1.A Case of Primary Malignant Lymphoma of the Orbit Treated by Radiotherapy.

Malignant lymphoma is mainly found in lymphnodes but extralymphatic organs may be rarely involved. Malignant lymphoma of the orbit is relatively rare representing less than 1% of all non-Hodgkin's lymphoma. All lymphoid tumors of the orbit are radiosensitive and are mainly treated wiht curative intent by irradiation. We report a case of orbital malignant lymphoma in a 41-year-old male patient. He received radiation therapy and subsequent enucleation and till now is clinically free of the disease.

3.A Comparison of Surgical Morbidity and Postoperative Voiding Function in Infants and 1-2 year-old Children after Ureteral Reimplantation.

PURPOSE: The advisability of early ureteral reimplantation in infants is controversial, and to our knowledge no long-term results are available. Whether early ureteral reimplantation in infants increases surgical morbidity, compared to ureteral reimplantation after the age of 1 year, was investigated, and the long-term voiding function after ureteral reimplantation evaluated. MATERIALS AND METHODS: Between 1993 and 1999, 130 children underwent ureteral reimplantation at our institution. The diagnosis was a primary vesicoureteral reflux, obstructed megaureter and an ectopic ureter. Two groups of 64 children that underwent ureteral reimplantation were compared. Group 1 included children younger than 12 months at surgery and group 2 children those between 1 and 2 years. The long term results were evaluated with regard to surgical outcome, voiding function and urinary tract infection. An analysis of the voiding function included family interviews, uroflowmetry and post-void residual urine measurements. RESULTS: Complete long-term data were available for 47 boys and 17 girls. In Group 1, there were no complications in 50 renal units(96%), while two had postoperative reflux, and underwent macroplastique injection. In Group 2, there were no complications in 35 renal units(97.1%), while one patient had postoperative reflux, which resolved spontaneously. No significant difference was noted in the postoperative post-void residual urine volumes between the two groups. CONCLUSIONS: In our samples, early ureteral reimplantation resulted in a technical success rate of greater than 95% and low postoperative morbidity, with the exception of urinary tract infection. Our data show satisfactory bladder emptying and normal uroflowmetry. When indicated, ureteral reimplantation can be performed safely in young children, without fear of damaging bladder function.

4.A Case of Presumed Ocular Toxocariasis.

Ocular toxocariasis in human is a rare disease, which occurs by ingesting infective ova accidentally. Clinically, patients with ocular toxocariasis are divided into 3 groups: group 1 consisted of those with diffuse destructive endophthalmitis, group 2 included those focal posterior pole granuloma, group 3 was made up of those with peripheral inflammatory masses in a relatively quiet eye. We experienced a 17 year-old female whose left eye exhibited a dense white elevated inflammatory mass over the macular area and connective-tissue strands extending from the mass to vitreous and surrounding retina. We report this case with a review of literature, but we couldn't get the aid of the ELISA test for antibody of Toxocara canis.

5.Multifocal Pyogenic Arthritis on four Joints: A Case Roport

Multifocal pyogenic arthritis more than four joints is very rare and considered to be related with some conditions such as immune deficiency, alcoholism, diabetes mellitus, steroid abuse, septicemia and gonococcal infection. In early stage, it is important to differentiate from acute rheumatic fever or acute rheumatic polyarthritis. Authors report a case of multifocal pyogenic arthritis on four joints simultaneously with good result.

6.A Clincal Study of Type IIIc Open fracture of the Forearm

In 1984, Gustilo subgrouped type III open fracture into III a, III b and III c according to the severity, soft tissue damage and vascular injury. Type III c open fracture presents serious problems especially possible amputation due to vascular insufficiency, so emergency vascular surgery with bone fixation by using plate, IM nail fixator is mandatory. We reviewed 10 cases of type III c open fracture of the forearm and evaluated the bone healing process according to fixation method, intramedullary nailing and plating treated at Orthopaedic Department of Sung-Ae General Hospital from Feb. 1986 to Aug. 1988. The folliwing results were obtained: l. All cases were industrial accidents, 9 cases in right, and the mean age was 24.1 ranging from 17 to 34. 2. There were 4 cases of the radial & ulnar artery rupture and 6 cases of the radial artery rupture, 7 cases were treated end to end anastomosis and 3 cases with vein graft. 3. The fracture level of ulna & radius was same in all cases, 7 cases in distal one-third, 2 cases in middle one-third and 1 case in proximal one-third. 4. The average time of union was 14.1 weeks in the radius and 14.5 weeks in the ulna. 5. The fixation methods were plating in 7 cases and IM nailing in 3 cases. 6. The complications were non-union in 1 case, osteomyelitis in 2 cases and refracture in 1 case. 7. The functional result was excellent in 1 case, satisfactory in 3 cases, unsatisfactory in 5 cases, and failure in 1 case according to the rating system of Anderson.

9.Dissecting Aneurysm of Vertebral Artery Manifestating as Contralateral Abducens Nerve Palsy.

Isolated abducens nerve paresis related to ruptured vertebral artery (VA) aneurysm is rare. It usually occurs bilaterally or ipsilaterally to the pathologic lesions. We report the case of a contralateral sixth nerve palsy following ruptured dissecting VA aneurysm. A 38-year-old man was admitted for the evaluation of a 6-day history of headache. Abnormalities were not seen on initial computed tomography (CT). On admission, the patient was alert and no signs reflecting neurologic deficits were noted. Time of flight magnetic resonance angiography revealed a fusiform dilatation of the right VA involving origin of the posterior inferior cerebellar artery. The patient suddenly suffered from severe headache with diplopia the day before the scheduled cerebral angiography. Neurologic examination disclosed nuchal rigidity and isolated left abducens nerve palsy. Emergent CT scan showed high density in the basal and prepontine cistern compatible with ruptured aneurismal hemorrhage. Right vertebral angiography illustrated a right VA dissecting aneurysm with prominent displaced vertebrobasilar artery to inferiorly on left side. Double-stent placement was conducted for the treatment of ruptured dissecting VA aneurysm. No diffusion restriction signals were observed in follow-up magnetic resonance imaging of the brain stem. Eleven weeks later, full recovery of left sixth nerve palsy was documented photographically. In conclusion, isolated contralateral abducens nerve palsy associated with ruptured VA aneurysm may develop due to direct nerve compression by displaced verterobasilar artery triggered by primary thick clot in the prepontine cistern.

10.Survey of the North Korean People's Social Consciousness-Study on North Korean Defectors in South Korea.

OBJECTIVES: North Korean people's thought and opinions on their contry, society, and economy were studied through North Korean defectors. METHODS: At Hanawon, 163 defectors were surveyed with 2 questionnaires in May 2002. RESULTS: North Koreans believe that socialism it was a right choice for North Korea. Because of the gap between the ideal and economic reality, however admitted sense of frustration. They think that the communist value system and the communal consciousness have been shrinking gradually, and their attitude to South Korea was one of ambivalence. Peoples views are different according to age and institutional education they received. CONCLUSION: For the day of unification, we suggest the followings. First, South and North Korea should put in more effort for building a common nationalistic consciousness. Second, reasonable criticism against communism and the advantages of alternative systems which are expected to be accepted by North Koreans through education after unification, need to be prepared. Third, the development of psychological conflicts and the frustration of North Korean people after unification are anticipated and their solutions must be saught after. Fourth, continuos studies for the understanding of North Korean people's psychological characteristics and it's change is needed.
