1.Clinical and Histopathological Observation on Pityriasis Rosea.

The present study was performed to evaluate clinical manifestations and histopatological findings of 95 patients with pityriasis roses who visited the Department of Dermatology in National Medical Center from October of 1979 to September of 1982. The results are as follows, 1. Clinical Manifestations: 1. The number of patients with pityriasis roses were 95, which occupied 0. 98/o of new out-patients in the department of dermatology in the same period. 2. In sex distribution, males were 36 and females were 59, with male to female ratio of I: l. 6. R. Age of onset varied from 13 months to 58 years with a mean of 26. 0 years and 68% of cases belonged to 15 years to 84 years. 4. Seasonal incidence was 32% in winter, 27% in summer, 22% in autumn and l9% in spring. 5. iMild prodromal symptoms developed in 8 cases(%) and itching was noted in 58 cases (61%), which was mild to moderate exeept for severe itching in 2 cases. 6. Herald patch was found in 42 cases (44%) and the predilection sites were the trunk and the thighs. 7. Daughter lesions could be classified into macular type 69 cases (73g), pa.pular type 22 cases (23%), pustular type 2 cases (2%), urticarial type 1 case (I,3 and purpuric type 1 case (1%) They appeared on the back (79%), the chest (74%), the abdomen(73%), the thighs (47%), the upper arms (40%), the lower legs (24%),the forearrns (22%), the neck (16%), the face (7 %), dorsa of the hands (4%) and dorsa of the feet(2%). 2.Transplantation of Cultured Keratinocytes in Autologous Fibrin Glue Suspension.

The use of a cultured autologous keratinocyte sheet has become a recognized method for the coverage of extensive bums during recent years. The disadvantages of these sheet grafts are a long time-lag until keratinocyte sheets are available, the fragility and difficulty in handling of grafts, an unpredictable take rate and extremely high costs. In this study we investigated the transplantation of cultured keratinocytes as single cells suspended in autologous fibrin glue. In a rat model with standardized full thickness wounds, this new transplantation technique was evaluated and compared directly to the conventional keratinocyte sheet grafting technique. After transplantation, wounds were evaluated for the degree of epithelial coverage, and then microscopic structures were evaluated under light and electron microscopy. The results were as follows: 1) The fibrinogen solution prepared from autologous blood had 12 times more fibrinogen compared to the original blood. 2) After transplantation of cultured keratinocyt-es in fibrin glue, the degree of epithelial coverage was 79% at 2 weeks, which was comparable to 17% for cultured keratinocyte sheet graft 3) Typical basement membrane structures were consistently found at 2 weeks after transplantation of keratinocytes in fibrin glue. 4) Rete ridges were found at 4 weeks after transplantation of keratinocytes in fibrin glue. In conclusion, the transplantation technique of keratinocytes in fibrin glue is available earlier than sheet grafts, it transfers actively proliferating cells and it simplifies the grafting procedure. As well, this technique leads to an earlier epithelial covering and an earlier restoration of the dermo-epidermal junction than sheet grafting.

4.The Effects of Long-term Care Hospitals’ Nurse Staffing Level on Patient Outcomes: Differences according to Region

Purpose:
The aim of this study was to examine the differences in nurse staffing levels and patient outcomes in long-term care hospitals by region and to identity the effects of nurse staffing level and registered nurse proportion on patient outcomes.
Methods:
A secondary analysis of national data that included general characteristics of hospitals and long-term care hospitals’ evaluation results from the Health Insurance Review and Assessment Services was conducted, and 1,205 hospitals were selected.
Results:
Results showed that the mean number of patients per nursing staff and registered nurse proportion were 4.27 and 0.43, respectively. The differences in long-term care hospitals’ evaluation results and nurse staffing levels according to region were significant. A logistic regression analysis showed that the number of patients per nursing staff affected the probability of the patient experiencing a decline in daily life activities, as well as a decline in the outcomes of patients who were non-dementia; registered nurse proportion affected the outcomes of patients with dementia.
Conclusion
The findings suggest that implementing policies to improve long-term care hospitals’ nurse staffing level and registered nurse proportion is important. Ensuring mandatory registered nurse staffing levels based on the severity of patients’ diagnoses is also necessary.
5.Fracture and Bilateral Facet Dislocation after Anterior Cervical Fusion in a Patient with a Distraction-Flexion Injury at the Lower Cervical Level

A bilateral facet dislocation after anterior cervical fusion with a plate in distraction-flexion injury without significant instability in plain radiography is very rare. This paper reports a case with a confirmed injury status (C6–7 and C7–T1) by magnetic resonance imaging and computed tomography with no visible fractures or displacement on plain radiography. Anterior cervical fusion (C6–T1) was performed with a cage and plate. On the other hand, three days later, C6–7 bilateral facet dislocation developed with severe neck and radiating pain, and posterior decompression and fusion were performed.

Neomorphogenesis of cartilage using chondrocyte-polymer constructs is a potential source for development of cartilage reconstruction. Current tissue engineering techniques of neocartilage rely on in vivo implantation of polymer-chondrocyte constructs. The purpose of this study was to find a way to bioengineer cartilage in vitro by entrapping chondrocytes in a molded autologous fibrin glue. Chondrocytes isolated from the cartilage of rabbit joints were combined with fibrinogen extracted by a single cryoprecipitation of autologous plasma, and they were then polymerized with thrombin to create a fibrin glue with a final cell density of 2.5x10(6) cells/ml. The collagen for a control study was used as a polymer. The polymer-chondrocyte constructs were cultured for 4 weeks and the fibrin-chondrocyte constructs molded in the shape of a human ear were cultured for 6 weeks in vitro. Morphometric, histochemical, and histomorphometric analysis including glycosaminoglycan quantitation confirmed the following results: 1) Highly-concentrated autologous fibrinogen was easily extracted by a single cryoprecipition of autologous olasma. 2) The fibrin-chondrocyte constructs demonstrated the presence of actively proliferating chondrocytes with the production of cartilaginous matrix(collagen and glycosaminoglycan) at 1 week after culture, as well as gross and histologic evidence similar to those of normal cartilage at 3-4 weeks after culture. 3) The collagen-chondrocyte constructs demonstrated lower degrees of hardness and transparency, as well as a lower density of cells and glycosaminoglycan during the culture period. 4) Neocartilage generated from fibrin-chondrocyte constructs in the shape of a human ear nearly retained their original configuration and size without degeneration for 6 weeks of culture in vitro. This study demonstrated a novel method for bioengineering the molded cartilage in vitro using autologous fibrin glue as a matrix scaffold. The generated cartilage showed gross and histologic evidence similar to those of normal cartilage, retaining the original gross dimension. With further refinement, this may be a new application of tissue engineering for the reconstruction of cartilage.

8.MR Imaging Findings of Avulsion Fracture of the Tibial Spine of the Knee, Focusing on Cruciate Ligament Tear.

PURPOSE: To determine the presence of cruciate ligament tears following avulsion injuries involving the ACL and PCL, and to correlate the findings with those of surgery. MATERIALS AND METHODS: Between March 1997 and May 2002, avulsion injury involving the ACL or PCL was diagnosed in 19 patients. Ten of these [8 males and 2 females aged 10-51 (avergae, 27.7) years] were included in this study. We assessed the presence of cruciate ligament tears at MR imaging, correlating the findings with those of surgery. Associated intra-articular injuries, treatment methods and follow-up results were also evaluated. RESULTS: Among Seven patients with ACL avulsion injury, this was assessed at MR imaging as complete tear (n=1), partial tear (n=5), or intact (n=1), while all MR images of PCL avulsion injury (n=3) showed that this was partial tear. All imaging findings corresponded with the surgical findings. In four patients there was associated knee injury involving, respectively, tears of the medial meniscus, lateral meniscus, PCL and MCL, and popliteal ligament. CONCLUSION: Our findings showed that with one exception, patients with avulsion injury of the ACL or PCL had suffered either a partial or complete tear. MR imaging may be useful in the diagnosis of tears of the cruciate ligament which have not been noticed at surgery or arthroscopy in avulsion injuries involving the ACL and PCL.

9.Radiation Therapy of Maxillary Sinus Cancer.

PURPOSE: Maxillary sinus cancers usually are locally advanced and involve the structures around sinus. It is uncommon for this cancer to spread to the regional lymphnodes. For this reason, local control is of paramount important for cure. A policy of combined treatment is generally accepted as the most effective means of enhancing cure rates. This paper reports our experience of a retrospective study of 31 patients treated with radiation therapy alone and combination therapy of surgery and radiation. MATERIALS AND METHODS: Between July 1974 and January 1992, 47 patients with maxillary sinus cancers underwent either radiation therapy alone or combination therapy of surgery and radiation. Of these, only 31 patients were eligible for analysis. The distribution of clinical stage by the AJCC system was 26%(8/31) for T2 and 74%(23/31) for T3 and T4. Eight patients had palpable lymphadenopathy at diagnosis. Primary site was treated by Cobalt-60 radiation therapy using through a 45degree wedge-pair technique. Elective neck irradiation was not routinely given. Of these 8 patients, the six who had clinically involved nodes were treated with definite radiation therapy. The other two patients had received radical neck dissection. The twenty-two patients were treated with radiation alone and 9 patients were treated with combination radiation therapy. The RT alone patients with RT dose less than 60 Gy were 9 and those above 60 Gy were 13. RESULTS: The overall 5 year survival rate was 23.8%. The 5 year survival rate by T-stage was 60.5% and 7.9% for T2 and T3, 4 respectively. Statistical significance was found by T-stage (p<0.005). The 5 year survival rate by N-stage was 30% for N(-) and 8.3% for N(+), but statistically no significant difference was seen(p30.1). The 5 year survival rate for RT alone and combination RT was 22.5% and 27.4%, respectively. The primary local control rate was 65%(20/31). CONCLUSION: This study did not show significant difference in survival between RT alone and combination RT. There is still much controversy with regard to which treatment is optimum. Improved RT technique and development of multimodality treatment are essential to improve the local control and the survival rate in patients with advanced maxillary sinus cancer.
