2.Influence of Awareness and Perceived Ease in Using Parental Leave on Job Satisfaction of Nurses in a Public Hospital

PURPOSE: The aim of this study was to identify whether awareness and perceived ease in using parental leave influence job satisfaction of nurses in a public hospital. METHODS: The participants were 425 nurses working in a public hospital. A questionnaire was used to collect data from October 12 to October 25, 2016. The influence of the factors on job satisfaction of nurses was analyzed by multiple hierarchical regression. RESULTS: Nurses taking leave accounted for 46.8% (75 nurses) of 160 married nurses. They worried about adaptation after return to work (30.7%) and complained about disadvantages related to using parental leave (45.3%). The percentage of nurses on leave replaced by new full time nurses was 70.7%. Nurses' awareness (β=.16) of perceived ease of using parental leave (β=.32) influenced the nurses' job satisfaction (R2=.21). CONCLUSION: To improve job satisfaction for nurses, it is necessary to increase awareness and perceived ease of using parental leave. Public hospitals should replace nurses on leave with temporary nurses and help returning nurses adapt to their jobs as they return to work. Nursing managers should create a healthy climate in which it is easy for nurses to take parental leave.

4.The Property of p53 Gene in Cell Lines of Squamous Cell Carcinoma.

The mutation of p53 tumor suppressor gene is the most common genetic variation of primary malignant tumors. The occurrence, progression and reaction for medical management of cancers can be different according to the characteristics of p53 gene, even if they are same kinds of malignant tumors. In this study, the property of p53 gene in 4 kinds of squamous cell carcinoma cell lines were investigated by using immunocytochemistry, PCR-SSCP, sequencing and Western blot methods. As a result, p53 mutation detected in 3 kinds of squamous cell carcinoma cell lines. Namely, it is found that T in codon 176 changed to A, and G in codon 281 changed to A in KUMA3 cell lines; CC in codon 241 changed to TT in KUMA4 cell lines; G in codon 266 changed to T in KUMA6 cell lines. In single nucleotide polymorphism of codon 72 of p53 gene, the genetic variations are Arg/Pro heterozygote in KUMA3 and KUMA4 cell lines; Arg/Arg homozygote in KUMA5 cell lines; Pro/Pro homozygote in KUMA6 cell lines. These results will provide useful data for p53 gene researches of various squamous cell carcinomas.

6.A Clinical Study on the Hypotensive Effect of Nilvadipine in Patients with Essential Hypertension.

BACKGROUND: As an antihypertensive drug, Nifedipine, a calcium channel blocker was introduced recently, which also has antianginal effect. But due to the relatively short duration of action, another antihypertensive agents having longer duration of action and stronger hypertensive effect were under investigation. Nilvadipine, a new calcium channel blocker, was introduced to have more prologned duration of action and to act more specifically on vascular smooth muscle. So the efficacy and safety of oral Nilvadipine on essestial hypertension was investigated and represented by our institute. METHODS: In order to investigate the efficacy and safety of oral Nilvadipine, daily doses of 4mg twice a day were administered in 30 hypertensive patients whose states were compatible to the criteria : 1) severity of hypertension rated in Stage I and Stage II according to the classification by WHO, 2) ages ranging from 30 to 74 years regardless of sex, 3) blood pressure with 95mmHg or higher but less than 115mmHg in diastolic pressure which was the mean in a sitting position at the last two out of not less three consultations in the 2 week observation period, 4) outpatients with informed consent for 6 weeks. Blood pressure and heart rate were measured every 2 weeks. The complete with blood count with platelet, uronalysis and the electrocardiography were performed at the beginning period and the 6th weeks of therapy. And kinds of side effects were questioned by examining physicians. RESULTS: The following results were obtained : 1) Blood pressure fell significantly in 6 weeks of treatment with Nilvadipine(Mean pressure+/-S.D., 6.00mmHg vs 108.90+/-9.68mmHg p<0.05), 2) There was no significant change in EKG in 6 weeks of treatment with Nilvadipine, 3) Pulse rate was decreased in 6 weeks of treatment with Nilvadipine(80.14+/-11.90/min vs 75.39+/-6.47/min, p<0.05). 4) No significant chsange in body weight was observed(64.50+/-8.7kg vs 63.50+/-10.25kg, p<0.05). 5) There were no significant changes in blood chemistry including blood sugar, cholesterol, electrolytes, serum creatinine and alkaline phosphatase values, 6) Hematologic findings and urinalysis findings reamained unchanged, 7) Total 10 patients(33.30%) had various side effects;facial flushing 30.00%, palpitation 23.33%, headache 20.00%, nausea 10.00%, drowsiness 3.33%, heaviness 3.33% and indigestion 3.33%. But there was no serious side effect that requires to discontinue the medication of the test drug. And there was no need to reduce the dosage due to the side effect, 8) The antihypertensive effect was judged to decrease markedly in 76.70%, decrease 20.00%, unchange 3.30% and increase 0.00%, 9) The utility which was assessed with the data from the overall safety and antihypertensive effect, the drug was judged to be very useful in 60.00%, useful 33.30%, useless 6.67% and inhibited 0.00%. CONCLUSION: From the above results, Nilvadipine in doses of 4mg twice a day was effective and useful in most cases without severe side effects in essential hypertensive patients with diastolic blood pressure of 95 to 115mmHg.

7.Maternal Perception of Children's Weight, Maternal Body Shape Satisfaction, and Maternal Feeding Styles in Preschool-Aged Children.

PURPOSE: The purpose of this study was to explore the correlates of maternal perception (identification and satisfaction) of children's weight, maternal body shape satisfaction, and maternal feeding styles in Korean preschool-aged children. METHODS: A cross-sectional survey design was utilized. Participants consisted of 287 pairs of preschool-aged children (3-6 years) and their mothers. Data were analyzed by chi2-test, Fisher's exact test, ANOVA, and the Pearson correlation coefficient. RESULTS: Among the 287 mothers, 47.7% underestimated their children's weight, while 15.7% overestimated it. 46.7% of the mothers wished their children weighed more, while 11.1% of them wished their children weighed less. The mean score of maternal body shape satisfaction was 83.75+/-28.77. The mean score of parent-centered feeding styles was 2.95+/-0.54, and the mean score of child-centered feeding styles was 3.33+/-0.42. There were statistically significant correlations between maternal identification of children's weight and children's body mass index (BMI) (r=-.366, p<.001). In addition, there were statistically significant correlations between maternal satisfaction of children's weight and children's BMI (r=-.484, p<.001), maternal BMI (r=-.126, p=.033), and maternal body shape satisfaction (r=-.127, p=.031). CONCLUSION: The results of the study suggest that intervention programs for mothers to develop more accurate perception of their child's weight should be established.

8.Extradural-Intradural Approach to Carotid-Ophthalmic Artery Aneurysm.

The authors report seven cases of carotid-ophthalmic artery aneurysms who were surgically treated using the combined extradural-intradural or intradural approach from March, 1991, to December, 1993. Carotid-ophthalmic artery aneurysms are of considerably surgical interest because some of these were considered to have technical difficulties of clipping and disastrous surgical results. Removal of the anterior clinoid process and unroofing of the optic canal are the key points to exposure the operative field adequately. On the basis of these experiences, authors concluded that this approach is very useful, because of adequate surgical field and better mobilization of the internal carotid artery and optic nerve for direct clipping of carotid-ophthalmic artery aneurysm and basilar tip aneurysm.

9.Topographic Difference of Inflammatory Reactions in Gastric Mucosa in Various Helicobacter pylori-Associated Diseases.

Gastric biopsy specimens from 140 patients (66 chronic gastritis, 33 gastric ulcers, 26 duodenal ulcers, 15 gastric cancers) were examined to investigate the topographic difference of inflammation, glandular atrophy, intestinal metaplasia, and Helicobacter pylori (H. pylori) colonization by the updated Sydney system. Density of H. pylori of the antrum was significantly higher in duodenal ulcers than in chronic gastritis, gastric ulcers, and gastric cancers. Inflammation of duodenal ulcers was predominantly antral and glandular atrophy and intestinal metaplasia of duodenal ulcer were significantly less than those of gastric ulcers and gastric cancers. Chronic inflammation of gastric ulcers and gastric cancers was higher in antrum than in corpus. Increasing atrophy of the antrum was associated with decreasing density of H. pylori of antrum itself, but increasing colonization of the corpus. This study reveals the inflammatory reactions of gastric mucosa differ in chronic gastritis, gastric ulcers, gastric cancers, and duodenal ulcers and suggests that antral atrophy fosters the colonization of oxyntic mucosa by H. pylori.

10.The effects of nebulized salbutamol on serum potassium concentration in bronchial asthmatics.

BACKGROUND AND OBJECTIVE: The knowledge about the effects of the nebulized B2-agonist on serum potassium is limited. We aimed to assess the possible hypokalemia following nebulization of salbutamol. METHOD: Seven patients(mean age 60 +- 7.1years) with acute exacerbated asthma were treated with salbutamol nebulization(5mg nebulization at 1 hour interval, 3 times) without concomitant use of steroid or other bronchodilator such as theophylline. RESULTS: There was a significant increase in FEV1, from 46.41+-25.91% at baseline to 62.86+-22.38% at 3 hours after treatment. Serum potassium concentration was significantly decreased, from 3.93+-0.58mEq/L at baseline to 3.41+-0.62mEq/L and 3.46+-0.53mEq/L at 1 hour and 3 hours after third nebulization, repectively. There was a significant prolongation of the QTc interval in EKG from 454.36+-27.07msec at baseline to 479.41+-35.64msec and 505.09+-58. 69msec at 1 hour and 3 hours after third nebulization, respectively. Serum salbutamol concentration was 4.18+-3.39ng/ml at baseline, and increased to 7.69+-6.94ng/ml and 9.84+10.34ng/ ml at 1 hour and 3 hours after treatment, respectively. Magnitude of the hypokalemia and the degree of prolongation of the electrocardiographic QTc interval were significantly correlated with the level of serum salbutamol concenturation. CONCLUSION: The results suggest that cardiac complication could develop due to hypokalemia during repeated salbutamol nebulization. Caution should be done in monitoring of serum potassium concentration when using nebulized salbutamol repeatedly for the treatment of acute exacerbated bronchial asthma.
