1.Fixation Failure after internal Fixation in Intertrochanteric Fractures.

Fixation failure is a major complication of intertrochanteric fracture. The causes of fixation failure were assessed in a series of 80 patients with intertrochanteric fractures, which had been internally fixed with either a sliding hip screw or a Gamma interlocking nail. The overall rate of fixation failure was 16.3%, in which the cutting-out of the implant from the femoral head was the only cause of the instance. The cutting-out rate was influenced by the accuracy of fracture reduction, the position of implant placement within the femoral head, and the bone mineral density. Anatomical reduction and the central placement of implant within the femoral head showed the most excellent results compared to other treatment modalities. Age, gender, fracture type and a kind of fixation implant had no significant effect. Our results showed that the intertrochanteric fractures should be reduced as anatomically as possible and it is essential that the central placement of the implant within the femoral head be obtained.

2.A Study on the CD34 Expression of Cutaneous Benign and Malignant Vascular Tumors.

BACKGROUND: CD34 is a 115 kD glycoprotein which is expressed on hematopoietic progenitor cells. It is also known as an immunohistochemical marker of endothelial cells. OBJECTIVE: This study was designed to investigate the patterns of CD34 expression on: (1) cutaneous benign and malignant vascular tumors and (2) on the mature and immature vessels of pyogenic grauloma and capillary hemangioma. METHOD: We performed immunoperoxidase staining using a monoclonal anti-CD34 antibody (QBEND/10) on formalin fixed, paraffin-embedded sections of 23 benign and malignant cutaneous vascular tumors. RESULTS: The results are summerized as follows: 1. In 3 cases of nevus flammeus and 6 cases of carvernous hemangioma, vascular endothelial cells of all hemangiomas showed CD34 expressions. In 5 cases of angiokeratoma, endothelial cells of hemangioma, did not express CD34. 2. In all 5 cases of pyogenic granulomas and one case of capillary hemangioma, endothelial cells of mature vessels, endothelial cells near the well-formed lumina and endothelial cells showing intracellular lumina showed strong positivity for CD34, wbile endothelial cells far from the lumina and endothelial cells without lumina formation mostly showed negative staining for CD34. 3. One cese of Kaposis sarcoma showed focall positivity for CD34 both in endothelial cells of the small, well-formed vessels and spindle cells. Two cases of angiosarcoma showed CD34 expression only in endothelial cells of well-formed, normal appearing vessels, whereas atypical endothelial cells of tumor vessels and spindle cells were negative for CD34. CONCLUSION: CD34 could be a marker for endothelium in mature, well-differentiated vascular structures and may serve as a marker of lumen formation or differentiation of endothelial cells.

3.A Case of Papillary Eccrine Adenoma.

The papillary eccrine adenoma is a rare benign sweat gland neoplasm charscterized by potentislly locally aggressive clinical course. We report herein a case of papillsry eccrine adenoma in a 28-year-old female who presented single intradermal tumor on the right postauricular ares. Histopathological findings showed dilated ducts containing eosinophilic or keratinous materials and intraluminal papillary rowths.

4.A Case of Intravascular Papillary Endothelial Hyperplasia with Angioleiomyoma.

Intravascular papillary endothelial hyperplasia is a relatively rare benign tumor, which is charaterized by the development of endothelial-lined papillary projections in a vascular lumen. They can occur as a pure form in which endothelial proliferation developes in a dilated vessel, a mixed form in which endothelial proliferation occurs within a pre-existing angioma. We herein report a case of intravascular papillary endothelial hyperplasia coexistent with angioleiomyoma occuring in a 54-year-old man, who had a slowly growing tumor on the right sole for 2 years. The histologic findings revealed a solitary encapsulated mass composed of smooth muscles and blood vessels in deep dermis and papillary endothelial hyperplasia in a neighboring blood vessel.

6.The Effect of Melatonin on Mouse Jejunal Crypt Cell Survival and Apoptosis.

PURPOSE: To evaluate protective mechanism of melatonin against radiation damage and its relationship with apoptosis in mouse jejunum. MATERIALS AND METHODS:' 168 mice were divided into 28 groups according to radiation dose and melatonin treatment. To analysis crypt survival, microcolony survival assay was done according to Withers an (l Elkind's method. To analysis apoptosis, TUNEL assay was done according to Labet-Moleur's method. RESULTS: Radiation protection effect of melatonin was demonstrated by crypt survival assay and its effect was stronger in high radiation dose area. Apoptosis index with 8 Gy irradiation was 18.4% in control group and 16.5% in melatonin treated group. After 18 Gy, apoptosis index was 17.2% in control group and 15.4% in melatonin treated group. Apoptosis index did not show statistically significant difference between melatonin treated group and control group. CONCLUSION: Melatonin shows clear protective effect in mouse jejunum against radiation damage but it.', protective effect seems not to be related with apoptosis protection effect.

9.The Effect of a Hydroxyapatite and Tricalcium - Phosphate Coating on Titanium Fiber - Mesh Stem.

Seventy-three uncemented total hip arthroplasties in the sixty-four patients were studied as a retrospective manner. Hydroxyapatite and tricalcium-phosphate (HA/TCP Calcicoat) coated titanium fiber-mesh stems were used in forty-eight cases and the identical components but without hydroxyapatite coating were used in the other twenty-five cases. The distribution of the patients in two groups showed no statistically significant differences and the same porous coated hemispherical acetabular components were used in two groups. At the time of two year follow-up after the operation, the mean Harris hip score and the Enghs radiographic assessment score were 95.5 and 19.8, respectively, in the HA/TCP Calcicoat tm group and 94.4 and 19.2, respectively, in the non-coated group. None of these differences were statistically significant. At three months after operation, the frequency of the thigh pain was 8.3% in the HA/TCP Calcicoat tm group and 20% in the non-coated group, which was also not significantly different (p=0.24). There were no revisions in either group. All the femoral components except one among the non-coated group showed stable bony fixation in both groups and no differences in Enghs radiographic assessment criteria were detected between the two groups. There seemed to be a tendency of early pain relief and more endosteal new bone formation in the HA/TCP Calcicoat TM group, which, however, failed to show a statistically significant clinical or radiographic differences. This result is considered as preliminary and longer follow-up should be required to uncover any hidden advantages or disadvantages of the HA/TCP Calcicoat TM coating on titanium fiber-mesh stem.

10.A Case of Hypothyroidism Induced by Alpha-Interferon Therapy for Chronic Hepatitis B.

Thyroid dysfunction has heen reported as a side effect of alpha-interferon(IFN) therapy. The incidence of thyroid disorders was 1.2-12%' duripg 1FN therapy for chronic active hepatitis. We experienced a case of hypothyroidism developed after IFN therapy in a 48 year-old man with chronic hepatitis B. Baseline thyroid function before IFN therapy was normal and thyroid auioantibodies were also negative. High tiers ol' antimicrosomal antibody and anti-thyroglobulin antibody appeared and severe hypothyroidism followed at 6 months after starting the IFN therapy.
