1.A Study of Stress, Stress Coping Style and Health Symptoms in 6th Grade Elementary School Children.

The purpose of this study was to examine the stressors and stress coping styles of 6th grade elementary school children and to explore the relationship between stressful life events and health symptoms and the effects of coping style which is theoretically considered to mediate the relationship between stress and health symptoms. The study subjects consisted of 329 in 6th grade elementary school children in Cheong-Ju city. Of the 329 subjects, 171 were boys and 158 were girls. For this study, three kinds of questionnaires were adopted as follows ; 1) Feel Bad Scale (FBS) by Lewis et al., 2) Lazarus-Folkman's Way of coping questionnaire 3) Hee Sun Shin's Health Symptom questionnaire(HSQ) The researcher visited the school and collected data in the class using the questionnaire method after an explanation of the purpose and procedures was given to the children. Data collection was done for 10 days(from 5th to 15th of July 1997). The data were analyzed by descriptive statistics, correlation analysis, multiple regression analysis using the SAS statistical program. The results of this study were as follows; 1. The mean score for the FBS was 211.37(range : 77-427). The most severe stressors perceived by children were parental divorce and other's smash or steal of my things. The most frequently experienced stressful life events were conflict with siblings and being scolded for other's fault. 2. The most frequently used stress coping style was the active coping (M=17.85), followed by passive(M=13.64) and magical one(M=13.42). 3. The mean score for the HSQ was 23.30(range : 0-72) The most frequently complained health symptoms were headache and having much worry about everything. 4. The was a significant relationship between stressful life events and health symptoms(r=.60, p<.001). Also, stressful life events were positively related with passive coping(r=.27, p<.001) and magical coping(r=.38, p<.001). Health symptoms were positively correlated with passive coping(r=.33, p<.001) and magical coping(r=.41, p<.001). 5. Stepwise multiple regression analysis revealed that the most powerful predictor was the variable of stressful life events. Health concerns, magical coping style, passive coping style and active coping style accounted for 49.15% of the variance in health symptoms. This study revealed that stressful life events correlated with health symptoms in 6th grade elementary school children and passive coping and magical coping had mediating effects on this relationship. The implication for nursing is that there is a need to develop supportive interventions for high risk population to decrease health problems due to stress.

2.Significance of Whole Body bone Scan in bone and Joint Tuberculosis

For confirmative diagnosis of bone and joint tuberculosis, it is made by culture method, inoculation to guinea pig, or histological examination of the specimen obtained from a lesion site. But plain reontgenogram is not valuable for early detection of the disease because specific, abnormal finding is not found in early stage and only found in late stage as progressive destruction of bone and joint. Many reports were made thst whole body bone scan, as diagnostic tool, was valuable in other orthopedic disease, but report for its diagnostic value in bone and joint tuberculosis is very rare. The study was carried out on 35 patients of bone and joint tuberculosis who whole body bone scan was performed before operation from Janusry, 1978 to December, 1987 in Depaartment of Orthopedic Surgery, Yonsei University college of Medicine. The following results were obtained. 1. Value of early detection of tuberculosis is not much as pyogenic infection. Intensity of uptake was decreased in tuberculosis than acute pyogenic osteomyelitis and arthritis, but increased than chronic osteomyelitis. 2. Intensity of uptake was decreased in female than male, and also increased in age. group under 10 and over 60 than other age group. 3. The longer duration of the disease, the more decreased intensity of uptake. 4. Intensity of uptake is increased in joint tuberculosis than bone, snd also increased in tuberculosis of spine than long bone. 5. Multifocal disease was detected in 14.3% by whole body bone scan.

4.Molecular Identification and Distribution of Aquaporins in Human and Rat Testes.

OBJECTIVE: Several water channels (aquapoins; AQP) that belong to the MIP (major intrinsic protein) family have identified. In the selected tissues including red blood cells or renal tubules, water movements are abundant and/or physiologically important. Unexpectedly, a high water permeability of human and ram sperm has been reported. Recent studies showed that AQP7 and AQP8 are present in testes so that the high water permeability of human sperm suggested to be mediated by AQPs. METHOD: To identify the identity of aquaporins expressed in testes, RT-PCR was performed using degenerative primers, which were designed to correspond to highly conserved sequences surrounding the Asn-Pro-Ala (NPA) motifs in the aquaporins. New expressed AQP series were reconfirmed by immunohistochemical study using rabbit polyclonal antibodies. RESULTS: DNA sequencing of PCR products revealed that AQP2 and AQP3 mRNA as well as AQP7 and AQP8 are expressed in human and rat testes, AQP2 are expressed in spermatozoa, interstitial cells and myofibroblasts and AQP3 are expressed in myofibroblasts of semineferous tubules on immunocytochemical stain. CONCLUSION: These results indicate that multiple aquaporins are expressed in testes, and that they may have important roles in the spermatogenesis and the germ cell function of testis.

5.Benign Neonatal Hemagiomatosis with Unusually Persistent Lesions and Conjunctival Hemangioma.

Most neonatal cutaneous hemangiomatosis are complicated by visceral involvement and. are associated with a high mortality rate in the first month of life. Some neonates with multiple cutaneous hemangiomas, however, may follow a benign course of spontaneous resolution without visceral involvement. Such cases are called benign neonatal hemangiomatosis (BNH). BNH is characterized by a lack of mucosal or symptomatic visceral involvement. rapid spontaneous regression of cutaneous hemangimas, and an excellent prognosis. We report herein a case of BNH which is atypical compared to previously reported BNFI cases. Our patient had a more prolonged course than those of usual cases with BNH and showed a conjunctival involvement.

6.A Case of Flexural Darier' s Disease.

Dariers disease is a domirantly inherited skin disease that is characterized by keratotic papules predominantly on the sebornieic areas, palmar pits, and a nail dystrophy. The diagnosis is difficult in the small group with flexural involvement because the features of flexural Dariers disease overlap clinically and histolopica.lly with those of Hailey-Hailey disease. The diagnosis of Dariers disease may be confirmed if the patient has the pathognomonic nail dystrophy. We presents a case of flexured. Dariers disease in a 65 year old woman. She has suffered from skin lesions which have some resemblance to those of Hailey-Hailey disease. We could diagnose her skin lesions as Dariers disease because she had the eharacteristic nail dystrophy.

7.A Significance of Abdominal CT Manifestration Associated with Hypovolemic Shock in Blunt Abdominal Trauma of Children.

BACKGROUND: We reviewed retrospectively the clinical features and contrast material-enhanced computed tomography(CT) after blunt abdominal trauma demonstrated a characteristic hypoperfusion complex. Our purpose were to evaluate the hypothesis that children with this 'hypoperfusion complex' CT finding were associated with a tenuous hemodynamic state, severe injury and a poor outcome. METHODS: Between January 1996 and December 1999, 39 consecutive children who sustained blunt trauma and were suspected clinically of having intraabdominal injury underwent CT. Demographic data, blood pressure, indication for CT, pediatric trauma score(PTS) and Glasgow coma score(GCS) were recorded at the time of the initial examination. RESULTS: Three of the 39 children(8%) demonstrated a characteristic finding at CT which was described as the hypoperfusion complex. The 'hypoperfusion complex' was present in the 3 of the 39 traumatized children(8%), but was the main radiographic finding in the 3 of the 14 children with a severe pediatric trauma score(PTS<8) and in the 3 of the 6 children with a Glasgow coma score(GCS<8) who were examined with CT. CONCLUSION: The CT finding in all children with the hypoperfusion complex by definition included marked, diffuse dilatation of the intestine with fluid; abnormally intense contrast enhancement of the bowel wall, mesentery, kidney and/or pancreas; decreased caliber of the abdominal aorta and inferior vena cava(IVC); and moderate to large peritoneal fluid collection. This 'hypoperfusion complex' is a relatively rarely observed in injured children but appears to be associated with severe injury and a poor outcome. The pediatric trauma score provides a useful profile of injury severity in children. Prompt diagnosis of hypovolemic shock is important so that supportive therapy can be instituted to prevent further metabolic abnormalities and their potential cardiotoxic effects. In summary, the intense multiorgan enhancement pattern seen in the hypoperfusion complex indicates tenuous hemodynamic stability and is associated with a poor outcome.

8.An Experimental Study on the Radiation-induced Injury of the Rabbit Lung: Correlation of Soft-tissue Radiograph and High-Resolution CT Findings with Pathologic Findings.

PURPOSE: To describe soft-tissue radiographic and high-resolution CT findings of radiation-induced lung injury of rabbit over time and to correlate them with pathologic findings. MATERIALS AND METHODS: 15 rabbits were irradiated in the right lung with one fraction of 2000 cGy. After 4, 6, 12, 20, 24 weeks, 3 rabbits in each group were sacrificed and soft-tissue radiographs and high-resolution CT of their lung tissue were obtained. Radiological findings were correlated with pathologic findings. RESULTS: On soft- tissue radiogram, radiation pneumonitis shown as consolidation with air-bronchogram occurred in 3 cases after 6 weeks, and in 1 case after 12 weeks of irradiation. In addition, pneumonic consolidation with adjacent pleural contraction was seen in 2 cases after 12 weeks of irradiation. Fibrotic changes indicated by decreased volume occurred after 20 weeks and combined bronchiectatic change and bronchial wall thickening appeared after 20 weeks(N=1), and 24 weeks(N=3). HRCT findings of radiation pneumonitis were homogeneous, increased attenuation after 4 weeks(N=3), 6 and 12 weeks (each N=I), patchy consolidation after 6 and 12 weeks(each N=2), discrete consolidation after 12, 20 and 24 weeks(each N=I) and solid consolidation after 20 and 24 weeks(each N=2). Pathologically radiation pneumonitis and pulmonary congestion were seen after 4 and 6 weeks. After 6 weeks, collagen and reticulin fibers were detected along alveolar wall. Mixed radiation pneumonitis and fibrosis were detected after 12 weeks. 20 weeks after irradiation, fibrosis was well defined in interstitium and in 24 weeks, decreased number of alveoli and thickening of bronchial wall were defined. CONCLUSION: Radiation pneumonitis was provoked 4 weeks after irradiation on rabbit lung and progressed into radiation fibrosis 20 weeks after irradiation on soft-tissue radiographs and high-resolution CT. High-resolution CT is more precise in detecting early radiation pneumonitis and detailed pathologic findings.

9.Job Stress and Organizational Effectiveness of Dental Technicians.

OBJECTIVES: To investigate the relationship between job stress and the variables associated with the organizational effectiveness of dental technicians. METHODS: The job stresses were divided into the following six components; somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, and hostility. The organi-zational effectiveness variables in this study included job satisfaction, occupational commitment, organizational commitment and intent to leave. The sample used in this study consisted of 248 dental technicians working in hospitals and dental laboratories. The data were collected with self-administered questionnaires between March 16 and April 29 in 2002, and the results analyzed with ANOVA and multiple regression analysis. RESULTS: Firstly, job stress of dental technicians was found to be significantly different in relation to the number of workers at the workplace and the working hours. The level of job stress was the highest when the number of workers were more than 10, or when the daily working hours were over 12. Secondly, of the six variables of job stress, five variables of somatization, interpersonal sensitivity, depression, anxiety, and hostility were found to have significant negative correlations with job satisfaction. Thirdly, all six job stress variables were found to have significant negative correlations with the occupational and organizational commitments of the dental technicians. Finally, depression was the only job stress variable found to have a significant effect on the dental technicians' intent to leave. CONCLUSIONS: The results of this study indicate that personal and organizational efforts are very important to improve the organizational effectiveness in reducing work related stress. Organizational support, especially for managing the depression of dental technicians is recommended.
