1.A Case of Bowen' s Disease of the Nail Bed Presenting as Longitudinal Melanonychia.

Bowens disease is an intraepithelial squamous cell carcinoma. It occur s anywhere on the surface of the body. Bowens disease involving the nail bed epithelium is very rate. Recently, some literatures showed that human papillomavirus is sssociated with squamous cell carcinoma of the finger by in situ hybridization or polymerase chain reaction. A 30-year-old housewife deve]oped longitudinal melanonychia with as imptomatic fissured nail plate on her right third finger. The hiat,ologic specimen showed diffuse anaplastic change confined to the nail bed epithelium. The subungual eyithelium was found to contain human papillomavirus DNA type 31/33/35 with the use of in situ hybridization.

2.A Case of Severe Hypertension associated with Growth Hormone Therapy.

The use of recombinant DNA technology to produce human growth hormone has resulted in a marked increase in availability of Growth Hormone(GH) to treat short stature due to GH deficiency and other conditions, such as Turner syndrome, familial short stature, chronic renal insufficiency and intrauterine growth retardation (IUGR). But, the GH therapy may result in the adverse events such as sodium and water retention, pseudotumor cerebri, slipped capital femoral epiphysis, growth of nevi, recurrence of tumor. We experienced a case of severe hypertension associated with GH therapy in a 14-year-old male who presented high blood pressure up to 190/100 mmHg and normalized at 2-3 weeks after discontinuation of GH. Therefore, we think that the blood pressure should be carefully monitored during GH therapy.

3.A Case of Basal Cell Epithelioma Arising in a Nevus Sebaceus during Childhood.

The potential for sebaceous nevi to evolve into secondary benign or malignant EuIror is well recognized. In general, malignant transformation does not occur until postpubertal life. We experienced a case of basal cell epithelioma arising in a nevus sebaceous on a 12-year-old boy´s parietal scalp. We recommended that surgical excision should be undertaken as early as possible to detect the malignant transformation of nevus sebaceous.

4.Five Cases of Agminated Lentigines.

Agminated lentigines may be defind as a circumscribed grouping of small pigmented macules arranged in a small or large group, often in a segmental pattern, each macule consisting of a lentiginous epidermal proliferation of melanocytes. We report five cases of agminated lentigines in otherwise healthy persons. Histologic exarnination of the pigmented lesion revealed findings consistent with lentigo simplex.

5.A Case of Cutis Marmorata Telangiectatica Congenita.

Cutis marmorata telangiectatica congenita(CMTC) is a rare congenital cutaneous vascular anomaly. The major skin findings are persistent, fixed cutis marmorata, telangiectasia, and phlebectasia. In approximately 50 % of patients, the disorder tends to be associated with various con-genital anomalies. We describe in this report a 4-month-old male infant with CMTC. The patient had red or violet marbled patches, telangiectases, and atrophy on the right arm present at birth. During the follow up period of 12 months, there was no change in the cutis marmorata pattern and telangiectases, whereas the atrophy has been less prominent than at the initial visit.

6.The Effects and Complications of Mitomycin-C for Prevention of Recurrence after Pterygium Operation.

The most common problem in pterygium operation is the recurrence. Therefore radiation therapy or instillation of mitomycin-C after pterygium operation have used to prevent the recurrence of pterygium. We evaluated recurrence rate and complication rate with mitomycin-C and without mitomycin-C after the operation of pterygium in 113 eyes. The 112 patients(113 eyes) underwent simple surgical excision, whose mean age was 50.7 tears, and whose follow-up period was 6 to 12 months. 0.04% solution of mitomycin-C was instilled to 61 eyes and not to 52 eyes. Recurrence was noticed in 12 out of 61 eyes(19.6%) among the group not using mitomycin-D and in 2 out of 52 eyes(3.8%) among the group using mitomycin-C after pterygium operation. One eye in patients using mitomycin-C showed severe corneal edema due to endothelial decompensation by the toxicity of mitomycin-C. It is concluded that mitomycin-C ws effective to prevent recurrence after pterygium operation, but we should be aware of the severe complications by the toxicity of mitomycin-C.

8.Long-term Follow-up for Penile Prostheses in Patients with Spinal Cord Injury.

PURPOSE: Recently the use of penile prostheses in patients with spinal cord injury(SCI) has been beneficial for sexual dysfunction, maintenance of external devices and the treatment of penile skin lacerations. We reviewed 50 patients with SCI who had been followed for more than five years after penile prosthetic implantation, particularly in regard to surgical success rate and complication rate. MATERIALS AND METHODS: From 1988 to 1993, 102 penile prostheses were implanted in 50 patients with SCI. Penile prosthetic implantation was done for sexual dysfunction in 29 patients and for sexual dysfunction and urinary management in 21 patients. The types of penile prostheses were malleable in 13 patients, Dynaflex in 35 patients and AMS 700 CXM in two patients. RESULTS: We experienced prosthetic infection in six patients, mechanical failure in three patients, urethral erosion in two patients and penile skin necrosis in one patient. The overall surgical success rate was 88% and the five-year complication rate was 24%. CONCLUSIONS: Though the complication rate of penile prosthetic implantation in patients with SCI was higher than in the general population, penile prosthetic implantation improves the quality of life with regard to sexual function and urinary management in patients with SCI.
