2.The Influence of isoflurane on systemic vascualr resistance during cardiopulmonary bypass.

The influence of isoflurane on systemic vascular resistances was studied during total cardiopulmonary bypass with membrane oxygenator, low rate of 2.4 L/min/m(2) and moderate hypothermia Data were obtained from 40 adult patients undergoing corrective surgery for congenital or aquired heart disease. The materials were randomly divided in two groups with 20 and 20 patients, respectively, Before cadiopulmonary bypass, the same anesthetic technique was applied to 2 groups. During cardiapuhnonary bypass, isoflurane was given into oxygenator at 0.5-1.5% concentration in group I. In group II, no isoflurane was given but 0.2 mg/kg midazolam was supplied into the oxygenator at the beginning of bypass. To both groups small amounts of fentanyl were given during bypass. There were no significant differences in mean cardiopulmonary bypass time and pump flow between the groups.In group I, systemic vascular resistance did not increase until 60 minutes, whereas in group II systemic vascular resistance increased significantly after 30 minutes and maximal increase was noticed at 60 minutes. Data for base excess demonstrated that significant fall after 40 minutes of perfusion in group II, but not changes in group I. It is concluded that isoflurane exerts a beneficial vasodilatory action during caardiopulmonary bypass and hypothermia. And the harzards of local reduction in organic blood flow are ehminated, because an adequate perfusion pressure and flow are maintained by the heart lung machine.

3.Effects of an Education Program on Sanitation Status at Centers for Children's Food Service Management: Focusing on Jung-gu and Dong-gu regions of Daejeon Metropolitan City.

OBJECTIVES: This study aimed to assess the improvement of teachers' sanitation performance and food distribution environment at Centers for children's food service management after a sanitation education program. METHODS: The subjects were 119 teachers working at child care centers registered in the Daejeon Center for Children's Foodservice Management in Dongu and Jung-gu, Daejeon. The sanitation education was provided three times from March to August in 2014, and the survey questionnaires were distributed before and after the education. The sanitation status of food service environment of the centers was examined by ATP(adenosine-5'- triphosphate) bioluminescence. RESULTS: After the sanitation education, the teachers showed higher levels of sanitation knowledge. Also, sanitation performance and recognition level of the importance of it significantly improved after the education program. The mean variation scores of importance and performance after the education were 0.14 and 0.23. According to the Importance-Performance Analysis (IPA) results of 26 sanitation attributes about the food service environment, the selection attributes with relatively low performance and importance were mostly distributed in the children's and distributer's personal hygiene management area. However, all attributes except using a personal water bottle and cup were moved to high performance and importance level. Also, the ATP examination results showed that the sanitation status of the food service environment was improved after the education program. CONCLUSIONS: The sanitation education program was effective in improving the recognition of the importance of sanitation and its performance with regard to food service management.

4.A Case of Desmoplastic Trichoepithelioma.

Desmoplastic trichoepithelioma is a rare benign adnexal tumor with female preponderance, most commonly located on the face. Clinically, the tumor measures from 3 to 8mm in diameter, and has a raised, annular border and a depressed nonulcerated center. The three characteristic histologic features are narrow strands of tumor cells, horn cysts, and a desmoplastic stroma. We report herein a case of desmoplastic trichoepithelioma in a 25-year-old woman who had an asymptomatic hard annular nodule with a raised border and depressed center on the left cheek.

5.Use of the Pill Questionnaire to detect cognitive deficits and assess their impact on daily life in patients with Parkinson’s disease

The Pill Questionnaire (PillQ) has been proposed as a simple way to evaluate cognitive deficits and their
impact on the daily lives of those with Parkinson’s disease (PD) by asking patients or caregivers about
whether patients can independently manage their pills. We used the PillQ to investigate the association
of ability to manage medication with cognition and activities of daily living (ADLs) in patients with
PD. Patients were divided into two groups based on PillQ scores. The no-impact group was able to take
their antiparkinsonian medication independently, and the impact group exhibited problems describing
their treatment or taking their drugs independently. A total of 208 participants (93 men) were included.
111 patients (53.4%) were included in the no-impact group, and 97 (46.6%) were included in the
impact group. The impact group showed significantly lower cognitive functioning, difficulties with
the performance of ADLs, and severe motor dysfunction. PillQ scores were significantly correlated
with Mini-Mental State Examination and the Montreal Cognitive Assessment, and Clinical Dementia
Rating scores. Management of medication by PD patients is associated with cognitive function, and
the PillQ is an easy and useful test for detecting cognitive impairment and its impact on daily life.
6.Alfa-Synuclein polymorphism and Parkinson’s disease in a tau homogeneous population

Background & Objective: The MAPT H1 haplotype and SNCA single nucleotide polymorphism (SNP)
rs356219 have been reported to have a synergistic effect on the risk of Parkinson’s disease (PD).
Because the H1/H1 genotype has been reported to predominate in Korean population, we investigated
the polymorphism of rs356219 in 878 PD patients and 559 controls. Methods: The SNCA SNP rs356219
was analyzed in 878 PD patients and in 559 healthy Korean subjects. Results: The G allele of SNCA
SNP rs356219 was found to contribute to PD susceptibility with odds ratios (ORs) similar to those
reported previously. However, the ORs were not as large as that of the SNCA rs356219 plus MAPT
H1/H1 combination reported in the literature, which cast doubt on the existence of a synergistic effect
between the two genotypes in our population.
Conclusions: This study supports that the G allele of the SNCA SNP rs356219 contributes to PD
susceptibility as reported previously, but it does not support the presence of a synergistic interaction
between SNCA and MAPT.
7.Midcarpal Fusion with Excision of Scaphoid for Scapholunate Advanced Collapse ( SLAC ).

Scapholunate advanced collapse (SLAC) deformity most often occurs with chronic rotary scaphoid instability from scaphoiunate ligament tear and scaphoid fracture. Prior to 1984 when Watson HK reported his concept on SLAC wrist deformity, the most popular procedure for this pathology was scaphoid implant arthroplasty with or without midcarpal fusion. We have reviewed 15 cases of SLAC deformity treated with scaphoid excision and midcarpal fusion from 1984 to 1993. Among them, scaphoid implants made of silicone were inserted in 4 cases. Scaphoid implant arthroplsty without carpal fusion has been excluded. The mean period of follow-up was 8.8 years and the mean age of patients was 47.5 years. Males and right wrists were predominant. Wrist motion was 56% and grip power was 71% of the normal opposite side. On the average overall, 8 patients had no pain and 7 patients had mild pain. The grade of pain was minimal, 0.5 in grade (Rating system: no pain-0, mild- 1, moderate-2, severe-3). However, all of the 4 cases of limited carpal fusion with a scaphoid implant developed silicone particulate induced synovitis and pain. The procedure appears to be effective in maintaining function of the hand with minimal pain up to 5.8 years. However, silicone scaphoid implant appears not to be necessary and is not recommended with a limited carpal fusion for this pathology.

8.The Influence of Emotional Labor, Positive Psychological, Capital, and Rewards on the Retention Intention among Coronavirus Disease 2019 Ward Nurses in Regional Public Hospitals

Purpose:
The purpose of this study was to analyze the influence of emotional labor, positive psychological capital, and rewards on the retention intention among coronavirus diseases 19 (COVID-19) ward nurses in regional public hospitals.
Methods:
The subjects were nurses who directly participated in nursing while wearing protective gear (level D or higher) in a COVID-19 ward. Data were collected from March 15 to March 29, 2021, and a total of 187 questionnaires were analyzed. The data were analyzed with descriptive statistics, independent t-test, one-way analysis of variance, Pearson’s correlation coefficient, and multiple regression using SPSS for Windows version 27.0.
Results:
The mean scores of emotional labor, positive psychological capital, and retention intention were 3.48, 3.74, 3.22, and 5.75. The factors influencing retention intension were positive psychological capital (β=.28, p<.001), and the explanatory power for retention intention was 20.0%.
Conclusion
These results indicate that in order to increase retention intention, it is necessary to foster nurses with high positive psychological capital who can make good use of positive emotions, and additional research is needed to clarify the factors influencing nurses’s retention intention to remain in the organization.
9.The Usefulness of Preoperative Ultrasonography on Decision of Operative Extent in Patients with Papillary Thyroid Microcarcinoma.

PURPOSE: Usually papillary microcarcinoma (PMC) grows very slowly with a good prognosis, although it frequently metastasizes to regional lymph nodes and shows multiple tumor formation in the thyroid. Therefore, how to treat papillary microcarcinoma has been controversial. Recently several studies reported that some ultrasonographic features may potentially reflect the biological aggressiveness of a lesion. We investigated which ultrasonographic findings can reflect aggressive characteristics and whether US can helpful in selecting the appropriate surgical treatment of PMC. METHODS: We retrospectively reviewed the preoperative ultrasonographic findings and pathologic risk factors of 68 patients who had undergone surgical treatment for PMC at the Wallace Memorial Baptist Hospital from January 2004 to December 2005. RESULTS: The incidences of multifocality, extrathyroidal extension, and lymph node metastasis of PMC were 42.6%, 48.5% and 20.6%, respectively. The Mean sizes of PMC were no significant differences according to age, multifocality, extrathyroidal extension, lymph node metastasis, stage and AMES risk group, and tumor size more than 5 mm was not linked to pathologic prognostic factors. Cases demonstrating multiple nodules in the unilateral or bilateral lobes, as well as those with fine strong calcifications in the tumor on US, were directly linked to multifocality on pathologic finding. US is insensitive technique for detecting lymph node metastasis and it's sensitivity was only 14.3%, but it's positive predictive value and specificity were high (100%). CONCLUSION: The date suggested that complete surgery with appropriate nodal dissection should be performed in patients with PMC which their US demonstrating multiple nodules in the unilateral or bilateral lobes, fine strong calcifications echoes in tumor and US-detected node metastasis.
