1.Comparison of Bone Mineral Density and Lipid Profiles in Pre and Postmenopausal Women.

BACKGROUND: Estrogen deficiency accelerates loss of bone mass and changes lipid profile in the postmenopausal women, so that the osteoporosis and astherosclerosis were developed. But it has not enough studies including the premenopausal women. So we have investigated about the differences of body mass index(BMI), lipid profile and bone mineral density (BMD) with pre- and postmenopausal women. METHODS: We have evaluated 201 premenopausal women and 322 postmenopausal women out of total 651 who had visited Health Screening Center in the hospital of Eul-Ji Medical College from November, 1995 to July, 1996. RESULTS: The mean age of total subjects, premenopausal women, postmenopausal women were 51.9, 43.9, 56.8 years, respectively. The mean period after menopause was 8.1 years. Significant difference was seen in BMI, lipid profile and BMD according to age and menopause(P<0.01). BMI was related to lipid profile(P<0.01), but not to BMD(P>0.1). In postmenopausal women BMI, BMD and lipid profile were related to postmenopausal period (P<0.05). In viewing their correlations BMD had strong adverse correlations with factors such as age, menopause, and postmenopausal period. Lipid profile had weak positive correlations with factors such as age, menopause, BMI(P<0.001). CONCLUSIONS: The lipid profile are related to factors such as age, BMI, menopause, and postmenopausal period. The BMD is related to above factors except BMI. Prospective study is needed to evaluate the influence of estrogen on BMD and lipid metabolism. Thus, it helps to the prevention and treatment of the osteoporosis and hyperlipidemia in the postmenopausal women.

In the general, Disodium estdronate (EHDP) had effects of inhibition in bone resorption, dissolution of hydroxyapatite crystal and decreasing the turn over rate in Paget's disease. Clinically it is used as the drug of treatment for the osteoporosis, heterotropic ossificatiom and Paget's disease inspite of some, controvesies, but there is few article about the effect of EHDP on osteoblast. Authors tries to observe the effect of EHDP on osteoblast using the MC3T3-El osteoblast cell line which has very similar chrateristics with human osteoblast and evaluate the effect by the criteria of changes of morphology, number of osteoblast, and alkaline phosphatase activity. The results are obtained as following: l. EHDP has direct inhibitory effect on the proliferation of osteoblast. 2. EHDP increase the alkaline phosphatase activity in vitro.

8.A Case of the Aberrant Retinal Vessels.

The cilio-retinal artery appears on the temporal side of the optic disc margin and is concerned with the supply of the portion between the optic disc and the macula. So, the cilio-retinal artery preserves vision when occlusion of the central retinal artery occurs. The aberrant macular artery is very rare and in most of the cases have been derived from the inferior temporal arterial or venous trunks. When this artery is appeared, the corrected vision is normal but sometimes diminished. The authors have experienced two aberrant retinal vessels of a large cilio-retinal artery and aberrant macular artery combined with the Stargardt's maculopathies. It was reviewed clinically with the literature.

Nephrocalcinosis is described as the deposition of calcium crystals in the renal parenchyma that result from prolonged states of hypercalcemia. Recently recognized is the deposition of calcium phosphate in the renal tubular injury by using sodium phosphate cathartics commonly used for the colonic cleansing. This phenomenon was termed phosphate nephropathy. Acute renal failure secondary to phosphate nephropathy has been increasingly recognized as a complication after the administration of sodium phosphate solution for colonoscopy. We report a case of acute phosphate nephropathy following oral sodium phosphate solution to cleanse the bowel for colonoscopy. A renal biopsy showed diffuse tubular calcium deposition.
