1.Treatment of Chronic Osteomyelitis by Using Antibiotic-Acrylic Bone Cement Composites: Preliminary Report of 7 Cases

Antibiotic-acrylic bone cement composites demonstrated antibacterial activity as a result of diffusion of antibiotic from bone cement into surrounding enviroment, accordingly, the use of antibiotic incorporation to bone cement for the prevention and treatment of infection after joint surgery replacement has been adovocated. Early report of clinical success by using antibiotic-acrylic bone cement composites are encouraging. Our authors applied this concept to the treatment of chronic osteomyelitis. Antibiotic contained with bone cement was chosen by sensitivity test on pus culture finding. Seven cases of chronic osteomyelitia treated by using antibiotic-acrylic bone eement composites were .studied in the Department of Orthopaedic Surgery, Kyung Hee University Hospital from July 1979 to March 1981. The follow-up period was from 3 months tc 22 months. The results were as follow: 1. Six cases showed no evidence of recurrence except one by using antibiotic acrylic bone cement composites. 2. The impregantion of antibiotic in acrylic bone cement shall be regarded to be effective in increasing the local antibacterial activity by leaching the antibiotic into surrounding tissue over a period continiously in treatment of chronic osteomyelitis. 3. The use of antibiotic-acrylic bone cement compositea into dead space by saucerization are consid- ered an one such approach to the decrease of dead space. 4. The impregnation of antibiotic in acrylic bone cement is regarded a new concept in treatment of chronic osteomyelitis.

3.New Radiologic Projection for Acetabular Rim

Author take off the pelvic bone from the cadaver and made the accruate reconstructed the pelvic bone as well as normal bone. Then author setted the pelvic bone into the specialized acrylic globe as the normal person erect position. For the acetabular posterior wall, projected the radiologic beam from the 0°to 45°cephalad direction and then pelvic bone rotated internally 0°~45°, each section is 5°. For acetabular anterior wall, projected the radiologic beam from 0° to 45° caudal direction and pelvic bone rotated externally 0°~45°, each section is 5° too. From the pelvic bone experiments, author get on the good radiologic angle for acetabular anterior and posterior wall. Ane then make the radiologic projection for normal person as the pelvic experiment. In pelvic bone model experiment, For acetabular anterior wall and anterior colum, external rotation 20°~30° caudal tilting 30°~45° angle projection has good visualization. For acetabular posterior wall and posterior column (including anterior column partially), internal rotation 15°~20° cephalad tilting 15°~30°angle projection has the good visualization. In normal person experiment, For acetabular anterior wall and anterior column, external rotation 20°caudal tilting 30°angle projection shows the good visualization. For acetabular posterior wall and column (including anterior column partially) internal rotation 20°cephalad tilting 30°angle projection shows good visualization. From the pelvic bone model and normal person experiments, the following conclusion are obtained. 1. For posterior wall and posterior column(including anterior column partially), internal rotation 20°, cephalad tilting 30°view is good. 2. For anterior wall and anterior column, external rotation 20°, caudal tilting 30°view is good.

4.Extraskeletal Neoplasm Resembling Ewing's Sarcoma: Report of one case

Extraskeletal Neoplasm Resembling Ewings Sarcoma was firstly reported with comprehensive review of 39 cases by L. Angervall and F.M. Enzinger in 1975. The tumors chiefly affected young adults and most commonly involved the soft tissue of lower extremity. Microscopically, they were consisted of solidly packed small, round or ovoid cells arranged in sheets or lobules, which were indistinguishable from those of the Ewings sarcoma of bone. The authors recently experienced a soft tissue sarcoma occurring in deltoid area of young male patient, which was diagnosed extraskeletal neoplasm resembling Ewings sarcoma. The case review is presented.

5.Clinical Study of Total Hip Prosthesis Replacement: Report of 69 cases

The goals of hip arthroplasty have been to eliminate pain, decrease deformity, increase morbility and obtain stabiIity. Currently total hip replacement is the best avaliable procedure to accomplish these goals. A prospective study of total hip arthroplasty was begun at the orthopedic dept of Kyung Hee Medical College in 1974. Between May, 1974 and June, 1977. 69 total hip replacements of the Charnley type, the Trapesoidal-28 type and the Muller type were peformed by the authors. Of the 60 Patients involved in this study, 9 had bilateral operations. The Charnley type Prosthetic device was used in 56 cases, the Trapesoidal-28 type in 7 cases and the Muller type in 6 cases. Complications included wire breakage, dislocation, loosening, femur shaft perforation, nonunion of greater-trochanter, acetabular protrusion, transient femoral nerve palsy, femur shaft fracture, infection, socket lateralization, ectopic bone formation and death. In 59 patients of the 60 patients, 53 patients felt that they were definitely improved, 5 patients felt their condition was unchanged and 1 patient felt her condition was worse.

7.Clinical Analysis for Prognostic Factors of Intertrochanteric Fractures

There are many problems in treatment of intertrochanteric fractures due to osteoporosis and unstable pattern of fractures in old ages and they are different from those in young ages, for example, type of fracture, degree of osteoporosis, and general condition, ect. Anthors studied about the prognostic factors of 42 cases among 88 patients with intertrochanteric fractures, who were treated and followed up from Jan. 1982 to Dec. 1987. 1. The patients under 50 years old were mainly males(94.4%) and its chief cause of injury was traffic accident or fall down(72.2%). The cause of injury over 50 years old was slip down on ground(62.9%). 2. The osteoporosis was increased according to increased age of patients and then the unstable type of fracture was increased. In yaung patients, the unstable intertrochanteric fracture was rare despite of high violent injury. 3. The bone union of the follow-up patients was gained at average 13.8 weeks. 4. The sliding degree of lag screw was increased with unstable fracture and the incidence of metal protrusion into joint cavity was increased in the patients of eccentric fixation of lag screw into femoral head. 5. The all cases of complication were occured in osteoporotic patients over 50 years old and their causes were inadequate fixation of unstable fracture. 6. The most important factors in prognosis of intertrochanteric fractures are initial pattern of fractures, degree of osteoporosis and adequacy of reduction state(postop.).

10.Comparison of Case Management between Tele Care Regions and General Care Regions in Korean Medicaid.

PURPOSE: The purpose of the study was to compare recipients' health behavior, attitude to using medicaid, medication compliance, and the changes in hospital cost and visit-day of in-patient and out-patient care between tele-care regions (TCR) and general care regions (GCR) in Korean medicaid. METHOD: The design of the study was ex-post facto comparing recipients in TCR and GCR. The sample included 625 persons in TCR and 410 persons in GCR. To collect materials, the case manager interviewed recipients of medicaid and filled out questionnaires which were analyzed through SAS/PC 9.1. RESULTS: In studying health behavior and medication, compliance was not significant. However, the attitude to using medicaid was significantly more positive in TCR than in GCR. In out-patients, the change of hospital visit-day was not significant between TCR and GCR, but TCR showed a reduction in hospital cost compared to GCR. For in-patient recipients, GCR showed a greater reduction in changes in hospital cost and visit-day compared to TCR. CONCLUSIONS: The results of the study show that attitudes to using medicaid via telephone are positive and results are more effective than hospital visit consultation, and the cost of out-patient care could be reduced.
