1.A Case of Polyagglutination due to T Activation.

Red blood cells that agglutinate with most normal adult sera but never with own sera are termed polyagglutinable and can be separated by patterns of lectin reactivity into various types. Among these polyagglutination, activation of the T cryptantigen occurs when carbohydrate structures on glycophorins A and B lose sialic acid and express the disaccharide Gal beta-l-3 GalNac which reacts with the peanut agglutinin, a lectin from Arachis hypogaea. T activation is a temporary condition due to exposure of the membrane antigen to the action of microbial neuraminidase. In T activated red cells, the following hazards, which are theoretically possible, are spontaneous polyagglutination of red cells in vitro, in vivo and severe blood transfusion reactions. We experienced a case of T activation in 6 month old girl with bacterial meningitis caused by Streptococcus pneumoniae. The reactivity to lectins indicated the patient's red cells were T activated. We report a case of T activation in an infant with the review of literature.

2.Office Blood Pressure is Higher than Home Blood Pressure, and Digital Electronic Sphygmomanometer is Useful for Self-Monitoring of Blood Pressure in Hypertensive Patients.

BACKGROUND: To investigate the possibie difference, if any, between office blood pressure(BP) and home BP may be important in the diagnosis and treatment of hypersensive patients. This report deails the difference between the two BP's and the usefulness of digital electronic sphygmomanometer(DES) for self-monitoring of home BP. METHODS: The BP's of 14 patients with essential hypertension were measured with mercury sphygmomanometers at outpatient department by physician and with DES at home(twice a day) by the patients. Patients were followed up every 2 weeks for 4 weeks and previous 2 weeks' average home BP's were compared with the office BP's of each 2 weeks' end. RESULTS: There was a significant difference between office and home BP(both systolic and diastolic) ; office average BP(151/95mmHg) was higher than home average BP(136/86mmHg). CONCLUSION: Caution may be needed in the interpretation of office BP unless it is measured several times after adequate rest.

3.The Study of Placing Clerkship of Medical Students.

The medical curriculum is designed to provide a core of scientific knowledge and technical skills that ultimately enables a student to care for primary patients. In addition to specific knowledge, the school offers an environment in which students have the opportunity to acquire the habits of study, the sound attitud, and the sense of responsibility for patients that characterize the true physician. To achive these purposes, the clerkship of medical student is very important. We investigated current problems in clinical clerkship of Chonnam University Medical School, and the problems were as follow: (1) relatively short period of clinical training especially in main subjects, (2) lack of presonnels in charge of clinical training, (3) lack of objective test, (4) lack of clinical training in primary care hospital, (5) lack of opportunity of self learning. The methods to improve clinical training were to promote interests in clinical education of staffs, to prolong the duration of clinical clerkship, to select and educate the personnel in charge of clinical education, to do the objective tests, to do the elective tests, to do the elective course, to use of integrated course and problem oriented education, and to let the student care the patients under supervision.

4.Surgical Treatment of the Acute Anterior Cruciate Ligament Injuries

The anterior cruciate ligament(ACL) is one of the major stabilizers of knee motion. The ACL does not heal once torn, unlike many other biological tissues. Its absence can lead to severe joint instability which can lead to disruptions of secondary restraints around the knee and then progress to degenerative joint disease. Thus many authors advocate surgical intervention for a torn ACL in order to restore stability to the joint. Yet, there is still controversy as to best treatment for the ACL injuries. It is desirable to be considered for the patients age, the severity of the injury, and the future activity level of the patient to decide treatment. We analyzed thirty ACL acute injuries of twenty seven patients who had an ACL injury and treated surgically between Apr. 1986 and Dec. 1991 at St. Pauls Hospital. The results were as follows: 1. There were twenty five males and two females. The mean age was 33.4 years ranged, 15 to 52 years. 2. The anterior cruciate ligaments were torn at proximal portion in twenty two cases, at midportion and distal portion in four, respectively. 3. Two knees had isolated ACL injuries and twenty eight had combined injuries. 4. Twenty one out of twenty six cases treated with primary repair, and three out of four cases treated with reconstruction had good-excellent results.

5.The Macular Circulation State on BRVO According to Occlusion Site.

We did clinical analysis on the macular circulatory state in 34 patients(34 eyes)who were diagnosed to have branch retinal vein occlusion and followed for over 6 months. After dividing them into two groups according to their occlusion sites, we evaluated the differences of foveal capillary ring state and size of macular ischemic area between the ones who had vein occlusion in their first retinal branches and the others in their second and the rest retinal branches. We were unable to find any differences between them. However, we could find statistical correlation between their visual acuities in acute stage of the disease and macular circulatory states even though we could not find any differences between their final visual acuities or degree of their visual improvement and circulatory states during follow up period.

6.Observational Analysis of Korean Society of Spine Surgery Annual Congress: (2001-05) Free Paper: From Presentation to Publication.

STUDY DESIGN: A review of all presentations at the Korean Society of Spine Surgery(KSSS) congress from 2001 to 2005. OBJECTIVES: To determine the presentation to publication rate in peer-reviewed journals of abstracts presented at the KSSS congress and evaluate the consistency between the presented abstract and published full-text counterpart. SUMMARY OF LITERATURE REVIEW: The rate of publication for presentation at international meetings has been determined but there has been no study for the KSSS. MATERIAL AND METHODS: This study reviewed all podium presentations of the past KSSS Spring and Fall meetings from 2001 to 2005. A computer search for each abstract was performed. The final published journal, time to publication and publication rate were evaluated according to subspecialty. The published full-text article was compared with the original abstract and evaluated for consistency with respect to the author, material and method, and result. RESULTS: A total of 288 free-papers were identified, of which 167 of them were published as full-text articles. The mean time to publication was 12.15 months. The publication rates according to the subspecialty were basic 67%, lumbar 68%, cervical 64%, deformity 50%, trauma 53%, minimal 14% and others 51%, respectively. CONCLUSION: The presentation to publication rate of the Korean society of spine surgery podium presentations was lower than other international meetings but the articles showed a high consistency rate.

7.A Study on the Accuracy of the record base of the Complete Denture to the Master Cast according to Kinds of Resin and Polymerization Method.

STATEMENT of THE PROBLEM: The record base in fabricating procedures of the complete denture, as a temporary form for reproducing denture base, is used to record upper and lower jaw relation and to align artificial teeth and try-in it in the mouth. The accuracy of jaw relation record is affected by the accuracy, stiffness and stability of the record base. So, the accuracy of record base is the most important requirements of jaw relation records. PURPOSE of STUDY: The purpose of this study was to evaluate the gap that occurred over the palatal area of a maxillary record base fabricated with autopolymerizing resin and light-curingresin. METHODS AND MATERIAL: The maxillary record bases were fabricated out of autopolymerizing resin that is used the most frequently in clinics and light-curing resin that attracts special attentionfor its several merits. The light-curing resin was made by two kinds of polymerization methods, which are one step curing method and multiple step curing method. All record bases were cut in certain positions of the master cast 1 hour and 1 day later after fabrication and the accuracy of the master cast was measured and analyzed with a microscope. RESULTS: A pattern of gap formation between the record base and the maxillary cast was observed in all specimens. According to kinds of resins, autopolymerizing resin was significantly more accurate than light-curing resin. There was no statistical difference according to time lapse, and in all three groups, the maximum discrepancy occurred at the posterior border in the midpalatal region. CONCLUSLON: The autopolymerizing resin is better than light-curing resin, and multiple step curing method is more accurate than one step curing method when using light-curing resin.

8.Effect of Estrogen Replacement on Vascular Responsiveness in Ovariectomized Spontaneously Hypertensive Rat.

BACKGROUND: Although postmenopausal estrogen replacement therapy is known to reduce cardiovascular mortality, the mechanism is not clear yet. Furthermore, the effect of estrogen on vascular tonus is reportedly variable according to the animal models, vascular beds and agonists used. MATERIALS AND METHOD: Bilateral ovariectomies were performed in 12 week-old, 18 spontaneously hypertensive rats (SHR) and 18 normotensive Wistar-Kyoto rats (WKY). Rats were divided into three groups according to the dose of 17beta-estradiol (E 2 ) pellets implanted subcutaneously two weeks after ovariectomy: control (no implantation), low-dose (0.5 mg) and high-dose (5 mg) E 2 replacement group. Two weeks after pellet implantation, organ bath experiments were performed using descending thoracic aortae. For endothelium-dependent relaxation, acetylcholine (10(-9) -3x10(-6) M) was cumulatively added into the vessels precontracted with 10(-7) M norepinephrine (NE). For vasoconstrictor responses, cumulative concentration-contraction curves were constructed in quiescent vessels using NE (10(-9) -10(-5) M), U46619 (10(-9) -3x10(-6) M), endothelin-1 (10(-10) -10(-7) M). In addition, contraction to angiotensin II (10(-7) M) was also obtained. Serum 17beta-estradiol levels were measured by radioimmunoassay. Blood pressure was measured by tail-cuff method in some SHRs before ovariectomy and after placebo/E 2 replacement. RESULTS: Endothelium-dependent relaxation to acetylcholine was impaired in WKY treated with 5 mg E 2 (pIC 50 : control vs 5mg E 2 : 7.75+/-0.13 vs 7.27+/-0.16: n=6: p<0.05). No significant effect was noted in SHR. Contraction to angiotensin II was inhibited by low-dose E 2 in WKY and high-dose E 2 in SHR (% of the contraction to 60 mM KCl: WKY: control vs 0.5 mg E 2 : 39+/-5 vs 25+/-2: SHR: control vs 5 mg E 2 : 34+/-4 vs 22+/-2: n=6 and p<0.05 in WKY and SHR). In contrast, NE-induced contraction was enhanced by E 2 replacement (both low- and high-dose) in WKY and SHR (WKY: control vs 0.5 mg E 2 vs 5 mg E 2 : AUC: 280+/-24 vs 387+/-26 vs 374+/-25: maximal contraction: 137+/-8 vs 166+/-8 vs 162+/-3: pD 2 : 7.63+/-0.11 vs 8.17+/-0.13 vs 8.13+/-0.13: SHR: control vs 0.5 mg E 2 vs 5 mg E 2 : AUC: 265+/-17 vs 349+/-16 vs 406+/-19: maximal contraction: 152+/-6 vs 181+/-9 vs 203+/-16: pD 2 : 7.45+/-0.13 vs 7.91+/-0.08 vs 8.04+/-0.04: n=6 and p<0.05 between control and treated groups in WKY and SHR for all parameters). Contraction to U46619 was enhanced by E 2 replacement in SHR (control vs 0.5 mg E 2 : AUC: 478+/-30 vs 574+/-23: maximal contraction: 181+/-9 vs 230+/-10: n=6: p<0.05 for both parameters). Maximal contractile response to endothelin-1 was also enhanced in SHR (control vs 0.5 mg E 2 vs 5 mg E 2 : maximal contraction: 165+/-7 vs 189+/-7 vs 199+/-8: n=6 and p<0.05 between control and treated groups) but not in WKY. Blood pressure was not different between placebo and E 2- treated SHR (171+/-2 vs 174+/-4 mmHg). CONCLUSION: In WKY, chronic high-dose estrogen replacement impairs endothelium-dependent relaxation to acetylcholine.: low-dose estrogen replacement does not affect endothelium-dependent relaxation in SHR and WKY. Estrogen replacement enhances the contraction to most of the contractile agonists tested except angiotensin II in both WKY and SHR. These results suggest that estrogen replacement affect the vascular tonus differently according to the vasoactive substances and/or hormones without significant effect on blood pressure.

9.Renal Replacement Therapy in Korea: Insan Memorial Registry 1997.

The status of renal replacement therapy in Korea as follows ; 1) The response rate to questionaire was 57.9% of the 226 hemodialysis centers in Korea and data file was sent in the 62.4% of responder. 2) The number of dialysis center was 226 and the number of hemodialysis machine was 3,507 and annual increasing rate were 8,7%, 14.0%, respectively. 3) The total number of ESRD patients was 20,244 and prevalance and incidence of ESRD were 431.9 and 101.1 patients per million population(ppm). 4) The proportion of renal replacement therapy was hemodialysis(HD) 53.3%, peritoneal dialysis(PD) 17.196, renal transplants(KT) 29.5%. 5) The proportion of long-term and elderly patients was markedly increased comparing 1985 (long term dialysis 20% vs. 2%, over 60 year old patients 26.2% vs. 13.8%. 6) The primary cause of renal disease was diabetic nephropathy 34.09o, chronic glomerulone-phritis 20.8%, hypertensive nephropathy 15.7%. 7) The mean BMI(body mass index,' kg/M)in over 18 year old patients was 20.7+/-2.7 kg/M in the HD and 22.23.0 kg/M in the PD. 71.9M of HD patients and 73.2% of PD were in the normal range (18.5-24.9). 8) The fistula survival rate was 6 month 93.4%, 1 yr 91.7%, 3 yr 87.6%, 5 yr 84.7%. 9) The cause of death were cardiac disease 31.8%, miscellaneous 23.5%, vascular disease 17.2%, infection 14.2%, liver disease 1.2%, social cause 5.9%.

10.Expression of Spinal c-fos in a Rat Model of Postoperative Pain.

BACKGROUND: The oncogene, c-fos, is a useful marker of nociceptive neurons activated by various types of pain.However, the relationship between c-fos and acute pain induced by surgical incision has not been studied.The purpose of this study was to describe the expression of spinal c-fos in a rat model of postoperative pain. METHODS: Sprague-Dawley rats were used to enumerate spinal fos-like immunoreactive (fos-LI)-positive neurons after making sham, skin, or skin-muscle incisions on the plantar surface of the rat hindpaw. The spinal cords were divided into the following 4 segments for immunohistochemical detection of fos-LI at 3 time points after the incision (4 hours, POD1, and POD3):thoracic, upper lumbar, lower lumbar, and sacral.For analysis of the laminar distribution of fos-LI, the dorsal horn was divided into three regions (I-II, III-IV, and V-VI). RESULTS: The number of fos-LI was increased in the ipsilateral spinal dorsal horn in the lower lumbar segment 4 hours post-incision and was significantly greater in rats with skin-muscle incisions than in rats with skin-alone incisions.In the skin-muscle group, laminae I-II and V-VI had a greater number of fos-LI neurons than laminae III-IV.The increase in number of spinal fos-LI neurons normalized by POD1 and POD3. CONCLUSIONS: c-fos expression is confined to neurons of the spinal dorsal horn known to be activated by nociceptive stimulation. The larger the injury elicited, the greater the expression of c-fos, suggesting that the expression of c-fos can serve as a useful marker of activated nociceptive neurons caused by acute pain following surgical incision.Further study is required to elucidate why the degree of tissue injury affects the laminar distribution of c-fos expression.
