1.Initial Results after Implantation of Coronary Artery Stents with Antiplatelet Agents.

Backgound: The placement of stents in coronary arteries has been shown to reduce acute closure and restenosis in comparison to balloon angioplasty. However, clinical use of intracoronary stents is impeded by the subacute stent thrombosis and hemorrhagic complications associated with the anticoagulant regimen. It's known that the complete stent deployment with high pressure inflation and new antiplatelet agents are effective in reduction of subacute thrombosis and hemorrhage. So we evaluated initial results (success and complication rate) after high pressure-stent deployment with new anticoagulation protocol. METHODS: One hundred and ninety one patients with 201 lesions were treated with 231 stents of various types. The high pressure balloon inflation and antiplatelets agents were used in all cases. Final high pressure balloon inflation guided by IVUS were performed in 23 consecutive cases with incomplete stent deployment according to angiographic findings. RESULTS: 1) The indications of stenting (n=210) were De novo in 124 (59%), bailout procedure in 57 (27%), suboptimal result after PTCA in 19 (8%), and restenosis after PTCA in 14 (6%). The location of lesions were LAD in 101, RCA in 67, circumflex in 28, ramus intermedius in 3, and LMT artery in 2 lesions. Angiographic morphologic characteristics were type A in 2, type B in 158 (B1: 57, B2: 101), and type C in 22 lesions. 2) The angiographic and clinical success rate was 96% (192/201) and 92% (186/201) respectively. 3) In angiographic analysis, the baseline average reference vessel dirmeter was 3.33+/-0.35 mm. Baseline minimum lumen diameter (MLD) was 0.58+/-0.29 mm, with baseline percent diameter stenosis of 82.86+/-8.64%. The final stent diameter was 3.37+/-0.29 mm, with mean final percent stenosis of 0.63+/-8.25. The mean MLD after stenting was significantly increased (p<0.001). The mean MLD within stent increased 14%, from 2.91+/-0.39 mm at the nominal balloon inflation (inflation pressure=7 atm) to 3.37+/-0.29 mm at high pressure balloon inflation (inflation pressure <0A65B>12atm) (p<0.001). The length of lesions in GR I (cook), GR II, and Micro II stents were significantly longer than ones in PS, Cordis, Wiktor, Nir (p<0.001). 4) In intravascular ultrasound analysis, the mean lumen CSA at the tightest point within stent increased 11%, from 8.4+/-2.4 mm2 at the intial intravascular ultrasound to 9.4+/-2.1 mm2 at the final intravascular ultrasound (p<0.001). 5) The procedural and postprocedural complications were 2 acute closures associated with AMI and emergent CABG, 1 subacute closure which was revascularized by bail out stenting, 5 major hemorrhage requiring transfusion associated with 1 CVA and 2 metabolic acidosis induced by acute renal failure, and 5 death. CONCLUSION: The high pressure stent deployment procedure and new anticoagulation protocol associating tidopidine and aspirin without coumadin or prolonged heparin infusion allow us to obtain an acceptably low subacute thrombosis or bleeding complication rate. These results are encouraging and allow a wide use of coronary stenting.

2.Azathioprine Therapy in Henoch-Schonlein Purpura Nephritis Accompanied by Nephrotic syndrome.

In 1989, the Bethesda System (TBS) was introduced as an attempt to standardize cervical/vaginal reporting systems. TBS nomenclature was created for reporting cytologic diagnoses to replace the currently used Cervical Intraepithelial Neoplasia (CIN) and Papanicolaou Class System, which are deemed less reproducible. The name for preinvasive squamous lesions was changed to squamous intraepithelial lesion (SIL), subdivided into low-grade and high-grade types. TBS recommends a specific format for cytologic report, starting with explicit statement on the adequacy of the specimen, followed by general categorization and descriptive diagnosis. Pathologic and epidemiologic studies performed over last 10 years have provided evidence that human papillomavirus (HPV) plays a significant role in the development of cervical neoplasia. TBS corresponds not only to currently held views of the behavior of preinvasive lesions and their HPV distribution, but also to the current guidelines for clinical management.

3.A case of Nutcracker Syndrome Associated with Proteinuria.

We evaluated the availability of toluidine blue stain in body fluids, such as peritoneal and pleural fluid and urine. Nine hundreds specimens, i.e., 400 pleural and 400 peritoneal fluids and 100 urine samples, respectively, from Jan. 1995 to May 1996 were included. We obtained the result of high sensitivity and high specificity in toluidine blue stained body fluid in comparison with Papanicolaou stained result. Additionally, we found the diagnostically important crystals in chylothorax and some urine samples, which can not be seen in routine Papanicolaou stain. We thought the toluidine blue stain in body fluid is one of very useful diagnostic methods.

4.The Effectiveness of Traumatic Symptom Checklist for Children(TSCC): Comparisons of Sexually Abused Children and Nonabused Normal Children.

OBJECTIVES: The research is designed in order to know how much Traumatic Symptom Checklist for Children(TSCC) is effective in the case of child abuses, especially in the estimation of the psychopathology of child sexual abuse. METHODS: A sample includes a group of thirty children of 4th, 5th and 6th grade whose sexual abuse have confirmed and also a group of thirty four normal kids who have no abuse. Along with TSCC, Children's Depression Inventory(CDI) and Revised Children's Manifest Anxiety Scale(RCMAS) were used to children and child behavior checklist(K-CBCL), sexual behaviors(CSBI), and Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and Lifetime version (K-SADS-PL) record sheets were used to kids' parents. RESULTS: In case of the sexual abuse group, the most meaningful difference from the analyzed result is shown at sexual concerns scale which is one of the clinical scale in TSCC(p<.01). Also, the sexual abuse group has the meaningful higher score than the general group's one in the stress scale after the other anxiety, depression, anger and dissociation (p<.01). CONCLUSION: The scale of TSCC has relatively shown the characteristic symptom and severity which were gained from the children who have experienced trauma, especially sexual abuse.

7.Effect of Preexcitation on Doppler Indexes of Ventricular Filling in Patients with Wolff-Parkinson-White Syndrome.

BACKGROUND: Diastolic dysfuction can be assessed by Doppler echocardiography of mitral inflow. Multiple factors including atrioventricular (AV) delay affect the mitral inflow Doppler indexes. This study was designed to assess the changes of mitral inflow patterns after successful radiofrequency catheter ablation (RFCA) of accessory pathway associated with a short AV interval during pre-excitation in patients with WPW syndrome. METHOD: Echocardiogram, ECG and BP were recorded before and after RFCA for treatment of accessory pathway in 15 patients with WPW syndrome (mean age : 39.7+/-14.6). Doppler indexes including E wave velocity and its velocity time integral (EVTI) ,A wave velocity and its VTI (AVTI), deceleration time (DT), isovolumic relaxation time (IVRT), atrial filling fraction (AFF)and total mitral inflow VTI were measured. RESULTS: 1) PR interval prolonged 94+/-18msec to 174+/-34msec (p<0.001) without significant increment of heart rate and blood pressure after successful RFCA. 2) E/A ratio decreased from 1.29+/-0.58 to 1.1+/-0.53 (<0.001) but E wave velocity, DT and IVRT were not changed significantly after RFCA. 3) A wave velocity and AFF was 55.8+/-17.4msec and 0.35+/-0.08 and increased to 61.8+/-19.9msec and 0.42+/-0.1, respectively after RFCA (p<0.05). 4) Total mitral inflow VTI was 13.6+/-3.5cm and 14.9+/-3.6cm before and after RFCA, respectively (p<0.05) CONCLUSION: These results suggested that normalization of the PR interval after RFCA in patients with WPW syndrome had beneficial hemodynamic effects on the stroke volume by changing mitral inflow Doppler indexes. Therefore, the effect of AV delay is an another parameter to consider when evaluating Doppler indexes of LV filling.

Hyalinizing trabecular adenoma is an uncommon benign thyroid tumor that is recently described in the literature. This tumor is easily confused with medullary carcinoma on surgical specimens and with papillay carcinoma on cytologic specimens. Herein we report the cytologic characteristics of a case of histologically proven hyalinizing trabecular adenoma of the thyroid gland. Cytologically, the aspirate showed trabecular or individually dispersed polygonal cells with finely stippled chromatin pattern, nuclear grooves, and eosinophilic nuclear pseudoinclusions. No colloid materials were noted in the background.

9.Usefulness of a Hook in Nasolacrimal Balloon Dilatation and Stent Placement.

PURPOSE: To evaluate the usefulness of a hook developed for pulling out the guide wire in nasolacrimal balloon dilatation and stent placement. MATERIALS AND METHODS: The head was made of stainless steel wire in ashape of a question mark, and the body was made of four stainless steel pipes. The head was attached to the body in three different ways : A) the head was anterior to the body, B) the head was posterior to the body, and C) the head was angled of 30 degrees forward from the direction of the body. 90 epiphora patients took part in a test tocompare and evalvate the usefulness of different types of hook. Average number of trials, success rate and average time of the procedure were recorded. RESULTS: The average trial number for different attachment methods A, B, andC was 4.5, 6.4, and 2.5, respectively, and the success rate 83, 70, and 97%, respectively. The average time of the procedure was 31 minutes. CONCLUSION: The C type hook was the most effective, and the new technique using the hook was more convenient and time-saving than the old technique using a nasal endoscopy.

10.One Case of Intra-Orbital Hemangio-endothelioma.

Here, we report one case of intra-orbital hemangio-endothelioma. The patient was a Korean male aged 37. The most outstanding complaint was exophthalmos of the left eye. His left eye began to protrude insidiously without pain since 4 years ago. A soft movable mass was palpated on the medial portion of the upper orbital margin and the ocular motility was limited to upward, but ocular deviation was not present. Pathologic diagnosis was intra-orbital hemangio-endothelioma, sclerotic, borderline malignancy. This tumor was well encapsulated and any evidence of involving bone and soft tissue was not found. The size of was 2cm X 2.5cm X 3.5cm. Extirpation was performed by means of superior orbital approach. Tumor mass was removed entirely with finger dissection. The vision was not changed but the cosmetic result was satisfactory.
