2.Leiomyoma of the Skin: clinicopathological study of 19 cases.

Nineteen cases of leiomyoma of the skin were examined clinicopathologically. This group included 12 cases of angioleiomyoma, 5 cases of solitary piloleiomyoma, and 2 cases of multiple piloleiomyomas. 1) All twelve angioleiomyomas occured as solitary lesion in the extremities. There was a preponderance in females with a ratio of 2:1. The ages of patients ranged from 24 to 80 years and only one was below the age of 30 years. Six tumors were either painful or tender. Nine tumors in subcutaneous fat were shelled out at surgery. All tumors did not exceed 4cm in diameter. Histologically they could be separated into ten cases of the solid type and two cases of venous type according to Morimoto's classification. Although actin or desmin was easily detected in all tumors, the diagnosis was better made using a combination of hematoxylin-eosin and Masson trichrome stains. 2) Five cases of solitary piloleiomyoma were slow-growing intradermal nodules. The ages of patients ranged from 10 to 77 years. All five cases were female. The lesions were located on the extremities, back and sholuder. Pain or tenderness was present in 3 cases among these tumors. Histologically, all tumors were characterized by subtle poorly circumscribed proliferation of benign smooth muscle in the dermis. 3) Two multiple piloleiomyomas from two female patients, aged 50 and 40 years, were situated on the shoulder and thigh, respectively. Pain was induced by change of temperature in the shoulder lesion. Histologically they were identical to the solitary piloleiomyoma.

5.Multiple neurotization in Preganglionic Whole Arm Type of Brachial plexus injury

In the whole arm type of brachial plexus injury, the nerve grafting method give the best result. As it is impossible, however to operate the preganglionic whole arm type by interfascicular nerve graft, the treatment of this type of injury is difficult. In this lesion, neurotization may be the only useful method. To evaluate it's efficacy, 38 cases of multiple neurotization have been reviewed. The follow up period was on average 45 months(24 months to 76 months). The results are as follows: 1. Motor or sensory improvement of good or better results was observed in 27 nerves(29%) and that of better-than-fair results in 54 nerves(57%). 2. The best results were obtained in patients less than thirty year old and in the patients where the operation was performed within the first six months after injury(19 cases, 42%). 3. The use of spinal accessory, phrenic, intercostal and supraclavicular nerves, as a source of neurotization produced similar results. Phrenic nerve neurotization was performed without any significant respiratory difficulty. 4. The results of neurotization were analysed by Kim's method. Functional recovery of the upper extremity showed relatively poor results. 5. Out of the 25 patients who had developed a painful syndrome before neurotization, 19 cases(60%) showed improvement of the symptom. Multiple neurotization in preganglionic whole arm type of brachial plexus injury is of a little value in improving upper extremity function. Nerve neurotization can not always make a paralysed upper limb useful, because it is impossible to control the digits and intrinsic muscles in the hand and to restorate fine sensation by it. Nevertheless, functional recovery of the paralyzed upper limb, compared with no previous muscle contraction, encourages patients who have suffered serious brachial plexus injuries to start an active their life again.

6.Metformin Preserves Peripheral Nerve Damage with Comparable Effects to Alpha Lipoic Acid in Streptozotocin/High-Fat Diet Induced Diabetic Rats

Metformin is widely marketed medication for the treatment of diabetes, but its pharmacological effect on diabetic peripheral neuropathy remains unclear. In this study, the effect of metformin on peripheral nerves in diabetic rats was investigated using diverse neuronal parameters of nerve fibers. Rats were assigned to one of four groups ( Both DM+metformin and DM+ALA groups showed similar trends to diverse sensory tests at 24 weeks compared to DM group although the degree of change were different according to the stimulated senses. There was no significant difference in the comparison of the intraepidermal nerve fiber density (IENFD) of peripheral nerves between the DM+metformin and DM+ALA groups (11.83±0.07 fibers/mm vs. 12.37±1.82 fibers/mm, respectively). Both groups showed preserved IENFD significantly compared with DM group (8.46±1.98 fibers/mm, Metformin has beneficial pharmacological effects on the preservation of peripheral nerves in diabetic rats and its effects are comparable to those of ALA.
7.Flow Cytometric DNA Analysis in Thyroid Neoplasms: With Emphasis on the Correlation between Ploidy Level and Pathologic Features.

Nuclear DNA content in 31 cases of thyroid neoplasm was determined by flow cytometry with the use of paraffin-embedded archival tissue. DNA aneuploidy was found in 6 cases (19.4%) of the 31 thyroid neoplasms; such as 2 of 8 (DI=1.16, 1.56) follicular adenomas, 1 of 6 (DI=1.10) follicular carcinomas, 1 of 15 (DI=1.18) papillary carcinomas and 2 of 2 (DI=1.76, 2.07) medullary carcinomas. The remaining tumors were diploid. No significant difference between follicular adenomas and carcinomas was detected with respect to the S phase fraction(SPF). In the papillary carcinoma group the SPF was higher than in the follicular neoplasm group, but it was statistically insignificant. Regional lymph node metastasis was present in 8 of 15( 53.3%) papillary carcinomas but absent in all of the 14 follicular neoplasms. In the medullary carcinoma group one case showed regional node metastasis at the time of resection and the other developed metastasis 11 months after surgical removal of the primary lesion. In this study tumors predominantly composed of Hurthle cells were found to have a significantly higher D.I. than those with few or no Hurthle cells. No significant difference was found between tumors with metastasis and those without metastasis.

9.The usefulness of thin-section rapid infusion CT for detection of cerebral aneurysm.

With 2mm-thick CT scanning during the rapid infusion of contrast material(TICT), cerebral aneurysms arising from the circle of Willis and adjacent vessels can be directly visualized. Twenty-five patients who had cerebral aneurysm confirmed by surgery were examined with TIGT and digital subtraction angiography. The authors examined TIGT prospectively to assess the detection rate of the cerebal aneurysms and to evaluate the clinical usefulness of TIGT. The detection rates of aneurysms by TIGT and digital subtraction angiography were 68% and 84%, respectively. TIGT is a rapid, safe and reliable method in the evaluation of patients with suspected cerebral aneurysm, permitting direct visualization of the aneurysm.
