ridging, loss of normal luster and brownish discoloration. This patient has not had any inflammatory skin diseases or systernic illness for the past 10 years. Biopsy examination from the dystrophic nail showed non-specific eczematous changes. The clinical and histologic features of this yatient was considered as the consistent findings with the ty enty-nail dystrophy, occured on the idiopathic base, coined by previous authors.

5.Crotamiton Resistant Scabies.

A case of crotamiton resistant scabies is described in a 21-year-old male patient The scrapings using mineral oil from lesions on the wrist and finger webs showed mites and eggs, typical of scabies. Ten percent crotamitn cream was applied 10 times in one month without any effect. We could not find any source of reinfection in the patient. After above treatment the scrapings still showed mites. and eggs. Retreatment with 1% gamma benzene hexachloride cream was effective in clearing of the skin rashes and itching sensation.

6.A Study on Obsessin & Lie in College Freshmen.

The authors studied obsession, using Derogatis'SCL-90, and lie tendency using MMPI, in the subjects of 2586 male and 1206 female college freshmen of Yeungnam University. The authors collected the reports of self-rating Obsession scale and Lie scale during the periods from January to February, 1988, and applied ANOVA and t-test on obsession and lie scores in order to compare than between various psychosocial factor, and sexes. The results as follows: 1. There was not significant difference in the mean average of total obsession scores between male and female students: male students scored 10.26±5.21 m female students scored 10.54±5.01. 2. High scored items were doubting, indecisiveness, inattention, obsessive thoughts and preoccupation with accuracy. 3. 37 male students (1.4%) showed high obsession scores of 25 or higher, while 9 female students (0.7%) showed the same scores. 4. There was strong tendency toward higher obsession scores in the students who were dissatisfied with their home atmosphere, college, department and familiarity of parents and those who had pessimistic view of self in the past, present of future in both group (respectively P<0.001). The male students who attended the department of fine arts, who grew at the Up& Whose educational fees were paid by brothers & sisters showed higher level of obsession scores (respectively P<0.05, P<0.05, P<0.01). The female students who grew at the Myun & who were self-cook showed higher level of obsession scores (respectively P<0.05). 5. There was not significant difference in the mean averages of total lie scores between male and female students: male students scored 4.73±2.49, female students scored 4.70±2.16. 6. High scored items were NO. 14, 8, 11, 9 and 1 in the table 6. 7. 98 male students (3.7%) showed high lie scores of 70 or high, while 27 female students (2.2%) showed the same scores. 8. There was strong tendency toward higher lie scores in the students who were satisfied with their home atmosphere, college, department and familiarity of parents and those who had optimistic view of self in the past, present or future in both group(respectively P<0.001). The students who attended the department of pharmacy, music, evening programs showed higher level of lie scores (P<0.05).

7.Electronmicroscopic Study of Familial Benign Chronic Pemphigus.

Familial benign chronic pemphigua is a rare disease characterized by a localized recurrent eruption of grouped vesicles and exudative lesions involving the neck, groin, axillae and intertriginous areas. There are no systemic manifestations but chronicity and pruritic lesions creates a significant social and psychological handicap to those afflicted. We presented a case of familial benign chronic pemphigus in a 39 years old man He has had recurrent vesicles, fissures and crust formation in inguinal area for 10 years. Light microscopic findings showed suprabasal vesiculation with acantholysis and villi formation. And electron-microscopic findings showed as follows: Bizzare microvilli changes with widened intercellular spaces, followed by loss of desmosome and complete acantholysis. Tonofilament-desmosome complex is disrupted. Many of acantholytic cells are associated with abnormal keratinization which is repreaented by perinuclear thickened bundles of tonofilament arranged in whorls or lumps. Authors diagnosed with clinical symptoms, light and electron-microscopic findings as Hailey-Hailey disease. The lesions were improved by systemic antibiotics and steroid with cold soaking.

8.A Study on Psychoticism in College Freshmen.

The authors studied psychoticism, using Derogatis's SCL-90, in the subjects of 3,499 male and 1,335 female college freshmen of Yeungnam University. The authors collected the reports of Self-Rating Psychoticism Scale during the periods from January to February, 1986, and applied ANOVA and t-test on psychoticism scores in order to compare them between various psychosocial factors, and sexes. The results are as follows: There was significant difference in the mean averages of total psychoticism scores between male and female students: male students scored 4.62+4.01, female students scored 5.03+3.89 (P<0.01). Thirteen male students (0.4%) showed high psychoticism scores of 20 or higher, while seven female students (0.5%) showed the same scores. The male students who have maturated in city and the female students whose educational fees were paid by self, showed higher level of psychoticism scores (P<0.05). The male and female students who believed Protestantism showed higher level of psychoticism scores (respectively P<0.05, P<0.01). There was a strong tendency toward higher psychoticism scores in the students who were dissatisfies with their home atmosphere, college, department and familiarity of parents and those who had pessimistic views of self in the past, present or future in both groups (P<0.001).

9.Comparison of Job Stressors between Managers and Employees in White-Collar Workers of an Electric Company.

OBJECTIVES: This study was intended to investigate the differences of job stressors between managers and low level employees among white-collar workers. Another objective of this study was to examine whether the effects of job stressors on mental health differ between the two groups. METHODS: Data was obtained from 204 managers and 251 low level employees who were employed in white-collar jobs. Fourteen job stressors and seven job stress variables were measured. RESULTS: Among the 14 job stressors, role overload, job insecurity, and work-family conflict were higher job stressors for the manager group whereas role conflict, work-aptitude incongruity, participation in decision making, and promotion problems were higher job stressors for the low level of employees. There were no differences in job stress scores between the two groups. However, differences in the effects of job stressors on job stress were found between the two groups. For the manager group, job insecurity, work-aptitude incongruity, and work-family conflict significantly affected in explanation of job stress whereas for the low level employees, role underload, peer satisfaction, and environmental problems significantly explaining the job stress variables. CONCLUSIONS: There were significant differences in job stressors between managers and low level employees among white-collar workers. Additionally there were differences in the effects of job stressors on job stress between the two groups.

10.A Clinical Study of Benign Bone Tumors Treated by Partly Deproteinised Xenograft(Kiel Bone)

The use of lyophilised bone from calves as a heterograft was introduced by Maatz and Bauermeister in 1957 as an alternative to autografts and freeze-dried homografts which were then in general use. The Kiel bone was known to possess no antigenecity and serial radiographs revealed that it is incorporated into the recipient site by a process of creeping substitution from surrounding bone. In patients with benign bone tumors, the surgical dead space was filled with Kiel bone graft in the departments of orthopedic surgery, Pusan national university hospital. The results were followed up for more than 14 months, and were evaluated by the palin film findings. The results were as follows : l. Of the 22 cases, satisfactory result was obtained in 18 cases(81,8%). 2. The mean lesion size(diameter of lesion/diameter of bone)of success cases was 61.6%, and the mean lesion size of failed cases was 82.8%. 3. The duration from the Kiel bone graft to the evidence of new bone formation and remodelling was 10.5 months on an average with a range of 7months to 15months, and the duratin from the Kiel bone graft to the complete resorption of the Kiel bone was 45.7 months on an average with a range of 29 months to 66 months. 4. The causes of the failed cases were thought to be incomplete removal of the lesion, decreased vascularity due to repeated surgery, large size of the lesion, and the potentiality of tumor, 5. It was difficult to evaluate success rate according to underlying disease because the number of the cases was not large enough, but giant cell tumor showed high recurrence rate.
