1.The Effect of Histoacryl(R) for Gastric Variceal Bleeding Which Failed by Ethanolamine Oleate Injection Sclerotherapy.

Treatment modalities of gastric variceal bleeding are endoscopic injection sclerotherapy, endoscopic variceal ligation, combined above two method, operation, TIPS, and percutaneous transhepatic embolization. Recently, Histoacryl was introduced as a new sclerosant of acute variceal bleeding failed by others. So, we studied 18 cases of acute gastric variceal bleeding uncontrolled by EIS using Ethanolamine oleate. Intravariceal injection of Histoacryl is considered as a safe and effective method for hemostasis of intractable acute gastric variceal bleeding uncontrolled by other sclerosants. But, it is important that endoscopist should be in harmony with assistant at injection of Histoacryl. And its shooting speed should be fast because of clogging within the catheter. It is necessary to do long term study for evaluation of eradication effect in acute gastric variceal bleeding.

2.Benign Lymphoepithelial Cyst: A case report.

An intraparotid benign lymphoepithelial cyst is a rare disease characterized by unilateral painless swelling of parotid region. The histogenesis is controversial. Surgical excision is recommended for diagnosis and curative treatment. We present a case of benign lymphoepithelial cyst arising in a patient with neurofibromatosis. A 46-year-old woman presented with a slowly growing multilocular cystic mass in the left cheek. The cystic mass measured 4 cm in maximal outer diameter and the cystic wall was thick and yellowish pale to gray, soft with well circumscribed margin. Microscopically, the multilocular cyst was lined by stratified squamous epithelium for the most part and underlying lymphoid tissue aggregates with follicles and sharply demarcated from adjacent salivary parenchyma which is of normal appearance and without lymphoid aggregates. Since this lesion is absolutely benign, it is important to separate this benign cyst from cystic salivary gland tumors.

3.Systemic Lupus Erythematosus Showing Vesiculobullous Eruptions: Report of Two Cases.

l6-year-old male and 48-year-old female with systemic lupus erythematosus (SLE) and renal disease presented with vesiculobullous eruptions during severe attack of the disease. They showed hypocomplementernia, and high ANA titers. Histopathologic findings revealed subepidermal blister and leukocytoclastic vasculitis, Direct immunofluorescence demonstratedlinear deposits of IgG, IgA and lgM at the dermoepidermal junction. The vesiculobullous eruption of SLE may be an important marker reflecting disease activity and prognosis.

4.Double Incision Technique for Carpal Tunnel Release.

A preliminary study was conducted to evaluate the immediate postoperative recovery and socioeconomic efficacy with the surgical release of Carpal tunnel syndrome by double incision technique. Sixty cases of carpal tunnel syndrome were operated by one surgeon between 1993 and 1995 and 44 cases in 39 patients were available for this study. Surgery was done under axillary block and patients were evaiuated preoperatively, 3 weeks and 6 weeks each postoperatively. The evaluation was focused on the immediate recovery in terms of subjective symptoms of pain and paresthesia, local tenderness, strength of grip power and pinch power and postoperative disability. 1l was noted that surgical release of carpal tunnel syndrome utilizing double incision technique was as effective as seen in the endoscopic release in terms of quicker recovery of pain, local tenderness and strength. It was also noted that this double incision technique was as safe and economically beneficial as observed in the conventional open procedure.

5.A Case of Nonischemic Central Retinal Vein Occlusion Developed in the course of Ulcerative Colits.

Ulcerative colitis is a chronic, inflammatory disease of colon which is rare in Korea. In the course of ulcerative colitis numerous extraintestinal complication may develop on the joints, skin, liver, kidney, vessel, oral cavity and eyes. The most common ocular lesions complicated in patients with ulcerative colitis are episcleritis and iritis. And the cases of retinal vascular disease in associated with ulcerative colitis are extremely rare. We experienced nonischemic central retinal vein occlusion complicated in a patient with ulcerative colitis, who is 32-year-old male and complained decreased visual acuity and metamorphosis.

6.A Case of palisaded, Encapsulated Neuroma.

Palisaded, encapsulated neuroma is characterized clinically by a solitary, slowly-growing and dome-shaped papule or nodule, usually accompanied by telangiectasia on the surface, and histopathologically by an encapsulated nodule in the dermis and the palisading arrangement of nuclei. A 57-year-old female patient presented with a 1cm-sized, solitary nodule on the right ala nasi which had been present for about 5 years. The nodule had a tendency to slow growth and it became a polypoid nodule. Telangiectasia was shown on the surface of the lesion. Histopathlogical findings showed a well-defined and encapsulated nodule in the dermis, composed of spindle cells with basophilic and plump nuclei in a palisading fashion. On immunohistochemical staining, the tumor cells of the nodule were positive for S-100 protein, while the capsule of the nodule was negative for S-100 protein. Epithelial membrane antigens were focally positive only on the capsule of the nodule. We report herein a case of palisaded, encapsulated neuroma, one case of which has been reported in Korea.

7.A Case of nevus Sebacens of Jadassohn Associated with Various Epidermal Appendage Tumors.

We report a case of nevus sebaceus of Jadassohn in a 31-year-old female associated with various epidermal appendage tumors, such as syringocystadenoma papilliferum, sebaceus adenoma and eccrine hidrocystoma. We supposed that the multiple lesions seem to support the concept of nevus sebaceus arising from the pluripotential primary epithelial germ.

Reactions from stinging insects are significant to dermatologists because of the wide variety of clinical presentations from mild local reaction to severe anaphylactic reaction. Although ant sting commonly occurs, it has been rarely reported in the literatures. We experienced a unique case with allergic reactions by the ant of the Cremawgaster iw6w urai vagala, subfamily Myrmicinae, family Formicidea, order Hymenoptera that has not been previously reported.

9.Ligament Injuries Combined with Proximal Tibia and Fibula Fracture.

It is well known that fractures around the knee joint are usually combined with ligament injuries but it is easy to miss the ligament injuries during the treatment of fractures. The invention of the MRI and the development of arthroscopic techniques has made it easy to diagnose and care for ligament injuries. Authors reviewed the patients who had fractures around the knee joint to evaluate the frequencies, types, treatments and the results of combined ligaments injuries. There were 57 cases with the fracture of the proximal tibia and fibula which could be follwed up at least one year: from January 1992 to June 1995. There were 19 cases (33%) which had combined ligament injuries. The ligament injuries were diagnosed by stress X-ray & MRI evaluation and confirmed by arthroscopic examination. Single lateral rim avulsion fracture of proximal tibia was the most common type of fractures (7 cases, 37%). There were 11 cases (57%) of combined injuries of anterior cruciate ligament and medial collateral ligament. It was the most common combined ligament injury. Except for one, eleven cases that were combined with lateral rim avulsion of proximal tibia had anterior cruciate ligament injury. There were ten cases (53%) of fibula head and neck fractures which were combined with ligament injuries. The total number of the injured ligament was 35 and the common rupture sites of the injured ligaments was the attachment site of femur (13 cases) and tibia (16 cases). There were seven meniscal injury cases (37%). The interval from injury to operation averaged 14.1 days because many cases were acute injuries. Primary repairs for ruptured ligament were done in 31 cases (88%) because of their attachment site injuries. There were no instabilities except one case which was treated with posterior cruciate ligament reconstruction using semitendinosus ligament. It had a grade I posterior instability at the last follow-up. Early diagnosis and proper treatment on the ruptured ligaments and the torn meniscus combined with proximal tibia and fibular fractures seem to be important for a good prognosis.

10.Effects of changes in lifestyle and biological parameters on blood lipid levels in middle aged men.

BACKGROUND: Death from coronary heart disease is increasing and this study is to evaluate the effect of longitudinal changes of lifestyle and biological parameters on the blood lipid levels, as the risk factor of the coronary heart disease. METHODS: Total cholesterol and total cholesterol to HDL-cholesterol ratio (atherosclerogenic index) as an indicator for risk of coronary heart disease were examined longitudinally in a sample (n = 463) of middle-aged men by 2 years. The independent variables were body mass index, smoking, alcohol, exercise, diastolic blood pressure, uric acid. All data was drawn from questionnaire, blood chemistry, and review of chart. RESULTS: ANOVA test according to categorized variables revealed that for total cholesterol, uric acid (P<0.01) and diastolic blood pressure (P<0.01), and for atherosclerogenic index, and body mass index (P<0.001) were significant. In correlation analysis, total cholesterol was associated with uric acid (r=0.20, P<0.001), diastolic blood pressure (r=0.15, P <0.001) and body mass index (r=0.03, P<0.05), and for atherosclerogenic index so was body mass index (r=0.18, P<0.001). In regression, uric acid (beta=6.07, P <0.001), diastolic blood pressure (beta=0.36, P <0.01) for total cholesterol, and body mass index (beta=0.22, P <0.001) for atherosclerogenic index were significant. But changes in alcohol consumption, smoking, and exercise were not statistically significant. CONCLUSION: In order to reduce risks of coronary heart disease, the more aggressive medical intervention for the uric acid, DBP, and BMI, would be essential. This study was done without medical intervention. So, further study with intervention, adequate duration and intensity for parameters modification is required.
