1.Electron Microscopic Demonstration of Sialoglycoconjugates in the Sinus Mucosa of Rabbits after Inoculation of the Influenza A Virus.

This study was conducted in order to observe ultrastructural changes in the expression of sialoglycoconjugates in maxillary sinus mucosa after inoculation of influenza A virus utilizing four different gold-labeled lectins : ckia amurensis (MAA), wheat germ agglutinin (WGA), sambucus nigra (SNA), and peanut agglutinin (PNA). A comparison of the affinities of these gold-labeled lectins demonstrated the varying distributions of sialoglycoconjugates in the ciliary layer and the granules in goblet cells. Examination of normal sinus mucosa labeled with four gold-labeled lectins showed the distribution of sialoglycoconjugates to be mainly in the ciliary layer and the granules in goblet cells and restricted to the surface of the cilia, microvilli and the secretory light granules. The application of an influenza A virus infection decreased the labeling intensity of gold-labeled MAA in the cilia and the secretory granules but not of WGA. SNA gold did not label the surface of the cilia and granules in either case. PNA gold particles, however, labeled the cilia and the secretory granules very weakly in normal sinus mucosa, but labeled moderately in cases of influenza A virus infection. These results suggest that the sugar residues of sialoglycoconjugates consist of Neu5Ac(alpha2, 3)Gal, GlcNAc, Neu5Ac. They also suggest that the sugar residues serve as a protecting factor or modulator against influenza A virus infection.

2.Two Cases of Spontaneous Subcapsular Hematoma of the Kidney.

Spontaneous Subcapsular hematoma of the kidney is relatively uncommon. Although several cases have been recorded in the literature, the diagnosis was seldom made preoperatively. Selective renal angiography provide an excellent means of diagnosing this lesion prior to surgery. Case I: A 37 years male was admitted with the right flank pain and hematuria. Excretory urogram showed moderate hydronephrosis and a cystic shadow compressing upper calices. Selective renal angiography showed strip-like curvilinear capsular density outside the nonopacifying mass. By exploration, blood clots, about 300 c. c., between renal parenchyma and renal capsule was evacuated. The cause of the hematoma is uncertain whether it is from ruptured aneurysm or due to hydronephrosis. Case II : A 54-year-old woman was hospitalized with right flank discomfortness and mild fever. Intravenous pyelogram revealed increased size of the right kidney and angiogram demonstrated the compressed and flattened renal parenchyma by a nonopacifying mass and renal capsule on the outer margin of the mass. Blood clots, about 200 c.c., inside the renal capsule which compressed renal parenchyma was evacuated.

3.A Case of Clear Cell Sarcoma of the Anterior Chest.

Clear cell sarcoma of tendon and aponeurosis is a rare malignant tumor. It occurs chiefly in young adults, predominates in women and is most common in the regions of the foot and ankle. We report a case of clear cell sarcoma of tendon and aponeurosis in s 22-year-old man. he pstient had had a asymptomatic, normal skin colored, relativerly hard, dome shsped nodule on the anterior chest for 6 months. Histopsthologic findings revealed uniform pattern composed of compact nests of round or fusiform cells which had clear cytoplasm and were surrounded by delicate framework of fibrocollagenous tissue, and the individual tumor cell had a fairly regular appearance of possessing round to avoid vesicular nucleus with prominent basophilic nucleolus. One year after surgical excision and post operative radiotherapy, there was no recurrence.

4.Effects of Nimodipine on Dose-Response Curves of Vecuronium, Rocuronium, and Atracurium Using Phrenic Nerve-Diaphragm Preparations of Rats.

BACKGROUND: Interactions between nimodipine, a calcium channel blocker, used perioperatively for the treatment of subarachnoid hemorrhage, and vecuronium, rocuronium, and atracurium were studied with phrenic nerve-hemidiaphragm preparations of rats. METHODS: Male 200-300 g Sprague-Dawley rats were randomly allocated into four groups (control, NMD(5), NMD(50) and NMD(500) group, n = 10, respectively) according to the nimodipine concentration, and three groups (control, NMD(2D) and NMD(7D), n = 10, respectively) according to the pretreatment duration. A square wave 0.1 Hz supramaximal stimuli was applied to the phrenic nerve-hemidiaphragm preparation and the twitch height response was recorded with mechanomyography. The dose-response curves were measured, and ED(5), ED(50), ED(90), and ED(95) of each vecuronium, rocuronium, and atracurium in different concentrations of nimodipine of 5, 50, and 500 ng/ml and rocuronium in pretreatment with nimodipine 2.5 mg/kg/d for 2 and 7 days were calculated using an inhibitory sigmoid Emax model. RESULTS: The dose-response curves of rocuronium and atracurium were significantly shifted to the left in NMD(500) group, and significantly shifted to the right in NMD(7D) group (P < 0.05). In NMD(500) group, ED(50), ED(90), and ED(95) of rocuronium and atracurium were significantly reduced, and those of rocuronium in NMD7D group were significantly increased compared with the control group (P < 0.05). CONCLUSIONS: Nimodipine 500 ng/ml in the phrenic nerve-hemidiaphragm preparation of rat increased sensitivity to rocuronium and atracurium, and the dose-response curve was significantly shifted to the left, but following pretreatment for 7 days, nimodipine decreased the potency of rocuronium, and the dose-response curve was significantly shifted to the right.

8.The Relation Between Clinical Manifestations of Dry Eye Patients and Their BUTs.

Although the measurement of tear film break-up time (BUT) is the most frequently used diagnostic test for dry eye syndrome, there have been no reports for the relationship between clinical manifestations of dry eye patients and the degree of their BUTs. To investigate the relationship, we analyzed retrospectively clinical manifestations of 466 dry eye patients and categorized them into two groups in terms of BUT, the group of BUT <5 seconds and BUT >or=5 seconds, in various clinical manifestations. The two groups were compared by X2-test and the results were as follows. The two groups did not show statistically significant difference in sex, age, duration of symptoms, degree of corneal erosion, and time from the beginning of management to symptom improvement. Therefore we thought that there was no statistically significant correlation beteween the above-mentioned clinical manifestations of this study patients and their BUT's. There were more patients whose BUT were below 5 seconds than those having BUT above 5 seconds in group complaining foreign body sensation as a main symptom and in group having associated chronic conjunctivitis. 9.Clinical Study of Eye-Me(R) Soft Contact lens.

A clinical evaluation of Eye-Me(R) (Paik San, Korea), daily and extended wear, soft contact lenses was performed for a 3-month period. The two kinds of lenses were worn by 20 persons (40 eyes) in each group. Measurements of the central corneal thickness were taken in 10 eyes of extended wear group following 8 hours of sleep with contact lenses. The mean percent corneal thickness change after 8 hours of sleep with lenses was 1O.6 +/- 1.9%. At 3 months after lens wearing, good corrected visual acuity (over 0.8) was obtained in 33 eyes 36 eyes (91.7%) of daily wear group, and 32 eyes of 34 eyes (94.1%) of extended wear group, respectively, During the follow-up period of 3 months, mild adverse signs were noted in 8 eyes of each group. 85% of daily wear group and 90% of extended wear group were satisfied with Eye-Me(R) lenses. The above results suggest that the visual outcome and the safety of Eye-Me(R) soft contact lens could be acceptible for its clinical application.

10.Cosmetic Repair of Nevus of Ota.

PURPOSE: To report a case of cosmetically successful treatment of nevus of Ota performed by scleral painting and scleral allograft transplantation. METHODS: Scleral painting and allograft transplantation were done in a 28-year-old woman who had cosmetic problem due to nevus of Ota, and she was followed up for 5 months postoperatively. RESULTS: The patient had no specific postoperative complication and was satisfied with the surgical results. CONCLUSIONS: For patients of nevus of Ota who have not been treated successfully, scleral painting and allograft transplation can be an effective cosmetic treatment.
