1.The Simplifed Hair Baiting Technique and its Practical Application.

In order to avoid contamination attributed to Vanbreuseghem hair haiting technique in the laboratory, the author contrived the following modified method. After adding 100ml of antibiotics solution (mixture of 500mg of Cyclohexamide, 20, 000u of Penicillin and 40mg of Streptomycin in 1,000ml of distilled water) in PVC phial containing 100gm of soil, it was gently agitated for an hour and lefted to be settled for 12 hours. The suspended solution obtained and a batch of horse hair were randomly distributed on a Petri dishfitted with blue cloths, then cultured at 24C'. The overall results obtained from this modified method were not as satisfying as that of the Vanbreuseghem method but a few advantages such as simplicity of the procedure and significantly lower laboratory contamination implicated its practicality. Furthermore, this method was also found to be useful in the first isolation of infected animal hairs, hair perforation tests and as a substitution for artificial media. For the cross mating, a patch of blue cloths was laid on top cf moist sterilized soil evenly spreaded on a Petri dish. Then hair-baiting procedure was performed and good results were obtained.

2.Two Cases of Onychomycosis due to Aspergillus repens.

Onychomycosis caused by the genus aspergillus is extremely rare in Korea and only one case due to A. sydowi has been reported. We experienced two casea of onychomycosis infected with A. repens, which were healthy adults. The first case was a 29 year-old male who developed thickening and dark discoloratian of 1st, and, 3rd and 5th toe-nails of left foot from about 5 months on. The second case was a 27 year-old male who developed thickening and yellow-whitish discoloration of the nail of right thumb from about 3 rnonths on. In both cases, direct examination of the affected nails showed many thick bluish hyphae and spore chains, and A. repens was isolated. Our cases were first reported through the Korean literature.

3.Bronchial artery embolization: clinical analysis of 129 cases.

Bronchial artery embolization is well-accepted and widely used for management of massive and recurrent hemoptysis. This may either provide a definite therapeutic measure or a stabilizing effect on the patents in preparation for surgery. Retrospectively we reviewed 129 cases(106 patients) of bronchial artery embolization with Gelfoam pudding & Ivalon for control of hemoptysis from July 1985 to january 1991. The causes of hemoptysis were pulmonary tuberculosis(80.2%). Bronchiectasis(11.3%), asperigilloma(2.8%), and others(5.7%). The cases of pulmonary tuberculosis included tuberculous bronchiectasis (40.0%), active(34.1%), undetermined(14.1%) and inactive(11.8%). @ES The results were as follows: @EN Immediate control of hemoptysis was achieved in 104 of 122 cases(85.2%). Immediate control of massive hemoptysis was achieved in 94 of 107 cases(87.6%) and of chronic intermittent hemoptysis in 10 of 15 cases(76.0%). Hemoptysis recurred in 39 of 90 follow up cases(43.3%) on follow-up studies performed ranging in period from 2 to 49 month after the initial studies. Thirty three of 81 cases of massive hemoptysis recurred(40.7%) and six of 9 cases of chronic intermittent hemoptysis recurred(67.0%). One years rebleeding rate of massive hemoptysis was 34.6%. The rebleeding cases of massive hemoptysis were controlled by conservative treatment in 25 of 33 cases(75.8%). In conclusion. Bronchial artery embolization for hemoptysis control is effective in massive hemoptysis, but nearly ineffective in chronic intermittent hemoptysis, The goal of bronchial artery embolization is lifesaving procedure without permanent effect. Especially hemoptysis related to pulmonary tuberculosis.

4.Comparison of CT findings with upper GI series and surgical findings

The authors made comparative studies of CT findings with upper GI series and surgical findings in 20 cases of histologically proven gastric cancers. The identification of thickening of gastric wall and local and remotemetastasis by CT was thought to be great value in treatment planning and prediction of prognosis of the patients. The results were as follows; 1. Of 20 cases, 19 cases demonstrated thickening of gastric wall. Among 3 cases suggested as early cancer on upper GI series, 2 cases demonstrated thickened gastric wall on CT and it was provedto be advanced cancer at surgery. 2. Out of 8 cases showed no definite metastasis on CT, 4 cases had malignant in filtration histologically in the regional lymph nodes. But they were less than 1.5 cm in diameter. 3. The frequency order of remote metastasis found by CT was pancreas, liver, left adrenal gland and lung in organs and retroperitoneal, regional nodes in lymph nodes.

5.Assessment of Genotoxic Hazard in Petrochemical Workers.

In order to evaluate the genotoxic hazard among workers potentially exposed to low level petrochemical substances, the analyses of micronuclei (MN) and sister chromatid exchanges (SCEs) in lymphocytes were performed in 46 male workers (as exposed group) and 46 nonexposed subjects (as control group). Mean frequencies of MN and SCEs (respectively, 12.9/1000 cells and 6.5/cell) in exposed group were very significantly higher than those (10.2/1000 cells and 5.4/cell) in control group. And there were also significant differences in mean frequencies of MN and SCEs adjusted for age, employment duration, smoking, and drinking between two groups. Median frequencies of MN and SCEs in exposed group were very significantly higher than those in control group. Frequencies of SCEs were higher in smokers than in non-smoker. Frequencies of MN in smokers, however, were similiar to those of non-smoker. Interaction between exposure and smoking on MN and SCEs induction was not observed. The results suggest that there is genotoxic hazard in high risk group like workers handling carcinogens in petrochemical plants and the analyses of MN and SCEs are useful biomarkers for the exposure to hazard substances even at the level below the exposure limit.

6.Teleradiology(TELEACE) system: Results of field trial.

We report the results of field operation of TELEACE system between Kyung-Pook National University Hospital and Ul-Jin Goon Health Care Medical Center from December, 1990 to September, 1991, which had been operated as a kind of Integrated Services Digital Nework projects by KOREA TELECOMMUNICATION Inc. Ul-Jin Goon Health Care Medical Center transmitted 414 plain radiographs to our hospital in speed of 9600BPS. Each image was composed of 1024X1024 pixelsX8 bits/pixel. In our hospital, the image files were displayed on high resolution monitor (1280×1024 pixets). Text files of image interpretations were transmitted to the health care medical center. The two radiologists who had interpreted the transmitted images, went to the health care medical center and read radiographic film with blind test method. We obtained the following results: false negative rate of 6.3%, false postitve rate of 2.4%, mean sensitivity of 81.4%, mean specificity of 96.3%, and mean accuracy of 91.3%. In predictive value of 0.05, there was no significant difference between results of these two types of radiographs. In conclusion, TELEACE system was valuable to the clinicians isolated from services of radiologists.

7.Study on Korean Daily Walking Activity <1>.

Since there is no report of daily walking activity of Korean, authors studied on normal adult daliy walking activity in korean of various kinds of occupation, personal life and patients. Total 134 cases were studied and in which there were 10 patients, 7 retired elderlies, 4 house wives, 48 employees of compaines, 30 nurses, 4 laboratory technicians, 2 pharmacists, 7 house staff doctors, 12 chief doctors, 4 cases of hicking and 6 cases of 18 holes golf players. The daily walking steps were measured with pedometer in 167 times in total and following results were, obtained. 1. Bed rest patients ; less than 1000 steps/day 2. 8.Long-term Follow-up of Metaphyseal Sclerotic Lines in Children Treated with Pamidronate.

PURPOSE: Bisphosphonates are widely used for the management steroid-induced osteoporosis (SIO) in children. With the increasing use of bisphosphonates, there have been reports of abnormal radiological findings in the growing skeleton. Therefore, their use in pediatric patients remains controversial. The present study was conducted to evaluate the long-term follow-up radiographic features, particularly metaphyseal sclerotic lines, in children who receive pamidronate therapy for nephropathy. METHODS: Twenty-four children with nephropathy treated with oral calcium and pamidronate (mean duration, 9 months; dose, 100 mg daily), were evaluated retrospectively. All patients had SIO secondary to chronic glucocorticoid therapy for treating nephropathy. Long bone radiographic imaging was performed before treatment with pamidronate, and at follow-up, several years later. Physeal growth rates were estimated by measuring the distance that the sclerotic lines moved on the radiographs during the corresponding time intervals. RESULTS: The mean follow-up period was 138 months. Long bone radiographs showed well-defined sclerotic lines at the metaphyseal ends, progressively moving from the physeal plate to the diaphysis, in all patients. The mean rate of movement of the sclerotic line was 6.21 mm per year. In 12 patients, the lines disappeared. The mean rate of growth in height was 7.33 cm per year. CONCLUSIONS: Results of long-term follow-up suggest that the metaphyseal sclerotic lines associated with pamidronate treatment tend to disappear without affecting overall skeletal growth. Bisphosphonate treatment for SIO in children with nephropathy seems to be safe, although further studies in larger number of patients are needed.

9.Treatment of Cardiac Neurosis with Propranolol (Inderal(R)).

Thirty-seven cases of cardiac neurosis were treated with propranolol (Inderal(R)) and following results were obtained. 1) Symptoms, such as palpitation, dyspnea, chest tightness and chest pain were disappeared or improved in all cases. 2) Heart rate reduced significantly after treatment from 99.8 per minutes to 74.8 per minutes. 3) Systolic blood pressure reduced significantly after treatment from 147.6 mmHg to 131.4 mmHg. but diastolic blood pressure were not reduced significantly. These results indicated that propranolol is recommendable medicine for improving cardiac symptoms and tachycardia in cardiac neurosis.

10.A Perception of Dietitians for Using Imported Foods and Pro-environment Farm Products for Elementary School Foodservice Operations in Busan.

The purpose of this study was to assess the use of food materials at elementary school in Busan area and provide school meal management with basic data to improvement of guideline. A survey was done by e-mail with a set of questionnaires, which was responeded by 122 dietitians in elementary school foodservice. The results were as follows; Sixty percent of the dietitians though to the general imported food materials had to use. Regarding mean score of using the imported food materials; safety was 2.12/5.00, freshness 2.48/5.00, sanitation 2.68/5.00, nutrition 2.73/5.00, preference 2.93/5.00, external form 3.09/5.00, package 3.12/5.00, and price 3.63/5.00. Thirty seven percent of the dietitians had purchase the pro-environment farm products(PEFPs) and 32.0% of them purchased those products for the students' health. But 52.4% could not purchase to the PEFPs because of expensive price. They had negative opinions about deficient supplying of PEFPs 4.51/5.00, too expensive price 4.34/5.00, and the truth or false in source of PEFPs 3.96/5.00. Most of them perceived that using the PEFPs had to apply the step by step. Among of them, 33.9% had the knowledge of PEFPs, but 16.4 of them did not know to the PEPPs. 57.4% of them hoped to purchase the food materials through a center of supporting school meal servcie(it called non-profit-making organizations).
