1.Effect of Tongbi Prescription on the Pathomorphological Changes of Ankles in Adjuvant Arthritis Rats

Objective To observe the effect of Tongbi Prescription on pathomorphological changes of ankles in adjuvant arthritis rats. Methods Rats were randomly divided into six groups: normal control group,model control group,three Tongbi Prescription treated group (respectively with high-,mid-and low-dosages),Tripterygium glycosides treated group (each n=10). Except for the rats of normal control group,the others were injected with Freund’s complete adjuvant. The pathomorphological changes of ankles of each group were measured. 2.Surgical Treatment of Chronic Pancreatitis with Mass in Head

Objective To summarize and discuss the diagnostic and treating experiences of chronic pancreatitis with mass in the head. Methods Eight patients of chronic pancreatitis with mass in the head who were misdiagnosised as carcinoma of head of pancreas were analyzed retrospectively in the past 10 years. Results All the patients exhibited abdominal pain,5 of whom were with jaundice and 3 with anorexia. All the patients were misdiagnosised as carcinoma of head of pancreas before the operation,but the pathology after operation indicated chronic pancreatitis. The pancreaticoduodenectomy was performed in 5 patients,the choledochojejunostomy in 2 patients,while the exploratory laparotomy in 1 patient. After the operations,the abdominal pain was relieved in 7 patients, while 2 patients who accepted pancreatoduodenectomy suffered from pancreatic fistula,1 of whom died in the end. Conclusion It’s hard to differentiate the chronic pancreatitis with mass in the head from the carcinoma of head of pancreas before operation. If the carcinoma of head of pancreas can’t be excluded during the operation,the pancreatoduodenectomy should be performed,while the duodenum-preserving total resection of the head of the pancreas or any intra-drainage operations should be done if chronic inflammation is found in the whole pancreas with a negative result of the biopsy of the pancreas through the needle aspiration.
3.Development condition of private non-profit medical institutions

Although private hospitals are generally encountering development difficulties, some private non-profit medical institutions thrived in the medical market. These hospitals have rich experiences in terms of manage-ment system, operation mechanism, and competition methods. In this paper, we summarize the experiences of those well-functioning private non-profit hospitals and analyze the essential conditions for developing private non-profit med-ical institutions. These conditions include meeting local health market needs, ensuring that the hospital management system and operation mechanism result in high-quality health services, ensuring hospital development meets local health plans ( in order to obtain policy support) , ensuring the goal of the hospital is public welfare and that the hospital has strong financial support, obtaining certain social and political capital, and ensuring high-quality managers and staff.
4.Application of Risperidone in Depression without Psychotic Symptoms

Objective To survey the value of risperidone in the treatment of depression without psychotic symptoms. Methods 205 depressive patients were randomly divided into 3 groups: risperidone group, fluoxetine group and combined group (risperidone+fluoxetine). They were assessed with Hamilton Depression Scale (HAMD), Treatment Emergent Symptom Scale (TESS) before treatment and in 2nd week, 4th week, 6th week and 8th week after treatment. Results The reduction rate of HAMD of combined group was the best among three groups(P<0.05), and that of fluoxetine group was better than that of risperidone(P<0.01). The significant difference in reduction rate of HAMD between combined group and other two groups was observed since 2nd week after treatment(P<0.05). No difference in scores of TESS has been observed in any time among three groups. Conclusion Risperidone can improve the efficacy of fluoxetine on depression without psychotic symptoms without increasing side effects, but itself is less effective than fluoxetine.
5.A case report of cerebral malaria combined with cyanotic congenital heart disease in a child and literature review

Abstract: Malaria is one of the deadliest vector-borne infectious diseases in Africa, seriously endangering human life and health. Among them, cerebral malaria (CM) has a higher morbidity and mortality rate in children, with clinical manifestations primarily involving neurological symptoms, and comprehensive treatment mainly using artemisinin derivatives. When children with cerebral malaria are complicated by congenital heart disease and induce acute hypoxic attacks, the diagnosis and treatment face great difficulties in the absence of effective examination and monitoring treatments. Similar cases have rarely been reported at home and abroad. The paper summarizes and reports a case of a child with cerebral malaria combined with cyanotic congenital heart disease, treated during an aid mission to Africa, who was improved and discharged from the hospital after receiving multidisciplinary anti-malarial, intracranial pressure lowering, neurotrophic and symptomatic supportive treatment. This paper also discusses the diagnosis and treatment process of this child under difficult conditions, combined with current literature reports, in hopes of providing a reference for clinical treatment.
6.Treatment effect of different dialysis modalities on pruritus in elderly maintenance hemodialysis patients

Objective To study the effect of different dialysis modalities on pruritus in elderly maintenance hemodialysis patients.Methods Totally 51 patients were randomly divided into hemoperfusion combined with hemodialysis group (HD+ HP),hemodiafiltration group (HDF) and hemodialysis group (HD).Plasma β2-microglobulin(β2-MG) and intact parathyroid hormone (iPTH) were measured by means of radio immunoassay at pre and post dialysis,4 weeks and 8 weeks after dialysis,cutaneous pruritus was scored.The remission rate of itching was calculated at 8 weeks after dialysis.The parameters were compared among different groups.Results The level of plasma β2-MG was lower in HD+HP group after dialysis than pre dialysis [(13.48±3.05)mg/L vs.(16.27±4.73) mg/L,t=2.044,P<0.05],at 4 weeks and 8 weeks after dialysis,its levels were decreased to (10.97±3.25)mg/L(t=3.808,P=0.002)and (6.47±2.35)mg/L(t=7.650,P=0.000),respectively.The levels of iPTH were also found decrease from(887.5 ± 242.7)ng/L to (688.3 ±223.4)ng/L(t=3.384,P=0.004)at 4 weeks and (467.2±102.5) ng/L(t=6.578,P=0.000) at 8weeks after dialysis in HD+HP group (all P<0.01).There were differences of the levels of plasma β2-MG and iPTH at 4 weeks and 8 weeks after dialysis in HDF group (all P< 0.05),but no differences of the levels of plasma β2-MG and iPTH during every period were found in HD group(all P>0.05).The scores of cutaneous pruritus were decreased from (21.17± 5.01) scores to (13.37±2.85) scores(t= 5.580,P=0.000)at 4 weeks and (8.52±4.38) scores(t=7.838,P=0.000)at 8 weeks after dialysis in HD+ HP group,and also the scores at 4 and 8 weeks after dialysis in HDF group (all P<0.01),but there were no significant differences of the scores during every period in HD group (all P>0.05).The remission rate of itching was better in HD+ HP group than in HDF group [88.24% (15/17 cases) vs.58.82% (10/17 cases),x2=14.44,P=0.000],better in HDF group than in HD group 23.53% (4/17 cases) (x2 =4.37,P=0.037).Conclusions HD+HP is superior to HDF in efficiently clear β2-MG and iPTH,and relief cutaneous pruritus,but HD can poorly clear β2-MG and iPTH or relief itching.

8.Private medical institutions in China:Policy review and analysis

The evolution of China’s policy on private medical institutions has been ongoing, and a comprehen-sive policy system has been established. In this paper, we review the development history and summarize the main policy points in terms of planning, approval, financial input, supervision and administration, support and incentives. We analyze current policy barriers from three levels:1 ) policy ideas, consensus on the function and role of private medical institutions have not reached, which may influence the corresponding policy making and implementation;(2) policy design, multiple policy bottlenecks still exist, such as lack of flexibility, poor convergence, vague stand-ards and other aspects; 3 ) policy implementation, the health planning polices and equal treatment polices haven’t been carried out, and there are many hidden obstacles. To solve these problems, we recommend that further attention be paid to the importance of private medical institutions;the running mode should be chosen properly; the develop-ment space should be expanded;the classified management policy should be improved; and the supervision and ex-amination of policy implementation on local governments should be intensified.

OBJECTIVETo investigate the effect of four traditional Chinese medicines distributed along lung meridian, namely Ephedrae Hebra, Zingiberis Rhizoma, Scutellariae Radix and Mori Cortex, on TLR2 and NF-κB expressions in mice with lung heat syndrome, in order to study the intervention effect of the four traditional Chinese medicines (TCMs) on the lung heat syndrome.
METHODOne hundred KM mice were randomly divided into the normal control group, the model group, the Ephedrae Hebra group, the Zingiberis Rhizoma group, the Scutellariae Radix group and the Mori Cortex group (20, 10 g x kg(-1)), nasally dripped with streptococcus pneumoniae to establish the mouse lung heat syndrome model, and then administered with different TCMs. The expressions of TLR2, NF-κB p65 proteins in lung tissues were analyzed by the immunohistochemical method. The expressions of TLR2, NF-κB p65 mRNA were measured by real time PCR.
RESULTCompared with the normal control group, the expressions of TLR2 and NF-κB p65 proteins in lung tissues in the model group were higher (P < 0.01), and the expressions of TLR2 and NF-κB p65 mRNA in lung tissues were up-regulated (P < 0.01 or P < 0.05). Compared with the model group, Ephedrae Hebra high and low dose groups, the Zingiberis Rhizoma low dose group and the Scutellariae Radix high dose group showed decreased expression of TLR2 protein (P < 0.05 or P < 0.01); Ephedrae Hebra high and low dose groups, the Zingiberis Rhizoma low dose group, Scutellariae Radix high and low dose groups and Mori Cortex high and low dose groups showed reduced expression of NF-κB p65 protein (P < 0.05 or P < 0.01). Ephedrae Hebra high and low dose groups, Zingiberis Rhizoma high and low dose groups, Scutellariae Radix high dose group and Mori Cortex high dose group showed down-regulated expression of TLR2 mRNA (P < 0.01 or P < 0.05).
CONCLUSIONEphedrae Hebra, Zingiberis Rhizoma, Scutellariae Radix and Mori Cortex can induce the TLR2/NF-κB inflammatory signal pathways by down-regulating the expressions of TLR2 and NF-κB p65 in protein and mRNA, so as to alleviate the lung tissue injury in mice with lung heat syndrome.
