5.Clinical Observation, of Chronic Subdural Hematomas (Burr hole, Drainage and Craniotomy, Membranectomy).

The authors have presented 44 cases of chronic subdural hematoma which were diagnosed by brain computerized tomographic (CT) scan at the Department of Neurosugery, Keimyung University, School of Medicine from May 1980 to July 1982. There were 42 men and 2 women. The common incidence of age was between fourth and fifth decade. Most of the cases(80%) had a history of head injury and nearly all cases complained headache(93%). The most common sign was papilledema(71%) in relatively young age patients and mental change(62%) in old age patients. All cases performed brain CT scan, which showed hypodense(41%), isodense(21%), hyperdense(21%) and mixed dense(18%) lesion. The thickness of hematoma which was measured by brain CT scan revealed 10 mm-30mm in most cases and showed the tendency of increase in old age patients. Of 44 cases, 23 cases were treated with craniotomy and membranectomy, 18 cases with burr hole and drainage, and 3 cases which were bilateral lesion with both methods. There was not any different result in both methods and most of all cases showed good postoperative course except 3 cases of poor preoperative state.

6.Chronic Radiation Dermatitis Accompanied by Acrosclerosis and Loss of Digits due to Osteoradionecrosis.

Chronic postirradiation changes of skin reflect the injury of dermal structures particularly the vasculature and connective tissue. The clinical signs include atrophy, partial or complete destruc tion of cutaneous appendatges, telangiectasis, sclerosis of underlying tissue, pigrnentary changes, and in rare instances, ulceration with or without var ious premalignant and malignant neoplasms. The patient, was a 65-year-old man, who had been exposed to a large amount. of X-ray irradi- ation on his hands incidentally or accidentally for a long time. The skin changes of his hand were thickening and hardening of all digits loss or focal consttict,ion of digits, and keratotic papules. Epidermal hyperplasia and fibrous thickening of collagen bundles were found on skin biopsy. Gradual narrowingal of the bony caliber with eventual resorption was noticed on radiologic examination. It has been rarely reported in the literature that chronic radiation dermatitis develops severe and unusal manifestions such as acrosclerosis and loss of digit due to osteoradionecrosis.

The reliability of emergency ultrasonographic(US) detection of hemoperitoneum and solid organ injury in blunt abdominal trauma was evaluated retrospectively. From October 1,1995 to August 31,1996,90 patients were included in the study. Ultra- sonographic findings showed a sensitivity, specificity, and accuracy of 97.6%, 97.9%, and 98.1%, respectively, in detecting intraabdominal fluid collection. We believe that US in an emergency center is a quick, safe screening method in the evaluation of blunt abdominal trauma. In our department, US has replaced diagnostic peritonaeal lavage(DPL) and computed tomography(CT) as the screening study of first choice.
