1.A Case of Neurilemmoma.

A neurilemmoma or schwann cell tumor is a rare and distintive nerve sheath tumor. Neurilemmomas usually are solitary and affect woman more frequently than men withaut particular affinity for any age group. They occur commonly on the cranial nerve, with the most acoustic nerve. When peripheral neurilemmomas are found, they are seen to occur on nerves in the scalp, face, neck, extremities and also visceral organs. Clinically, they are rounded or ovoid with uniform texture and consistency varyingfrom soft to moderately firm in gray, pink or yellowish color and are accompanied with intermittent radiating pain along the course of a peripheral nerve. We present a case report in a 28 year old man who developed two coin size ovoid painful masses on the left extremity for 6 months and describe a brief review associated with the analysis of the statistics of 56 cases of neurilemmoma for the past 9 years' duration in our hospital.

2.A Case of Accidental Vaccinia.

Accidental vaccinia may occur either in a recently vaccinated person through autoinoculation by veticular fluid or in a person who is in close personal contact with a recently vaccinated person. Accidental vaccinia occurs most co@mmonly on the eyelid, lips, nose or vulva. An outbreak of similar lesions in a family is unusual. We presented a case of accidental vaccinia in a 29-year old married woman. She has been relatively well until 5 days prior to visit, when she began developed painful grouped papules with central umbilication around both mammary areolas and nipples and marked painful swelling of both axillary lymphnodes. Family history revealed that her baby has also suffered from vesicopustuIar eruptions on upper lip 10 days before her onset and her nephew had similar skin eruption on left index finger for the last 1 week. Contact tracing revealed her neighboring child who was smallpox vaccinated inoculated her baby and then she was contracted from her baby, so-called, third hand vaccinia. Physical examination was not remarkable and laboratory findings were within normal limit. The biopsy finding showed findings consistent with viral infection. After symptomatic treatment and daily dressing, the akin lesion has completly healed in about 2 weeks.

4.Irreducible Dorsal Dislocation of First Metatarsophalangeal Joint by Closed Method: Report of a Case

Traumatic dislocation of the first metatarso-phalangeal joint is rare injury. The mechanism of this injury is hyperextension of the great toe, causing displacement of the proximal phalanx on the dorsum of the first metatarsal head and neck. The complex soft tissue anatomy surrounding this joint interferes with the closed reduction of the dislocation. This paper describes a patient with complete dorsal dislocation of the first metatarsophalangeal joint treated at the Paik hospital which cannot be reduced by closed manipulation.

6.Treatment of Tibial Fracture by Interlocking Intramedullary Nailing

The intramedullary nailing for the tibial fracture has been used in selected cases of fresh diaphyseal fracture and nonunion. However, with modern technical improvement such as image intensifier and interlocking, the indications have been expanded considerably. Interlocking intramedullary nailing has been popularized because it enables preservation of the range of motion of the joint, early weight bearing and early bony union. Between August, 1989 and July, 1991 interlocking nailing in the tibial fracture has been performed for 53 cases with follow up more than one year. The results were as follows; 1. All were treated with closed nailing and static locking was performed as a principle. Only three cases needed dynamization of the 47 patients treated with static locking nailing. 2. The union rate was 96.296 and mean period of fracture union was 15.2 weeks. 3. In the distal one third of fracture, rigid fixation could be achieved by adjusted length by cutting the distal end. 4. In 7 proximal tibial fractures, 4 fractures were accompanied with complications such as nonunion, angulation deformity and shortening. Therefore, interlocking nail is inadequate treatment of proximal unstable fractures. 5. In the delayed and nonunion treated by interlocking nailing, bony union was achieved in all 3 cases without bone graft and cast immobilization.

7.A Clinical Analysis of the Scapula Fractures

Fracture of the scapuls is one of the rare bony injuries seen in hospital practice. Authors reviewed the records and the roentgenograms of forty three patients from April 1976 to March 1987. The results were as follows : l. Of the 43 cases, peak incidence was in the middle aged men with high activity, and age range was 30 to 50 years. 2. The most common cause of injury was the motor vehicle accident. 3. Twenty eight cases of the scapular fracture occurred in the body, and eleven had fractures more than two parts in the scapuls. The most common type was the combined fracture of the body and neck. 4. A severe force was required to produce the injury and 79% of the patients had combined injuries of head and chest. 5. Delay in diagnosis was frequent because of the masking effect of the associated injury. 6. Although results of conservative measurements on the scapular fracture are generally good, we feel that treatment on more active line should be undertaken in young and active patients with fracture involving glenoid.

9.An Analysis on the Effect of Patient-controlled Analgesia Performed by Orthopaedic Department or Postoperative Pain Control after Shoulder and Elbow Surgery.

BACKGROUND: We investigated the effectiveness of pain management and the adverse events of intravenous (IV) patient-controlled analgesia (PCA) after orthopedic surgery. METHODS: From September 2014 and August 2015, we performed a retrospective analysis of 77 patients who underwent orthopedic surgery of the shoulder or the elbow in our hospital. The composition of the intravenous PCA administered to the patients was as follows: 250 mg of dexketoprofen trametamol, 70 mg of oxycodone, and 0.6 mg of ramosetron, which were made up to 79 ml of normal saline. We evaluated and statistically analyzed the difference in the visual analogue scale (VAS) scores for pain at immediate postoperation, at 24 hours of PCA, at 72 hours of PCA, and after discontinuation of PCA and in the incidence of adverse events. RESULTS: We found that VAS score decreased for 3 postoperative days and that with discontinuation of IV PCA a meaningful change in VAS score was no longer seen. Of the 77 patients, 22 presented with adverse events (28.6%). We terminated IV PCA temporarily in the 21 patients who presented with adverse events; we terminated analgesia permanently in one patient (1.2%). Consequently, 76 of 77 patients carried out IV PCA till the designated period. CONCLUSIONS: Intravenous PCA after orthopedic surgery of the shoulder or the elbow may be accompanied with adverse events. Careful assessment of the patient and treatment of the adverse outcomes are key to a successful maintenance of PCA and to a successful management of postoperative pain.
