6.Surgical Treatments of the Acute Anterior Cruciate Ligament Injuries

Primary repair of anterior cruciate ligament is superior to late reconstruction procedures for two reasons; objective stability is more easily restored due to intact secondary restraints not subjected to chronic stress, and the quality of the joint is superior with greater chance of intact menisci and normal articular cartilage not yet damaged through recurrent subluxation. We present the result of thirty-eight kness in thirty-seven patients who had been treated with primary repair of augmented repair for acute ACL rupture from March, 1984 to August, 1990. Twenty-one knees were treated with primary repair and seventeen knees augmented repair. At an average 33 months follow up, the results obtained were as follows: 1. The patients were 23 males and 14 females with an average age of 27.1 years. 2. Twenty four knees were injuried during athletic endeavors, 9 knees by automobile accident, and 5 knees by landing from height. In 18 knees anterior cruciate ligament was ruptured at the proximal attachment, in 17 knees at the midsubstance tear and in 3 at the near distal attachment. 3. The average interval from injury to operation was 7.5 days (ranged from 1 to 14 days). 4. The results were evaluated by Hospital for Special Surgery knee rating scale (modified by Marshall) at an average 33 months (range from 13 months to 7 years and 6 months) follow-up study. 16 (76%) out of 21 knees treated with primary repair were rated as good or excellent. 12 (86%) out of 14 knees treated with augmented repair were rated as good or excellent. As the result of this study, acutely torn anterior cruciate ligaments appear to be successfully treated with primary repair or augmented repair according to torn site and shape of ligaments.

7.A Comparative Study of Arthroscopic Meniscectomy and Open Meniscectomy: A Review of 143 Cases

We performed the partial or total meniscestomy in 143 cases with the meniscal injury by the arthroscopy or arthrotomy during the period from May, 1978. to November, 1987. There were 89 cases treated with arthroscopic partial, 11 csses treated with arthroscopic total, 16 cases with open partisl and 27 cases with open total meniscectomy. We did not include the cases of the discoid meniscus in the study. The statistic analyses were as follow ;1. The ratio of male to female was 106 to 37, and right to left was 79 to 64. 2. There were 82 cases in medial meniscus, and 61 cases in lateral meniscus. 3. The most common chief complaint and physical finding was pain, tenderness & McMurray(+). 4. The average period of hospitalization were 5 days in arthroscopic and 20 days in open meniscectomy. 5. The most cornmon associsted injury was the ACL injury in 36 cases. 6. The most common site and type of tear was the longitudinal tear at posterior horn. 7. In the preoperative arthrogram, the diagnostic accuracy was 78.3%. 8. The satisfactory results were 79.2% in arthroscopic and 64.7% in open meniscectomy.

8.Treatment of Chronic Osteomyelitis by Using Antibiotic-Acrylic Bone Cement Composites: Preliminary Report of 7 Cases

Antibiotic-acrylic bone cement composites demonstrated antibacterial activity as a result of diffusion of antibiotic from bone cement into surrounding enviroment, accordingly, the use of antibiotic incorporation to bone cement for the prevention and treatment of infection after joint surgery replacement has been adovocated. Early report of clinical success by using antibiotic-acrylic bone cement composites are encouraging. Our authors applied this concept to the treatment of chronic osteomyelitis. Antibiotic contained with bone cement was chosen by sensitivity test on pus culture finding. Seven cases of chronic osteomyelitia treated by using antibiotic-acrylic bone eement composites were .studied in the Department of Orthopaedic Surgery, Kyung Hee University Hospital from July 1979 to March 1981. The follow-up period was from 3 months tc 22 months. The results were as follow: 1. Six cases showed no evidence of recurrence except one by using antibiotic acrylic bone cement composites. 2. The impregantion of antibiotic in acrylic bone cement shall be regarded to be effective in increasing the local antibacterial activity by leaching the antibiotic into surrounding tissue over a period continiously in treatment of chronic osteomyelitis. 3. The use of antibiotic-acrylic bone cement compositea into dead space by saucerization are consid- ered an one such approach to the decrease of dead space. 4. The impregnation of antibiotic in acrylic bone cement is regarded a new concept in treatment of chronic osteomyelitis.
