1.The effects of plaque control instruction in orthodontic patients.

The purpose of this study w-s to predict the respose to the chincap therapy from the initial cephalometric measurements and to obtain the indication of chincap therapy. 40 patients selected for this study were classified into two groups by the occlusal stability after completion of permanent dentition and the iiu4n ovement of facial profile, after chincap therapy. One was g response group which consisted of 25 children and the other was poor response group with 15 patients. Various measurements of the craniofacial structure in the initial lateral cephalogram were calculated and analyzed by t-test and discriminant analysis. The results were, as follows 1. Good response group had more horizontal growth pattern in initial stage of treatment, and the contributing measurements were Bjork sum, anterior-posterior facial height ratio, gonial angle, lower gonial angle and occlusal plane to AB plane angle. 2. The critical points and predictive values of the influential skeletal measurements were calculated. 3. The discriminant function was obtained from three. major influential measurements; Bjork sum, gonial angle and occliml plane to AB plane angle, and this function could discreminate correctly in 85% of this samples.

2.Thrombospondin-1 and -2 Expressions in Hepatocellular Carcinomas: an Association with Tumor Angiogenesis and p53 Overexpression.

Background : It has been suggested that thrombospondin (TSP) is a p53-dependent negative regulator of tumor angiogenesis. TSP expression and localization in hepatocellular carcinomas (HCCs) and its association with overexpression of p53 protein were investigated. Methods : TSP-1 and -2 expressions were examined in 40 HCC specimens by immunohistochemical staining and in 4 HCC cell lines by Western blotting. In addition, p53 protein expression and microvessel density (MVD) were correlated with the TSP expression. Results : Strong immu- nopositivity for TSP-1 was observed in fibroblasts, vascular endothelial cells, and some vas- cular smooth muscle cells of the stroma in 18 cases (45%), and in tumor cells in 3 cases (7.5%) of 40 cases of HCC. Immunoreactivity for TSP-2 was observed in only the sinusoidal lining cells of the tumor in 15 cases (46%), and in tumor cells in 2 cases (6%) of 32 cases of HCC. TSP-1 expression was inversely correlated with MVD (p=0.028), but TSP-2 expression did not show any correlation with MVD. Although p53 was overexpressed in 17 cases, there was no significant correlation between TSP and p53 expressions. None of the HCC cell lines expressed TSP-1 or -2. Conclusions : These findings indicate that TSP-1 is mainly derived from nonparenchymal cells, and may decrease tumor angiogenesis in HCC.

3.Biliary - Gastric Fistula : Report of Two Cases.

Spontaneous internal biliary fistula is not an uncommon complication of neglected cholelithiasis, peptic ulcer, and carcinoma. The indidence, as given by various authors, differs but seems to vary between 3 and 5% of all cases of biliary disease. Until development of endoscopy, diagnosis has depended on the presence of air or barium in the biliary tree as radiologic findings or symptoms. Recently endoscopic examination, biopsy in appropriate case, and cannulation of the fistula for precise radiographic delineation will help to make a diagnosis. The main typesof fistulas are cholecystoduodenal, cholecystocolic, choledochoduodenal. Cholecystogastric or choledochogastric fistula is very rare type of internal biliary fistulas. Recently we encountered two cases who had suffered from fever and right upper abdominal pain with pneumobilia as ultrasonographic findings. They were confirmed as having cholecystogastric fistula, and choledochogastric fistula due to complicated gallstones by ERCP and surgical exploration. So we report two cases of biliary-gastric fistula of these patients with a review of relevant literatures.

6.A Study on the Projection of Critical Care Advanced Practical Nurses(APNs).

PURPOSE: This descriptive study was conducted to project the number of critical care APNs needed in critical care units in an acute care hospital setting, up to the year 2020. METHOD: Necessary data and information were collected from various funded reports, professional literature, web-sites and personal visits to national and private institutions. The demand of critical care APNs were projected based on two critical care APNs per critical care units. RESULT: The projected number of critical APNs for the critical care units in acute care hospital settings as follows: 1) The total projected number of critical care APNs needed for critical care units were 1,270 in 2001. 2) By the year 2020, total number of projected critical care APNs needed in critical care units will be 1,080-1,700. CONCLUSION: In order to match the supply to the need, the professional organization should direct their efforts toward enacting legislation. Educational systems should identify strategies in initiation of critical care APN programs in masters level as well as standardizing curriculums across the programs.

9.Temperamental Characteristics of Korean Children from the Viewpoint of Family Environment and Development.

OBJECTIVES: The purpose of this study was to find the differences of temperamental characteristics of Korean children according to family environment and developmental history. METHODS: 1) The mothers whose children had been educated at twenty-five Samsung Child Care Centers nationwide participated in the survey using Parent Temperament Questionnaire for Children (PTQ) and the Child Development Questionnaire (CDQ). 2) The temperament of 1,175 children whose mothers completed PTQ were classified into 5 diagnostic clusters (Easy, Intermediate Low, Intermediate High, Difficult, Slow-To-Warm-Up) according to the method proposed by Fullard et al. 3) The results of classification were compared and analysed statistically according to each CDQ parameters. RESULTS: Statistically meaningful difference in the distribution of temperamental clusters were found in the CDQ parameters such as birth order of children, number of intimate friends, children's relationship with their peers, children's relationship with their siblings, children's relationship with their parents, type of milk feeding, motor development of children during 2 years after birth, motor development during from age 2 to 5, stranger anxiety, present health state of children, and relationship between parents. After all, the proportion of Easy type was greater in the children who were first-born, or who had good relationship with their peers or family, fast motor development, mild stranger anxiety, good health condition, or good relationship between each parent. CONCLUSION: This study shows that the distribution of temperamental cluster was significantly different in several parameters of family environment and developmental history of Korean children. The children with fast development, good interpersonal relationship or favorable family environment are more likely to be the Easy type.

10.Reversible Pontine MRI Lesion in Acute Thalamic Infarct: Reversible Encephalopathy due to Hypertension?.

Posterior reversible encephalopathy syndrome associated with hypertension rarely presents with predominant involvement of the brainstem and is relative sparing of the supratentorial regions. A relative paucity of brainstem signs and symptoms, despite extensive neuroimaging abnormalities therein, support the diagnosis. Although elevation of blood pressure is common in acute cerebral infarction, concomitant brainstem edema has not been reported. We describe here the clinical and neuroimaging features of an unusual brainstem hyperintensity associated with acute ischemic stroke. The neuroimaging abnormalities improved after stabilization of blood pressure, distinguishing this syndrome from brainstem infarction.
