1.THE VARIOUS ROLE OF LOCAL INJECTION OF BOTULINUM A EXOTOXIN.

Botulium toxin A has been used therapeutically in humans for over 20 years for a variety of medical indications. Some wrinkle and unsightly facial expressions are due to hyperkinetic muscle. For the past year, the author has injected it for variant purpose, so we describe the our experience with the variant extended use of the toxin including correction for just dynamic wrinkle, used with subperiosteal face lifting or peeling, post-traumatic twitching, and facial paralysis and relevant anatomy are discussed. Also we have another concept about muscle anatomy which have superficial and deep portion. The superficial portion is for harmonious action with SMAS during facial expression, which is also related to fine wrinkle, and the deep portion play role gross movement. Botulium toxin is safe and effective in varient field without complication. Its use is associated with a high degree of patient and physician satisfaction.

2.A Comparative Clinical analysis of Arthroscopic Meniscectomy and Arthroscopic Meniscus Repair

There have been controversies on the rationale, surgicl indication, technique and results of meniscus repair.. Authors comparatively studied 100 cases of meniscus injury in 88 patients (arthroscopic total menisectomy: 16 cases, arthroscopic partial menisectomy: 68 cases, arthroscopic meniscus suturing: 16 cases) from February 1985 to March 1992. The results were as follows: 1. Male (61.4%) was much more affected than female. 2. The third decade group was most commonly affected and the most common causes was sports injury (43.2%) 3. The lateral meniscus was more injured (71%), than the medial (29%) 4. In arthroscopic finding, longitudinal tear (37%) was the most common. 5. According to the Tapper and Hoover's criteria, satisfactory result was 56.3% in arthroscopic meniscus repair group.

2 patients of forearm deformities with ulnar defect and radial head dislocation were treated with the Ilizarov apparatus at Chungnam University Hospital. One patient had a severe hypoplastic remnant-like ulnar fragment and a anterolaterally dislocated radial head due to complicated osteomyelitis of ulnar. We applied an Ilizarov apparatus to transport the radial head distally, and fixed to remaining proximal ulnar fragment. The other had a large bony defect at the ulnar shaft and a dislocated radial head due to complicated osteomyelitis of ulna. We performed internal transport of ulna to decreased the ulnar defect and to descend the dislocated radial head. The clinical-radiological features and functional results of those 2 forearm deformities were described with brief review of the articles about the Ilizarov method for correction of forearm defor- mities.

7.Medullary Carcinoma of Thyroid Gland with Co-existing Papillary Carcinoma.

We report a medullary carcinoma of the thyroid gland with a coexisting occult sclerosing papillary carcinoma. This rare case, in that the two types of carcinoma are completely separated and not intermixed together, is different from the previously reported mixed follicular and parafollicular cell carcinomas of the thyroid gland, so-called "differentiated thyroid carcinoma, intermediate type". The two separate tumors of the present case are undoubtedly of coincidental double primaries, and it can be a assumed that at least some of the mixed follicular and parafollicular cell carcinomas are results of collision of carcinomas derived from two embryogenetically different cell types.

Eccrine porocarcinoma is a rare tumor of the skin. A case report of an eccrine porocarcinoma metastasizing to epidural space of spinal cord and inguinal area with a nine year follow up is described. The patient had a nodular growth of the abdomen with both inguinal lymphadenopathy three years before its first excision. After a follow up of nine years, he complained a weakness of lower extremities and back pain. Extradural mass of 10th thoracic vertebra and left inguinal mass were found. Subsequently, the masses histologically identical to the skin tumor were found.

10.Twin Pregnancy and Delivery After Intracytoplasmic Sperm Injection Followed by Calcium Ionophore with Spermatozoa from a Globozoospermic Man: A Case Report.

Our purpose is to describe a successful twin pregnancy and delivery after intracytoplasmic sperm injection (ICSI) followed by calcium ionophore with spermatozoa from a globozoospermic man. On the second attempt of ICSI, all of eight metaphase II oocytes were fertilized with treatment with calcium ionophore. Day 3 transfer of six normally developing embryos resulted in an ongoing twin pregnancy, and two preterm healthy babies were born in the 33th week of gestation. To the best of our knowledge, this is the first report of pregnancy and delivery after ICSI followed by calcium ionophore with spermatozoa from a globozoospermic man in Korea.
