2.Fracture strength of zirconia ceramic crowns according to tooth position.

PURPOSE: The purpose of this study was to compare the fracture strength of the zirconia ceramic crowns according to tooth position. MATERIAL AND METHODS: After 10 metal dies were made for each group, the zirconia ceramic crowns were fabricated using CAD/CAM system (Lava(TM) All-Ceramic System) and each crown was cemented on each metal die with resin cement (Rely X(TM) Unicem). The cemented zirconia ceramic crowns mounted on the testing jig were inclined with 30 degrees to the long axis of the tooth and the universal testing machine was used to measure the fracture strength. RESULTS: 1. The fracture strength of the zirconia ceramic crown in the lower 1st molar (2963 N) had the highest and that in the lower central incisor (1035 N) had the lowest. 2. The fracture strength of zirconia ceramic crown was higher than that of the IPS Empress crowns in all tooth position. 3. The fracture mode of the crowns was similar. Most of fracture lines began at the loading area and extended through proximal surface perpendicular to the long axis of the crowns. 4. There were no significant differences on the fracture strength of the zirconia ceramic crowns according to tooth position except in premolar group. CONCLUSION: Within the limitations of this study, the results suggested that strength of zirconia ceramic crown is satisfactory for clinical use.

3.Herpes Zoster of Oral and Maxillofacial Area: Cases Report.

Herpes zoster is acute viral infection characterized by the appearance of vesicles that occur on the skin and mucous membrane along the pathway of an involved sensory nerve. The incidence is increased in those who have history of trauma, malignant tumor, radiation therapy and the immuno-suppressive drug users. Although herpes zoster is usually as benign viral infection, complications may occur especially when cranial nerves are involved. So careful treatment is necessary when cranial nerves are involved. We have treated two cases. One is involved trigeminal nerve mandibular branch, another maxillary branch. We have used conservative therapy, pain control, acyclovir on this area and obtained good result. After 17 and 8 months respectively, no complications occur in our two cases. So we present two cases with review of literature.

4.Floating Hem-o-Lok Clips in the Bladder without Stone Formation after Robot-Assisted Laparoscopic Radical Prostatectomy.

Hem-o-Lok clips (Weck Surgical Instruments, Teleflex Medical, Durham, NC, USA) are widely used in robot-assisted laparoscopic radical prostatectomy because of their easy application and secure clamping. To date, there have been some reports of intravesical migration of these clips causing urethral erosion, bladder neck contractures, and subsequent calculus formation. We report the first case of bladder migration of Hem-o-Lok clips without stone formation after robot-assisted laparoscopic radical prostatectomy. The Hem-o-Lok clips were found during urethral dilation with a guide wire for bladder neck contracture under cystourethroscopy. The Hem-o-Lok clips were floating in the bladder without stone formation and were removed by a cystoscopic procedure.

5.Unilateral Nevoid Acanthosis Nigricans with a Submammary Location.

Unilateral nevoid acanthosis nigricans (AN) is an exceedingly rare form of AN, which is a benign disorder of keratinization. The lesions of unilateral nevoid AN are distributed unilaterally while the histopathologic findings are similar to the common form of AN, and this is characterized by hyperkeratosis, papillomatosis and moderate acanthosis. A 19-year-old female presented with asymptomatic brownish pigmented plaques on the medial side of the left submammary area. Based on her clinical and histopathologic findings, a diagnosis of unilateral nevoid AN was made. She was treated with topical tretinoin and her skin lesions were almost cleared 9 months after her first visit to our clinic. Various previous reports have described the localization of this disorder, but there has been no previous case report with the clinical manifestations on the unilateral submammary area. Therefore, we herein report the first case of unilateral nevoid AN with an unusual submammary location.

6.A Prospective Study on Ceftriaxone-associated Biliary Pseudolithiasis: A Dose-related Comparison.

PURPOSE: Ceftriaxone, a parenteral third-generation cephalosporine, is widely used in the treatment of various bacterial infections. It possesses high calcium-binding affinity, forming complexes with calcium in bile salts to develop precipitate that mimics gallstone on ultrasonography. Biliary pseudolithiasis resolves completely with cessation of therapy, but several symptomatic patients have undergone cholesystectomy. We prospectively evaluated the incidence, risk factors and dose- related comparison with ultrasonography. METHODS: Between November 1998 and August 1999, 81 cases of inpatients on ceftriaxone treatment in Dongguk University Pohang Hospital were enrolled for this study. They were divided according to dose of ceftriaxone, high-dose and low-dose groups. Repeated sonography was performed on 1, 3, 5 and 7 days after initiation of ceftriaxone treatrnent and then weekly until pseudolithiasis were resolved. RESULTS: Thirty-eight percent of the subjects acquired pseudolithiasis. Sonographic abnormalities appeared from 1 to 10 days after ceftriaxone therapy and completely resolved from 1 to 24 days after cessation of ceftriaxone therapy. The incidence of pseudolithiasis was significantly higher in the high-dose group(P<0.001). In the high-dose group, fasting over a day was a significant risk factor of pseudolithiasis(P<0.01). Sex, age, duration of ceftriaxone therapy, laboratory findings, type of infection or chief complaint were not significant risk factors for pseudolithiasis. CONCLUSION: We suggest that abdominal ultrasonography should be considered in all children who receive high dose ceftriaxone with fasting over a day. If pseudolithiasis was developed, we can detect the most of resolution after 30 days of cessation of therapy.

8.Actinic Granuloma Showing the Histopathologic Findings of Suppurative Granuloma.

Actinic granuloma is a rare chronic granulomatous dermatosis that is characterized by the elastophagocytosis by multinucleated giant cells. This malady presents in middle-aged individuals and it manifests as asymptomatic papules or annular plaques in the sun-exposed area. There are four main histopathologic patterns of actinic granuloma, including the giant cell, necrobiotic, histiocytic and sarcoidal variants. However, there are no previously reported cases of actinic granuloma associated with the histopathologic findings of suppurative granuloma. We herein report on an unusual case of actinic granuloma that showed the histopathologic findings of suppurative granuloma in a 58-year-old female.

9.Cavernous Hemangiolymphangioma of the Testis without Cutaneous Hemangiomatosis in an Elderly Patient.

Hemangiolymphangioma is an extremely rare malformation of both the lymphatic and blood vessels. To date, however, there are no reports in the literature of a hemangiolymphangioma of the testis. An 84-year-old man visited our hospital for investigation of a 1-month episode of a rapidly growing mass in his right scrotum. Scrotal ultrasonography revealed a multilobulated mass with septation in the testis. Testicular tumor markers were within the normal limit. Radical orchiectomy was performed. At surgery, a red, wide-based, nodular tumor was found on the testis. Histological examination of the resected specimen showed it to be a cavernous hemangiolymphangioma. Here we report this first case of a cavernous hemangiolymphangioma of the testis without cutaneous hemangiomatosis in an elderly patient.

10.Cerebrovascular Complications after Coronary Bypass Surgery.

BACKGROUND: Cerebrovascular accidents after coronary bypass surgery(CBS) are serious and responsible about 10% for postoperative mortality. Recently, the rate of cerebrovascular complication after CBS has increased with increasing number of the operations for high risk patients with old age, hypertension, diabetics and etc. This study was intended to identify the risk factors of cerebrovascular accident after CBS by reviewing the medical records of the patients who underwent CBS. MATERIALS AND METHOD: We reviewed the medical records and statistically analyzed the data of 185 patients who underwent CBS between March 1991 and July 1999. RESULT: The rate of cerebrovascular complication was 7.5%(14 patients) of which there were 5 deaths. Total number of postoperative death was 11 during the same period, and 45.5% of postoperative deaths were associated with cerebrovascular complication. Postoperative arrhythmia(p=0.0064), history of previous CVA(p=0.0090), bypass time(p= 0.0181), atherosclerosis of aorta(p=0.03575) and diabetics(p=0.0452) were statistically related with the development of postoperative cerebrovascular complication. Two patients underwent carotid endarterectomy concomitantly with CBS, neither of them developed cerebrovascular complications. The 3 patients over 75 years did not develop cerebrovascular complication after CBS. CONCLUSION: Risk factors associated with the development of postoperative cerebrovascular complication after CBS were postoperative arrhythmia, history of previous stroke, bypass time, atherosclerosis of aorta and diabetics.
