1.Compartive Accuracy of the SRK II, SRK/T and Holladay Formulas for Intraocular Lens Power Calculation in Severe Myopic Eyes .

The predictive accuracy of the SRK II, SRK/T and Holladay formulas was evaluated in 22 eyes with sever myopia who had phacoemulsification and implantaton of posterior chamber IOL without suture. These patients were followed up for at least 6 months postoperatively. Correlation coefficient between the predicted refraction and actual refraction was 0.57(P<0.01) in the eyes using the SRK/T, 0.566(P<0.01) in the Holladay and (0.06(P=0.791) in the SRK II. The mean absolute error in the SRK/T 0.85 was smaller than that of the Holladay 1.04 and the SRK II1.08(p<0.05). The percentage of the eyes with absolute error within 1D was 68% in the SRK/T, 68% in the Holladay and 53% in the SRK II, but there was no statistically significant difference between them(p>0.05). The percentage of the eye with hyperopic shift(more hyperopic actual refraction than the predicted refraction) was 77% in the SRK II, 77% in the SRK/T and 86% in the Holladay, but there was no statistically significant difference between each formula(p>0.05). In conclusion, the SRK/T has the better predictive accuracy than the SRK II and Holladay in severe myopia and hyperopic shift tends to occur with the application of all three formulas.

2.Arthroscopic Treatment for Septic Arthritis of the Shoulders in Neonates: A Case Report.

Septic arthritis in neonates is a rare condition. A failure to make an early diagnosis of septic arthritis in neonates may leave a permanent disability as a result of a delayed treatment. Thus, septic arthritis, requires a prompt diagnosis and a timely treatment especially in this subset of patients. In this case report, we describe our treatment protocol for septic arthritis and concurrent osteomyelitis in the right shoulder of a 28-day-old newborn. Using 2.4 mm wrist arthroscopy, we performed an arthroscopic irrigation and drainage, to remove intra-articular debris and inflammatory tissue, and multiple drilling. We report a satisfactory clinical outcome without any postoperative complications or side effects.

3.Treatment of Neovaseular Glaucoma Using Modified Schocket Implant.

Seven eyes of 7 patients had undergone modified Schocketimplant surgery for neovascular glaucoma. The mean preoperative intraocular pressure(IOP) was 49.1 +/- 10.1 mmHg(range 38 to 70 mmHg). With a follow up of 6 to 18 monnths, the mean postoperative IOP was 20.3+/-5.1 mmHg(range 16 to 30 mmHg). The author's personal method to make two oblique valve incisions on the obliterated distal end of the draining silicone tube, prevented early postoperative complications(flat anterior chamber and hypo-tony). The one oblique incision on silicone tube turned into a open valve at a pressure between 17.3 and 21.6 mmHg(average 19.3 mmHg) and two oblique incisions opened at a oblique incisions seemed to be more available for pressure sensitive valve. The complications included minimal hyphema(4 eyes) and progression of cataract(1 eye). Modified Schocket implant are an effective and simple technique in the management of neovascular glaucoma.

5.Radiologic analysis of the spinal tuberculosis

Tuberculosis remains high incidental disease in Korea with an estimated incidence of 2.5% in generalpopulation. Among the tuberculosis of bone, spinal tuberculosis is high incidence and curable disease, but earlytreatment demands early diagnosis. Authors reviewed clinical aspects of 376 cases, and reviewed conventional filmsof 74 cases and CT of 8 cases, confirmed histopathologically as spinal tuberculosis from Jan. 1976 to May 1985 atYonsei medical center, Yonsei University. The results were as follows: 1. The frequent site of involvement werelower thoracic and lumbar vertebra, 4th lumbar vertebra was the most common lesion site among them. 2. Theseperated lesions were 10.2% among spinal lesion. 3. The most common type and pattern of bone density wasintervertebral type and mixed pattern each other. 4. Paravertebral abscess, kyphosis and narrowing ofintervertebral disc space were discovered more than 80% in reviewed conventional films. 5. In children, there isno predilection site. 6. Spinal CT was more accurate diagnostic method than conventional study in the evaluationof following aspects: 1) extent of lesion 2) degree of spinal canal involvement 3) changes of surrounding organ.

6.Spinal Canal Remodelling after Stabilization of Thoracolumbar Burst Fractures.

About half of all burst fractures at the thoracolumbar junction lead to neurological impairment and several clinical series have demonstrated a statistically significant correlation between canal encroachment and neurologic impairment, but not directly related. Spontaneous canal remodelling over time due to bone resorption has been observed in conservatively treated burst fractures. The aim of this study was to measure spinal canal remodelling after stabilization of burst fractures. So, we evaluated 22 cases of surgically stabilized burst fractures of thoracolumbar junction about pre and postoperative spinal canal stenotic ratio and canal remodelling by bone resorption over time. The results were as follows; l. Pedicle splaying increases the spinal canal area and necessitates correction. 2. Patients with neurological deficits had average 53% encroachment and the neurological normal patient had a canal compromise of 33.9%. 3. Postoperatively canal encroachment had decreased to a mean of 17.4% and further reduced by resorption of bony fragment to a mean of 8.3% within 14 months. In conclusions, remodelling of the spinal canal by resorption of encroaching bone fragments is a consistent feature in surgically stabilized thoracolumbar burst fractures and most patients regain their prefracture canal demensions within 14 months.

7.Hydrogen Peroxide Production in Neutrophils after Tourniquet Release

The use of lower extremity tourniquets for procedures of the lower leg is considered routine in orthopedic surgery, but, lower extremity tourniquets do harm occasionally. While the tourniquet is inflated, metabolic changes such as increased PaCO2 , lactic acid, and serum potassium and decreased level of PaO2 and pH occur in the ischemic limb. Deflation of tourniquet results in release of anaerobic metabolic products during ischemia into systemic circulation. In this ischemia/reperfusion situation, oxygen free radicals could potentially be produced during the reperfusion period by several mechanisms. One of these mechanisms is release of intracellular superoxide or hydrogen peroxide by activated neutrophils in the area. These reactive oxygen species(ROS) could be a causative factor for the postreperfusion no-flow, lung injury, induction of tourniquet shock, etc. The purpose of this clinical study was to investigate the effect of tourniquet deflation on the hemodynamic changes, changes of blood gas analysis, and hydrogen peroxide production using flow cytometric analysis of fluorescent DCF(Dichlorofluorescein). Quantitative analysis of fluorescent DCF was performed in resting and fMLP(N-formyl-methyonyl-leucyl-phenylalanine) or PMA(phorbol myristate acetate) stimulated neutrophils. Also differences of these factors between two groups of tourniquet time, one is less than one hour and the other more than one to two hours, were analysed. The hemodynamics(blood pressure, pulse rate), arterial PO2, bicarbonate, base excess, and hydrogen peroxide production showed no significant change before and after tourniquet release(p>0.05). Arterial pH and PaCO2 decreased significantly until 10 and 5 minutes after tourniquet release, respectively(p>0.05). Tourniquet time didn’t reveal any significances differences. These results indicate that tourniquet application with400mmHg pressure and less than 2 hours does not release significant hydrogen peroxide into systemic circulation during reperfusion period after tourniquet release.

9.A Case of Eosinophilic Pustular Folliculitis (Ofuji's Disease) in a Newborn.

We describe a case of eosinophilic pustular folliculitis in a 1-month-old Korean male infant. The patient was suffering from erythematous papules and pustules affecting the scalp, trunk, and extremities, detected at birth. The flare was accompanied by leukocytosis and eosinophila. Histopathology revealed folliculitis with a predominant eosinophilic infiltrate. Lesions showed partial response to topical steroid and disappeared in four weeks. Three months after the resolution of the lesions, he presented a similar clinical picture as the previous episode.

10.Membrane activator of the 72 kDa type IV collagenase in malignant breast carcinoma patients: Expression of membrane-type 1-matrix metalloproteinase (MT1-MMP).

In this study, we determined proMMP-2 activating capacity of membrane extract prepared from the tissue of invasive ductal carcinoma of breast by zymogram gel analysis. We compared the effect of membrane extract on the activation of the latent type IV collagenases with that of the organic mercurial compound leg, APMA)-induced self cleavage of the latent type IV collagenases. We also compared the expression levels of MT1-MMP between invasive carcinoma and normal tissue by Western blot, Northern blot and semi-quantitative RT-PCR analysis. Our result demonstrated that the specificity of processing by breast carcinoma membrane activator corresponds to the specificity of MT1-MMP, which clearly showed the conversion of 72-kDa proMMP-2 to the activated form while APMA processed both 72- and 92-kDa proMMPs to their activated forms. MT1-MMP protein and mRNA were expressed both in invasive carcinoma and normal tissues, and the expression levels in both tissues were comparable. Quantitative analysis of the mRNA level by RT-PCR revealed that the difference of MT1-MMP mRNA between carcinomas and normal tissues was not statistically significant on Wilcoxon signed-ranks test (P>0.05). The results from the study on the expression of MT1-MMP gene suggest that the cellular activation of MMP-2 in breast tissue, requires additional effects in addition to up-regulation of MT1-MMP.
