1.Giant cell tumor in long bones: The significance of marginal sclerosis for the differential diagnosis.

Plain radrographs of thirty nine patients with giant cell tumor of long bone and CT scans of twenty patients among the thirty patients were reviewed retrospectively to evaluate the frequency and significance of sclerosis of the tumor margin. The sclerosis of the tumor margin was observed on plain radiographs in thirteen patients(33.3%) and they were located either on epiphyseal or on both epiphyseal or metaphyseal portion of the tumor. The authors concluded that the giant cell tumor should not be excluded from the differential entities eventhough the tumor has the marginal sclerosis.

2.Two Cases of Extramammary Paget' s Disease Showing a Good Response to Radiotherapy.

We report the clinical evolution of two elderly male patients with extramammary Paget's disease who were treated with radiotherapy. Both cases primarily involved the scrotum and penile root, and were not associated with any underlying or visceral malignancy. The total doses administered were 6000 cGy and 3000 cGy, respectively. A good clinical response was achieved in both cases. No major side effects were observed. The patients are being followed for any signs of recurrence and have been completely asymptomatic to date

4.A case of annular elastolytic giant cell granuloma.

Annular elastolytic giant cell granuloma(AEGC(3) has been called as atypical necrcibiosis lipoidica, Mieschers granuloma of the face and scalp, and actinic granu- Joma and needs differen(iation from granuloma annulare, elastosis perforans serpiginosa and nerrobiosis lipoidica. We report a case of AECCG occurring in a 53-year-old male. The patient. had an annular erythematous plaque measuring 4 cm in diameter with serpiginous raised edges and slightly atrophic centers on the right forearm for 5 years. The histopathologic finding showed patchy dermal infiltration of lymphocytes, histiocytes, and multinucleated giant cells engulfing elastic fibers. The. skin lesion cleared after intrader mal injection cf cortieosteroids.

5.MR Imaging of the Trachea:In Vivo and In Vitro Study.

PURPOSE: To establish signal intensity characteristics of the trachea according to the histologic layers, we performed in vivo and in vitro MR studies. MATERIALS AND METHODS: We performed MR imaging of the trachea at 1.5T unit in 11 patients mediastinal masses, vascular anomalies, tracheal stenosis or iatrogenic tracheoesophageal fistula, aryepiglottic fold thickening or mass, tracheal carcinoid, one healthy volunteer and one cadaveric trachea. By using anterior, volume neck or 3 inch dual coil with various pulse sequences, axial and coronal images of the trachea were obtained. The tracheal layers with different signal intensity on MR images were correlated with the histology. RESULTS: In vivo and in vitro MR studies revealed two layers of the trachea ;the inner layer had intermediate to high signal and the outer had low signal. The tracheal cartilage showed low signal intensity in all pulse sequences. The submucosa appeared as intermediate signal intensity on T1 weighted images but high signal intersity on other images due to its abundant mucous and mucoserous glands. However, the mucosa and perichondrium could not be defined on MR images. CONCLUSION: Characterization of the signal intensity according to the histologic layers of the trachea might be helpful for the evaluation of intrinsic lesions of the irachea or the possibility of tracheal invasion from the adjacent tumors.

7.Reconstruction and Optic Never Decompression Following the Removal of Fibrous Dysplasia in the Orbit and Cranial Base.

Fibrous dysplasia in the orbit and cranial base may involve the optic canal. Although fibrous dysplasia is benign, it may produce a mass effect along the course of the optic nerve which can then induce visual disturbance as well as contour deformities of the skull and facial bone. The treatment of fibrous dysplasia in the orbit and cranial base is to resect the lesion as much as possible and then reconstruct immediately. As well, if there is any evidence of optic canal involvement and disease progression, the treatment of fibrous dysplasia may include optic nerve decompression. It is generally understood that some patients experience improvement of visual function after optic nerve decompression. We performed radical excision and reconstruction by means of autogenous calvarial bone graft and methylmethacrylate in 7 cases. The autogenous calvarial bone was used to reconstruct the orbit. The methylmethacrylate was used to reconstruct bony defect in the temporal area. The orbit was reconstruced into one block which was made of autogenous calvarial bone with a microplate and screw. This method is superior compared to the previous multifragment wiring method with regard to stability, operation time, and appearance. The patients in our series showed satisfactory appearance. In 6 cases, we performed optic nerve decompression. Therapeutic optic nerve decompression was done in 3 cases and prophylatic optic nerve decompression was done in the others. Following therapeutic optic nerve decompression, visual acuity was improved in 2 cases while the others showed a decrease in visual acuity. There was no change of visual acuity and visual field in 1 case after prophylactic optic nerve decompression. However, the others showed decrements in visual acuity or visual field. Therefore, we believe that more attention should be paid during optic nerve decompression procedure and strict indications to that procedure should be applied.

8.Operative Treatment of the Carpal Scaphoid Nonunions.

We had treated 24 patients who had nonunions of the carpal scaphoid from June 1987 to September 1995. 17 of 24 nonunions of carpal scaphoid were treated by K-wires fixation and autogenous iliac bone grafts. Another 7 cases were treated by Herbert screw fixation and autogenous iliac bone graft. 24 patients were followed up average 42.6 months and obtained following results; l. Among 24 cases, 23 cases were men and 19 cases were the third and fourth decade. Most common injury was the falling accidents (11 cases: 45.8%). 2. Most common pattern of fractures was waist type (20cases: 83.3%) in anatomical site and transverse type (15 cases: 75.0%) in pattern of fracture line. 3. Preoperative scapholunate angles (mean 52.9degrees: K-wires, 69.1degrees: Herbert screw) has been improved after open reduction and internal fixation by K-wires or Herbert screw (mean 46.2degrees: K-wires, 51.1degrees: Herbert screw). 6 cases had DISI deformity preoperatively with scapholunate angles over 70degrees (mean 80.0degrees: K-wires, 84.0degrees: Herbert screw) has been corrected after ORIF (mean 48.0: K-wires, 58.0: Herbert screw). 4. Bony unions were obtained in average 13.2 weeks in K-wires group and 9.8 weeks in Herbert screw group. 5. 13 cases (76.5%) in K-wires group and 6 cases (85.7%) in Herbert screw group were excellent and good result by Maudsley method. 6. The complications and sequalaes were present 4 cases in K-wires group with degenerative arthritis on radioscaphoidal and scapholunate joint and collapse of proximal pole and 1 case in Herbert screw group with screw malposition. So, We were thought that ORIF with K-wires and Herbert screw was effective treatment for nonunions of carpal scaphoid, especially Herbert screw fixation and autogenous iliac bone graft. In comparison of bony union and results of treatment, Herbert screw group was more effective than K- wires group.

10.Effect of Persistent Mild Hyperglycemic Hyperinsulinemia on Development of Insulin Resistance in Rats.

The effect of persistant mild hyperglycemic hyperinsulinemia on the development of the insulin resistance in rats was studied in vivo. Also, the characteristics of the insulin resistance compared with the insulin resistance of STZ diabetic rats. Persistant mild hyperglycemic hyperinsulinemic rat model was produced by ingestion of glucose polymer for 8 days. The glucose disappearance and infusion rate was measured by hyperinsulinemic euglycemic clamp to"Imique at steady state of blood glucose and insulin levels. The clamped level of blood glucose was 100 mg/dl, and the clamped levels of insulin were 70 pU/ml (physiologic condition) and 3000 pU/ml (supramaximal condition). Hepatic glucose producticon rate was calculated using measured data. And the glycogen synthetic capacity of skeletal muscle(soleus) and liver was measured after 2 hours of hyperinsulinemic euglycemic clamp study. The glucose disappearance and glucose infusion rate in glucose polymer group was decreased in the both physiological and supramaximal insulin level compared to the rate of the normal control group. The rate of STZ diabetic group wase lowest at supramaximal insulin level among two another experimental groups. The hepatic glucose production rate of glucose polymer group was decreased compared to normal control but increased in STZ diabetic group.
