1.Pineal Anlage Tumor: A case report.

The term "pineal anlage tumor" has been recently proposed and few cases have been reported. We report the first Korean case of pineal anlage tumor in a 6-year-old girl who complained of headache and vomiting for 2 months. Brain MRI revealed a well defined, lobulated, calcifying mass in the pineal region. Tumor was totally removed. Pathological examination revealed a primitive pineal parenchymal tumor with melanotic epithelial component that was similar to histologic findings of melanotic neuroectodermal tumor of infancy, so-called retinal anlage tumor and of the developing pineal gland. The tumor was composed mostly of small, undifferentiated cells, Flexner-Wintersteiner rosettes, and ganglionic differentiation. The tumor also contained the cartilage and skeletal muscle cells.

2.Prevention of chemotherapy-induced oral mucositis in patients with acute leukemia by the two oral care protocols : the comparisons of Sodium Bicarbonate-normal saline gargling and Chlorhexidine gargling.

Oral mucositis continues to be a major complaint of patients who have chemotherapy for the acute leukemia. An innovative and inexpensive remedy which produces favorable results for those afficted is not yet introduced. So we tried to develop two oral care protocols for reducing the level of oral mucositis during cytotoxic therapy through literature review and our clinical experience. The one is sodium bicarbonate-normal saline gargling, and the other consists of chlorhexidine gargling. This quasi-experimental study was performed to compare the efficacy of these two different oral care protocols. Twenty subjects were assigned to one of the two specific diagnosis of leukemia, aim of the chemotherapy. The Oral Assessment Guide(OAG), the Beck's perception of oral comfort, WHO Grading system fot mucositis and the discomfort of oral gargling solution were used to assess oral status and subject's oral discomfort during chemotherapy. Each subjects were observed daily from the start of the chemotherapy until Absolute Neutrophil Count(ANC) reached 1,000. It continued about 2-4 weeks. The data analyzed by Mann-Whittney U test and ANCOVA. The result was follows as: The patient who used sodium bicarbonate-normal saline gargling showed significantly higher mean score of the discomfort of oral gargling solution than chlorhexidine gargling. The other scores were not significantly different between two groups. However the subjects using the sodium bicarbonate-normal saline gargling showed a lower level of oral mucositis. We concluded that oral using sodium bicarbonate-normal saline gargling was between to reduce the level of oral mucositis during chemotherapy and nursing assessments of the oral cavity seemed to promote patient's compliance with the oral care regimen.

4.Orbital Pseudolymphoma: A case report.

Lymphoid tumors of the orbit are rare, and sometimes it is not possible either clinically or histologically to differentiate between lymphoid tumor and pseudolymphoma. Some authors assert that the degree of cytologic differentiation appears to be the single most important factor for determining the prognosis of patients with orbital lymphoid lesions. However, the cytomorphologic basis is not so helpful to diagnose and classify our case, which shows some discrepancy between pathological findings and clinical and radiological findings. At first we misdiagnosed our case as orbital malignant lymphoma on the basis of cytomorphology and immunohistochemical study. But no responce to local intensive radiotherapy and the follow up study of the patient suggest orbital pseudolymphoma.

5.Is 99mTc-MDP Mammoscintigraphy Useful in Patients with Breast Mass Lesions ?.

PURPOSE: Piccolo et al. advocated the usefulness of Tc-99m-MDP breast scan in differential diagnosis of breast mass with high accuracy. But there are little or no follow-up studies about it as we know. We studied Tc-99m-MDP uptake pattern and lesions/background ratio in patients complaining palpable breast lesions to evaluate the clinical usefulness of Tc-99m-MDP scan. MATERIALS AND METHODS: Total 34 patients were studied with physical examination, mammogram and Tc-99m-MDP scan prospectively. Anteroposterior and both lateral view of breast were obtained 5 minutes after iv injection of 740 MBq Tc-99m-MDP. Breast uptake pattern of Tc-99m-MDP was analyzed by a grade system: 0=no uptake, grade 1=bilateral diffuse uptake, grade 2=asymmetric faint uptake, grade 3=focal hot uptake. 20 cases were pathologically confirmed by excision biopsy or aspiration biopsy. 14 cases were normal in physical examination and mammo-grain. RESULTS: Pathologic results showed 7 carcinomas, 6 benign solid tumors, and 7 fibrocystic changes. Grade 3 pattern of Tc-99m-MDP uptake was noted in 4/7 carcinomas, 3/6 benign solid tumors, and 1/7 fibrocystic changes. Grade 2 pattern was 2/7, 0/7, 3/7 respectively. The average L/B ratio was 1.66 in carcinomas, 1.68 in benign solid masses, 1.20 in fibrocystic diseases, 1.05 in normal patients. L/B ratio was higher in carcinoma and benign mass groups than in fibrocystic change and normal control groups(p=0.005). But there was no statistical difference between L/B ratio of malignant mass group and benign mass group. CONCLUSION: Tc-99m-MDP scan is not suitable to routine clinical use for breast mass diagnosis. It might be used in limited conditions when whole body bone scan is planned.

6.A Study of Bcl-2 Oncoprotein Expression in Endometrial Carcinoma Correlated with Hormone Receptor Status.

Bcl-2 is a proto-oncogene initially described in follicular lymphoma, associated with chromosomal translocation(14;18). Recent studies have shown the presence of Bcl-2 in nonhematolymphoid tissue, especially in hormonally responsive tissue. The endometrium is an attractive model for studying the hormone dependent regulation of Bcl-2 expression. We have studied the immunoreactivity of Bcl-2 oncoprotein in relation to the immunoreactivity of estrogen receptors(ER) and progesterone receptors(PR) by immunohistochemistry in 52 human endometrial carcinomas, according to nuclear grade. The results obtained are summarized as followings, 1) Immunohistochemical grade of Bcl-2 showed a significant inverse correlation with nuclear grade. 2) Immunohistochemical grades of ER and PR also showed a significant inverse correlation with nuclear grade, and were well correlated with each other. 3) Immunohistochemical grades of Bcl-2 and hormone receptors showed a strongly significant correlation. On the basis of the above results, we suggest that Bcl-2 expression may be under hormone dependent control and that it can be used in prognosis and choice of hormonal therapy in the presence of hormone receptor.

7.The size comparison of the diameter of anterior segmental bronchus and that of anterior segmental artery.

It is thought to be reliable roentgenologic sign of pulmonary plethora that the diameter of anterior segmental artery(d-ASA) is larger than that of anterior segmental bronchus(d-ASB). To evaluate the reliability of this sign, we compared d-ASA with d-ASB on chest rentgenograms of 100 normal adults. Of the total 105 cases. d-ASA was larger than d-ASB in 40 cases (38.1%), smaller than d-ASB in 34 cases(32.4%) and equal to (d-ASA and) d-ASB in 31 cases(29.5%). The ratio(ABR) and difference between d-ASA and d-ASB ranged from 0.70 to 1.58(mean, 1.05±0.30) and from -1.85mm to +2.45mm(mean of absolute value, 0.75mm±0.57). These results showed that the relationships between d-ASA and d-ASB were variable. Conclusively, d-ASA may be larger than d-ASB in even normal adults and mild degree of pulmonary plethora cannot exectly evaluated with this findings only.

8.Radiological Findings of Chronic Granulomatous Disease of Childhood.

PURPOSE: Chronic granulomatous disease(CGD) is a group of genetic disorders characterized by recurrent pyogenic infections of the respiratory tract, skin,and soft tissue. The aim of this study is to describe the radiological findings of CGD. MATERIALS AND METHODS: We retrospectively analyzed radiological findings of 11 patients of CGD, which were diagnosed by nitroblue tetrazolium test. We analyzed the pattern of pneumonia on chest radiograph in all infants. Three cases of chest CT and one case of digital subtraction angiography were performed. According to infant's symptom, abdominal ultrasonography(n=8), abdominal CT(n=5), simple bone radiography(n=2), and brain CT(n=I) were performed. RESULTS: Repeated infiltration(100%), mass-like consolidation(73%), hilar or mediastinal lymph node enlargement(64%), scattered nodules(55%), cavity formation(27%), and pleural effusion(27%) were found on the chest radiographs(n=11) and CT(n=3). On the abdominal imagings(n=8), there were hepatosplenomegaly(n=6), calcifications in the liver(n=2) and kidney(n=1), hepatic granuloma(n=1) and renal abscess(n=1). Two patients had osteomyelitis. There were findings of meningitis on brain CT(n=1). CONCLUSION: We believe that the combinations of these radiological findings may suggest the diagnosis of CGD in patients with history of reccurent infection.

9.Biohazard surveillance of allergic contact dermatitis in genetically-modified Zoysia grasses using patch testing.

PURPOSE: Genetic modification technique is widely used in plants for improving crop yields and qualities, and reducing pesticide use. Zoysia japonica is one of the widely planted grasses for lawning. Pollenless herbicide-tolerant genetically modified (GM) grasses (JG21-MS1 and JG21-MS2) were developed under surveillance for possible biohazard. Grasses may cause allergic contact dermatitis with direct contact on human skin. Patch testing with allergens was adopted to compare the incidences of allergic contact dermatitis to GM grass leaves with those of wild-type grass. METHODS: Patch testing with controls and leaves of wild-type and GM grasses was performed in individuals with informed consent. RESULTS: Ninety-seven individuals (mean age, 32+/-8 years; 48% males) were enrolled. For nickel sulfate as a positive control, 38 subjects (39.2%) were positive. For GM grasses, 10 (10.3%) showed positive patch test results and 11 (11.3%), for wild-type grass, the proportions of which were similar among the tested grasses. CONCLUSION: JG21-MS1 and JG21-MS2 are not more biohazardous than wild-type grass in the risk of allergic contact dermatitis.

10.Cauda Equina Syndrome Following Epidural Anesthesia: A case report.

Severe neurologic complication after epidural anesthesia is very rare. We experinced a case of cauda equina syndrome following epidural anesthesia with 2% lidocaine 20 ml, alkalinized with sodium bicarbonate and 1: 200,000 epinephrine added in female patient underwent abdominal hysterectomy. She complained hypoesthesia of buttock, perineum, left leg and weakness of left leg. She also suffered from urinary retension and defecation difficulty and abdominal discomfort 2 days after epidural block. In MRI taken 4 days after block, spinal stenosis (L4-5) and mild disc bulging (L3-4) were noted. She was treated with medication, bladder training and physical therapy and recovered slowly but gradually, finally discharged 2 months after block with mild weakeness of left leg. We suggest the causative factors are the temporary neural compression due to spinal stenosis and the spinal cord ischemia due to vascular spasm because of added epinephrine.
