2.Effects of glutamate on dehydroascorbate uptake and Its enhanced vulnerability to the peroxidation in cerebral cortical slices.

Pro-oxidant properties of ascorbate have been studied with uses of brain tissues and neuronal cells. Here we address potential mechanism of ascorbate coupling with glutamate to generate oxidative stress, and the role which oxidized ascorbate (dehydroascorbate) transport plays in oxidative neuronal injury. Ascorbate in neurones can be depleted by adding glutamate in culture medium since endogenous ascorbate can be exchanged with glutamate, which enhances ascorbate/ dehydroascorbate transport by depleting ascorbate in the neurons with the glutamate-heteroexchange. However, ascorbate is known readily being oxidized to dehydroascorbate in the medium. Glutamate enhanced the dehydroascorbate uptake by cells via a glucose transporter (GLUT) from extracellular region, and cytosolic dehydroascorbate enhanced lipid peroxide production and reduced glutathione (GSH) concentrations. Iso-ascorbate, the epimer of ascorbate was ineffective in generating the oxidative stress. These observations support the current concept that the high rates of dehydroascorbate transport via a GLUT after the release of ascorbate by glutamate leads to peroxidation, the role of glutamate on ascorbate/ dehydroascorbate recycling being critical to induce neuronal death via an oxidative stress in the brain injury.

3.Dynamic MR Imaging of Hepatic Hemangioma and Hepatocellular: Findings and Differential Diagnosis.

PURPOSE: We performed dynamic MR imaging using GdDTPA to find characteristic enhancement pattern of hepatic hemangioma distinguishing from hepatocellular carcinoma. METHODS AND MATERIALS: 28 hepatic hemangiomas and 10 hepatocellular carcinomas were evaluated. Serial dynamic scans after Gd-DTPA(0.1mmol/kg) intravenous injection were obtained by using 0.5T machine and analyzed contrast-to-noise ratio(CNR) of the lesion and enhancement pattern on each scan. RESULTS: Hepatic hemangiomas had positive CNR from 1-2 minute images, and revealed typical "fill-in phenomenon" on early phase with prolonged enhancement in 26 cases(92.8%), and early homogeneous enhancement with isointensity on delayed phase in 2 cases(7.2%) of small hemangiome~ Hepatocellular carcinomas revealed inhomogeneous enhancement with hypointensity on delayed phase in 10 cases(100%) and 3 cases (30%) of capsular enhancement. CONCLUSION: Hepatic hemangioma can be easily distinguished from hepatocellular carcinoma by using Gd DTPA enhanced dynamic MR imaging according to its typical enhancement pattern of "fill-in phenomenon" and prolonged enhancement.

4.Factors Affecting the Performance of Nurses in Delirium Care

PURPOSE: The aim of this study was to identify the factors influencing nursing performance in caring for patients with delirium.METHODS: This study included 166 nurses who worked for more than 6 months at 4 general hospitals in Jeju Province, South Korea. Patients were administered a self-reported questionnaire. The study was carried out from May 20, 2014, to June 19, 2014. Data analyses were conducted using stepwise multiple regression, Pearson's correlation coefficients, t-test, and analysis of variance using the SAS WIN 9.2 program (SAS Institute, Cary, NC, USA).RESULTS: The factors associated with nurses' performance in delirium care were work position (β=.22, t=2.58, p < .001) and having received education on delirium care (β=.16, t=2.24, p=.026).CONCLUSION: The results showed that the nurses' work position and having received education on delirium care affected nursing performance in delirium care. In order to improve nurses' performance in delirium care, hospitals should provide a delirium education program for nurses and establish standard guidelines on delirium care.

7.A traumatic pseudoaneurysm of the superficial temporal artery.

Pseudoaneurysm arising from the superficial temporal artery (STA) is very rare and is most commonly caused by blunt trauma. Most pseudoaneurysms of the STA usually present as a painless pulsating mass, with concomitant symptoms according to location, and their size may rapidly increase. The treatment of choice is ligation and resection. We present a case of pseudoaneurysm arising from STA after a penetrating injury caused by broken glass. We describe the history, findings of physical examination, Doppler sonography, angiography, histopathology, and the outcome of treatment. We also include a brief review of this condition.

8.Laparoscopic-assisted resection of ileal lipoma causing ileo-ileo-colic intussusception.

Adult intussusception is rare, and the majority of cases has an underlying cause that requires surgical resection. We report a case of a 39 yr-old man with ileo-ileo-colic intussusception caused by ileal lipoma that was successfully managed by a laparoscopic-assisted surgical maneuver. Using a three-cannula technique, ileo-colic intussusception was reduced laparoscopically. Then, through a 4-cm transverse incision in the right lower quadrant abdomen, ileo-ileal intussusception was reduced manually, and a resection of the tumor-bearing ileal segment and end-to-end anastomosis was performed extracorporeally. Although the role of laparoscopy in managing intussusception is not clearly defined, laparoscopy may be an alternative approach to the surgical treatment of adult intussusception in selected cases.

9.Basic Symptoms in Schizophrenic Patients.

OBJECTIVES: This study is to explore the frequency of basic symptoms and their relationship with current clinical variables in patients with schizophrenia. METHODS: Eighty-eight schizophrenic patients diagnosed with SCID were interviewed with the Bonn Scale for the Assessment of Basic Symptom (BSABS) and Positive and Negative Syndrome Scale (PANSS). RESULTS: Most frequent basic symptoms were related to abnormalities in perception, cognition and stress reactivity. After controlling age effect, number of admission was correlated negatively with cluster 3 (impaired tolerance to normal stress). Onset age was correlated positively with cluster 3 and cluster 4 (disorders of emotion and affect) and total score. Scores of cluster 1 (thought, language, perception, motor disturbances), and cluster 2 (impaired bodily sensations), and total score of BSABS were significantly lower in paranoid type than residual type of schizophrenia. Scores of cluster 1, cluster 2, and total score of BSABS were correlated positively with subscales for positive and negative symptoms and general psychopathology and total score of PANSS. Cluster 3 was correlated positively only with general psychopathology score and cluster 4 with both positive symptoms and general psychopathology score. CONCLUSION: These findings suggest that certain basic symptoms aggregate significantly in schizophrenia, especially in paranoid type schizophrenia and that systemic evaluation of basic symptoms can be used for the prediction of onset and progress of schizophrenia.
