3.Problems in Humeral Interlocking with Seidel Nail

Seidel nail is one of locked humeral nail and Seidel himself reported excellent result in his preliminary report using his nail. But our early experience showed disappointing result with high rate of complications, So we reviewed our 16 cases of humeral shaft fracture with polytrauma treated with Seidel nail and studied anatomical fitness of the nail within the distal medullary space using CT scan. The most frequent complication was intraoperative fracture of the proximal humerus < 4 cases> and this seemed to be due to lateral placement of pilot hole. There were 3 cases of nonunion and these cases showed insufficient distal anchorage, either primarily or postoperatively. Anatomical fitness study showed insufficient spreading of nail flanges in 5 cases of distal canal width below 9mm and nonunion cases were in this group. Other problems of Seidel nail were technical difficulties due to occasionally incorrect proximal targeting device and frequent breakage of adaptor screw. Study of more cases may be needed to confirm the design problem of Seidel nail.

4.Congenital Syphilis: Hematologic Findings of Early Congenital Syphilis.

A retrospective study was made of the previous medical records of 47 infants with congenital syphilis born from mothers with untreated syphilis at Severance Hospital, Yonsei University College of Medicine during the years 1972 through 1983 The VDRL titer of these infants was higher than 1: 8 with reactive FT.A-ABS test. They were subjectd to hematologic examinations and the results were as follow s, 1. Of 46 infants with congenital syphilis, anemia was observed in thirty-one (68%) Reticulocytosis occurred in seventeen(89%) of nineteen infants with congenital syphilis having the mean value of 6. 0%. 3, Of 4.7 infants with congenital syphilis, leucocytosis was noted in seveenteen 4, Thrombocytopenia was observed in twenty (64%) of thirty-one infants with congenital syphilis.

10.Incidental gallbladder opacification after intravascular contrast infusion.

Opacification of the gallbladder after parenteral administration of contrast material usually indicates diminished renal function but can occur with normal renal function. The authors recently experienced 13 cases of such vicarious excretion of contrast media. Among 13 patients. 9 patients had renal disease unilateral ureteral stone(n=3), staghorn calculi(n=2), acute renal failure from sepsis(n=1), renal contusion(n=2) and unilateral renal artery involvement by dissecting aortic aneurysm(n=1). Of these 9 patients, 3 patients showed abnormal serum creatinine levels at the time of presentation or shortly thereafter. In 6 patients, injection of a large amount of contrast media was possibly the additional cause of vicarius excretion. There was no detectable cause in the remaining 4 patients. Heterotopic excretion of contrast media is clearly a complex phenomenon, the reason for which it is difficult to establish with certainty in each individual case. Anyhow, vicarius excretion of contrast media occurs more frequently than previously thought, and it can occur not only with abnormal renal function but with normal function also.
