1.The Effect of Lactic Acid Concentration on Cell Morphology and Phenotype in Cultured Intervertebral Disc Cell of Rabbit.

STUDY DESIGN: Changes of morphology and phenotype of cultured cells in media added lactate were observed. OBJECTIVES: To evaluate the effect of lactate on morphology and phenotype of cultured intervertebral disc cell. SUMMARY OF LITERATURE REVIEW: It was reported that lactate and pH were important factor in the degeneration of intervertebral disc. However the effect of lactate on morphology and phenotype of cultured intervertebral disc cell have not been studied. MATERIALS AND METHODS: Cells were dissociated enzymatically from rabbit nucleus pulposus. After attaining monolayer growth, the cells were incubated in media added 2mM or 5mM lactate. Total cell counts and morphological changes of the cells were periodically observed. Changes in cell phenotype were investigated by use of anti-collagen antibody stain. RESULTS: The cell groups added no lactate and 2mM lactate showed no difference in cell counts, morphology and phenotype. The cell group added 5mM lactate showed a reduction in final cell Counts and highel'ratio of fibroblastic cell in total population. Anti-collagen I Ab stained the Intra-and extra-cellular area of fibroblastic cells and intracellular area of chondrocytic cells. CONCLUSIONS: The current study suggests that high concentration of lactate inhibit intervertebral disc cell proliferation and accelerate morphological and phenotypical change to fibroblastic cell.

3.Adenocarcinoma Arising in Sacrococcygeal Teratoma: A case report.

We experienced a case of adenocarcinoma arising in sacrococcygeal teratoma. The patient was a 52-year-old woman. She was admitted due to one month of sacral pain. She had a sacral mass since birth. On physical examination, anal fistula was present at the perianal area and pus drainage was noted. MR image showed multiple variable-sized cysts with inhomogeneous density. Resected specimen, mesuring 12.5 7.0 cm in diameter, showed multiple variable-sized cystic lesions admixed with grayish solid portion. The cysts contained mucoid material. The microscopic examination showed mature teratoma composed of cysts lined by pseudostratified ciliated columnar epithelium, intestinal mucosa, mature cartilage, bone, and fat tissue. A moderately differentiated adenocarcinoma developed from the cystic area in the mass.

5.Nasogastric Tube Insertion using Savary-Gilliard Wire Guide(R) in a Comatose Patient : A Case Report.

The insertion of nasogastric tubes in comatose, obtunded or anesthetized patients is often difficult, frustrating and time-consuming. A large variety of methods inserting nasogastric tubes in those uncooperative patients have been reported. As a new effective method, we used Savary-Gilliard Wire Guide(R), which is designed for introducing Savary-Gilliard Dilator(R) into a strictured esophagus, for inserting a nasogastric tube in a comatose patient who was intubated with a ballooned tracheostomy tube. The insertion was successful in the first attempt and no complication occurred.

6.Three-dimensional evaluation of maxillary anterior alveolar bone for optimal placement of miniscrew implants.

OBJECTIVE: This study aimed to propose clinical guidelines for placing miniscrew implants using the results obtained from 3-dimensional analysis of maxillary anterior interdental alveolar bone by cone-beam computed tomography (CBCT). METHODS: By using CBCT data from 52 adult patients (17 men and 35 women; mean age, 27.9 years), alveolar bone were measured in 3 regions: between the maxillary central incisors (U1-U1), between the maxillary central incisor and maxillary lateral incisor (U1-U2), and between the maxillary lateral incisor and the canine (U2-U3). Cortical bone thickness, labio-palatal thickness, and interdental root distance were measured at 4 mm, 6 mm, and 8 mm apical to the interdental cementoenamel junction (ICEJ). RESULTS: The cortical bone thickness significantly increased from the U1-U1 region to the U2-U3 region (p < 0.05). The labio-palatal thickness was significantly less in the U1-U1 region (p < 0.05), and the interdental root distance was significantly less in the U1-U2 region (p < 0.05). CONCLUSIONS: The results of this study suggest that the interdental root regions U2-U3 and U1-U1 are the best sites for placing miniscrew implants into maxillary anterior alveolar bone.

7.Development of Toxic Anterior Segment Syndrome Immediately after Uneventful Phaco Surgery.

PURPOSE: We report on 15 cases of suspected toxic anterior segment syndrome after uneventful phaco surgery. METHODS: We retrospectively reviewed the charts of patients who had developed toxic anterior segment syndrome (TASS) after uneventful phacoemulsification for senile cataracts between April and December of 2005. Clinical features and all possible causes were investigated including irrigating solutions or drugs, surgical instruments or intraocular lenses, sterilization techniques for instruments, or any other accompanying disease. RESULTS: The patients consisted of 2 males and 13 females with an average age of 64.7+/-10.9 years. Five different surgeons had performed their phaco surgeries. No abnormal preoperative or operative findings were reported. Nevertheless, all 15 patients developed a moderate degree of corneal edema. Ordinary treatments were not helpful. We suspect that lack of sterilization resulted in the development of the syndrome, because after ethylene oxide gas sterilization was replaced with autoclaving, no such incidents have occurred. CONCLUSIONS: Toxic anterior segment syndrome requires special attention and thorough management, including sterilization of reused surgical instruments.

8.Inhibitory Effect of Luteolin Liposome Solution by Animal Model for Atopic Dermatitis in NC/Nga Mice.

Atopic dermatitis is a common chronic inflammatory skin disease, associated with marked inflammatory cells (of mast cells and eosinophils) and severe itching, which leads to clinical problems in the pediatric population. This study was designed to investigate the inhibitory effects of luteolin liposome solution, that is entrapped the hydrophobic luteolin (one of the flavonoids) into ethosome to improve its stability, by using hapten-induced atopic dermatitis animal model (NC/Nga mice).The luteolin liposome treated mice showed anti-inflammatory effect as evidenced by the lowering of erythema and edema in clinical observation, reduction of inflammatory cell infiltration and epidermal thickness in histopathological examination, when compared with TNCB induced controls. Luteolin liposome solution also reduced the serum IgE level which played important roles in the atopic dermatitis model. These results suggest that luteolin liposome solution has some merit in this formulation showing inhibitory effects for the atopic dermatitis.

9.Clinical Comparison between Glaucoma Triple Procedure and Small Incision Cataract Surgery using Foldable silicone IOL.

The purpose of this study was to evaluate the postoperative outcomes between the glaucoma triple procedure and the small incision cataract surgery using foldable silicone intraocular lens(IOL). This prospective randomized study comprised of 42 eyes which had the glaucoma triple procedure and 41 eyes which underwent small incision cataract surgery at chonnam University Hospital from May, 1994 to December, 1996. At least 3 months follow-up period was needed. Corrected visual acuity of 0.5 of better was obtained in 50% of triple procedure group and 88% of small incision cataract surgery group with the average lines of improved visual acuity of 5.6 and 7.2 respectively. There was no significant change in the refractive cylinder during the early postoperative period in both group. In the glaucoma triple procedure group, posterior capsular opacification(PCO) developed in significantly low incidence in the Mitomycin C using group and the incidence of PCO in eyes with IOL decentration was greater than in those without IOL decentration. In conclusion, patients who underwent glaucoma triple procedure using foldable silicone IOL showed excellent postoperative IOP control with success rate of 81%, rapid visual rehabilitation and refractive stability. We also found that glaucoma triple procedure with Mitomycin C was associated with lower incidence of PCO.

10.Epidemiology of Leisure Sports-Related Ocular Trauma.

PURPOSE: We investigated the epidemiological and clinical characteristics of leisure activity- and sports-related ocular trauma for application to its treatment and prevention. METHODS: A retrospective chart review was conducted on 121 patients who visited Gachon University Gil Medical Center from January 2006 to December 2007 because of leasure activity- and sports-related ocular trauma. The incidence of ocular injuries, sex and age, monthly and seasonal distribution, causes, diagnosis, and visual acuity were reviewed. RESULTS: Among the 1592 patients of ocular trauma, 121 patients (7.6%) were related to leasure activity- and sports. The incidence was higher in males (81.8%; 4.5-fold) than in females. The mean age of patients was 23.7+/-7.1 years. Ocular injuries occurred least in winter (19.8%) and occurred more during the week than on weekends (1.6-fold). The most common cause was soccer (39.7%), more specifically being hit with the soccer ball (47.1%). The initial LogMAR visual acuity was worse in the females and in the patients older than 30 years. Traumatic hyphema (19.7%) was the most common cause of visual disturbance, and corneal erosion (11.3%) occurred frequently. CONCLUSIONS: Leasure activity- and sports-related ocular trauma is becoming an increasingly significant cause of ocular morbidity, and trends relating to characteristics such as sex and age were found. These outcomes can be put to practical use in the prevention and treatment of sports-relatedocular injuries.
