Objective To investigate the clinical efficacy of combination of umbilical compress therapy with Xiaotan Lishui Gel and intraperitoneal chemotherapy with cisplatin in patients with gastric cancer ascites. Methods A total of 78 patients were included and divided randomly into treatment group (39 patients) and control group (38 patients). Patients of both groups received intraperitoneal chemotherapy with cisplatin, and the treatment group received additionally umbilical compress therapy of Xiaotan Lishui Gel. Two weeks were regarded as a course of treatment. All patients were followed up for 180 days. The total effective rate, puncture-free survival, interval time of puncture, median survival time, life quality, improvement of symptoms and safety of the two groups were observed. Results Finally 73 patients completed the observation. The total effective rate was 56.76%(21/37) in the treatment group and 33.33%(12/36) in the control group, with statistical significance (P=0.044). The puncture-free survival and interval time of puncture were 59 d and 62 d, respectively, and 49 d and 54 d in the control group, respectively (P<0.05). There was no difference in median survival time between the two groups. Karnofsky performance status in the treatment group was significantly higher than the control group (P=0.012), and decreased mobility, abdominal pain, abdominal distension, early satiety and urinary frequency in the treatment group were better improved than the control group (P<0.05). Conclusion Combination of umbilical compress therapy with Xiaotan Lishui Gel and intraperitoneal chemotherapy with cisplatin can postpone the ascites increasing speed of patients with gastric cancer ascites, and improve patients’ life quality.

Objective To investigate the role of hypoxia inducible factor-1α (HIF-1α) and von Hippel-Lindau (VHL) in murine endochondral ossification. Methods The knockout of HIF-1α or VHL gene in murine osteoblasts was accomplished by conditional knockout technique at 4th, 8th and 12th week, and the differences between wild-type group and knock-out group in endochondral ossification were detected by HE staining, micro-CT scanning, trabecular bone area measurement, calcium content measurement, tetracycline fluorescence labeling, Real-time PCR and Western blotting. Results After knockout of HIF-1α gene in osteoblasts, the expression of vascular endothelial growth factor ( VEGF) reduced, the rate of new bone formation stepped down, the content of calcium became less, and the trabecular bone volume decreased (P <0.05) . After knockout of VHL gene in osteoblasts, the expression of VEGF increased, the rate of new bone formation stepped up, the content of calcium became more, and the trabecular bone volume was promoted (P < 0.001). Conclusion During murine endochondral ossification, VHL/HIF-1α signal pathway promotes angiogenesis through the stimulation of VEGF expression, which subsequently accelerates osteogenesis.

Objective To explore the expression and regulatory mechanism of hypoxia inducible factor-1?(HIF-1?)during fracture healing. Methods Mouse models of tibia fracture healing were established, and callus samples were collected 1,3,7,14,21 and 28 days after fracture. The development of callus and new bone formation were evaluated with roentgenology,Micro-CT and tetracycline double labeling method, and the expression of HIF-1?,vascular endothelial growth factor(VEGF),Runx2 and ALP in callus were detected with RT-PCR,Western blotting and immunohistochemistry. The relationship between HIF-1? and fracture healing was analysed. Results The expression of HIF-1? was detected in cells in the fracture sites as well as in evolved osteoblasts, chondrocytes and osteocytes in early callus under hypoxia. The highest expression rate of HIF-1? achieved on the 7th day after fracture, lasted for about 7 days, then decreased gradually, and returned to intact level on the 28th day after fracture. The expression tendency of VEGF resembled that of HIF-1?.Bone formation activity was more active in early callus, and the callus volume peaked on the 14th day after fracture and decreased gradually. The mineralization of callus mainly took place in the late healing period(14th to 28th day after fracture). Conclusion Cells involved in fracture healing are hypoxia-responsive cells, which express HIF-1?.HIF-1? can regulate cell state and function, and can promote angiogenesis so as to play a crucial role in fracture healing.

OBJECTIVETo investigate the effect of compound Danshen Dripping Pill (DSP) on carotid arterial intima-media thickness (IMT) in patients with type 2 diabetes mellitus (T2DM).
METHODSOne hundred and thirty T2DM patients were assigned to four groups, 32 in the Group A, the control group treated with blood glucose (BG) and blood pressure (BP) controlling; 32 in the Group B, with BG, BP and blood lipid (BL) controlling, 32 in Group C with BG, BP, BL controlling and vitamin E administration, and 34 in Group D with BG, BP, BL controlling and DSP administration. Patients in Group D were subdivided by Chinese medicine syndrome differentiation into four types, 8 of Yin-deficiency with flourishing heat type (YDFH), 5 of both qi-yin deficient type (BQYD), 8 of both yin-yang deficient type (BYYD) and 13 of blood-stasis and qi-stagnant type (BSQS). Fasting blood glucose (FBG), BP and BL in patients were observed periodically, and IMT in them were measured by ultrasonography before treatment, as well as at the end of the 1st, 3rd, and 5th year of treatment to dynamically observe the changes of IMT and condition of plaque formation, and analyze the relation between them with FBG, BP and BL.
RESULTSThe 5-year follow-up was performed in 105 patients. In the observation period, level of total cholesterol (TC) showed a decreasing trend and level of high density cholesterol (HDL-C) showed an increasing trend in all the 4 groups, the improvements in Group C and D were slightly better than those in Group B, while significantly superior to those in Group A; the changes of FBG and glycosylated hemoglobin (HbAlc) were insignificant in the 4 groups. IMT and numbers of atheroma plaque increased gradually in all groups in the observation period, however, the changes in Group D were lesser than those in other groups, showing significant difference (P < 0.01). It was showed that the increasing of cervical carotid IMT in T2DM patients was correlated with levels of HbAlc, HDL-C, LDL-C, triglyceride and TC, especially in Group D.
CONCLUSIONDSP might delay the occurrence and development of diabetic macro-vascular disease.

Journal of Practical Radiology 2014;(10):1653-1656,1664
Objective To evaluate two methods of selecting region of interest (ROI)to measure the apparent diffusion coefficient (ADC)of breast lesions on diffusion-weighted imaging (DWI).Methods MRI findings (including plain MRI,dynamic contrast-en-hanced MRI and DWI)of 5 3 cases with breast mass confirmed by pathology were analyzed retrospectively. Two observers independ-ently measured the ADC of breast lesions by the method of fixed ROI with the size of 10 mm2 ±2 mm2 and the routine method. The mean ADCs of benign and malignant breast lesions were compared between two methods and two observers by using paired samples T-test.The values of ADC diagnosing breast cancer were compared between the two methods by using the receiver operating charac-teristic (ROC)curve. Results Among 62 lesions of 53 cases,33 lesions were malignant, and 29 lesions were benign. For the first observer, the mean ADCs of benign and malignant lesions were (1.647±0.359)×10-3 mm2/s and (1.136±0.227)×10-3 mm2/s measured by the fixed ROI method, respectively. The area under the ROC was 0.897.The mean ADCs of benign and malignant le-sions were (1.603±0.415)×10-3mm2/s and (1.251±0.237)×10-3 mm2/s measured by the routine method, respectively. The area under the ROC was 0.768.For another observer, the mean ADCs of benign and malignant lesions were (1.647±0.357)× 10-3 mm2/s and (1.130±0.250)×10-3 mm2/s measured by the fixed ROI method, respectively. The mean ADCs of benign and malignant lesions were (1.590± 0.390)×10-3 mm2/s and (1.250±0.237)×10-3 mm2/s measured by the routine method,respec-tively.There was no significant difference for the mean ADC of benign lesions between the two methods. However, there was sig-nificant difference for the mean ADC of malignant lesions between the two methods. The diagnostic value of ADC measured by the fixing method was better than that of the routine method (P=0.008).There were no significant differences for ADCs of benign and malignant lesions between the two observers. Conclusion DWI has an important value for differential diagnosis of benign and malignant breast lesions. The measurement of ADC by the fixed and smaller ROI is better than that by the routine ROI for dis-tinguishing malignant from benign breast lesions.
