1.Radiologic and clinical observation of tuberculous cavity in initial treatment

Tuberculous cavity is important in diagnosis and observation in the course of pulmonary tuberculosis. Authoranalyzed the radiologic findings of cavity and average months of negative conversion in AFB culture in 89 cases ofinitial treatment. The results were as follows: 1. The more number of cavities, the longer period in negativeconversion of AFB culture. 2. No relation between sums of diameter and thickeness of cavity and average months ofnegative conversion in AFB cultlure. 3. In the cases of cavity with air-fluid level took longer period in negativeconversion of AFB culture than those of cavity without air-fluid level, significantly. 4. No relation betweenradiologic findings of cavity and results of chemotherapy for pulmonary tuberculosis.

2.Adaptability & openbite tendency in early fuctional physiotherapy afte IVRO of the mandibular prognathism.

IVRO is a popular procedure to setback prognathic mandible. But guided functional physiotherapy after IVRO is the most important factor to get successful result, because there have been some concerns over an openbite tendency following this procedure. The purpose of this study is to evaluate the early adaptability to fuctional phsiotherapy and early openbite tendency associated with the kinds of surgery performed. The study was performed in fifty patients who had received bilateral IVRO from 1993 to 1994. After two weeks of maxillomandibular fixation(MMF), active physiotherapy was carried out according to Park's protocol and detail of openbite tendency was checked. We recorded the number of days for physiotherapy to analzye the adaptability in early physiotherapy until there was no more openbite tendency. The following results were obtained. 1. The adaptability in early physiotherapy to prevent openbite tendency following IVRO was more favorable in two-jaw surgery group than one-jaw surgery group.(p<0.005) 2. The adaptability in early physiotherapy was more favorable in lesser mandibular setback group than larger setback group, but there was no statistical significance. (p<0.05). 3. The adaptability in early physiotherapy was more favorable in posterior impaction group than non-posterior impaction group, but there was no statistical significance.(p<0.05). 4. The cases which MMF was applied again because of the strong openbite tendency were more plentiful in one-jaw surgery group than in two-jaw surgery group. 5. First two or three days after releasing of maxillomandibular fixation might be the critical period to prevent anterior openbite tendency, and if the partient shows strong openbite tendency during this period, 4-5 days of reapplication of MMF was enough to prevent openbite tendency .

3.Fracture of the Cartilagenous End Plate of the Lower Lumbar Vertebral body: 3 Case Report

Fracture of the vertebral end plate in the lumbar spine has been reported as a rare lesion occurring in the adolescent period. It is characterized by variable degrees of sign and symtom related to the degree of block on myelogram and the level of end plate fracture. Trauma or strenuous sport activity seems to play an improtant role in the production of fracture of vertebral body in the adolescent period. We experienced three cases of fracture of lower lumbar vertebral end plate in teenaged patients, two males and one female. Two patients showed upper end plate fracture with severe neurologic deficit on the involved root level and a strong positive on straight leg raising test, and one patient showed lower end plate fracture with slight neurologic deficit. Two patients with upper end plate fracture were treated with partial laminectomy with removal of fracture fragment and one patient with lower end plate fracture was treated with bilateral laminectomy and posterolateral fusion. The method of treatment will depend upon the type of fracture and the degree of neurologic symptoms. We obtained satisfactory results with posterior decompressive laminectomy and removal of fracture fragment with or without fusion.

4.Pelvic Arteriovenous Malformation: A Case Report.

Arteriovenous mafformation(AVM) of female pelvic organ is a rare disease of unknown cause. The authors' report a case of pelvic AVM which was incidentally found during US examination of the patient with choriocarcinoma after chemotherapy. The real-time sonography revealed several cystic lesions around the uterus with adjacent dilated tortuous vessels. The color Doppler sonography depicted abundant blood flow mixed with red and blue colors within the cystic lesions and rapid turbulent systolic and diastolic flows. CT showed well-enhancing round vascular lesions with elongated vessels in the pelvis, and MRI depicted signal-void cystic lesions on both T1 and T2 weighted images with small portions of high intensity within the lesions on T2 weighted image. The angiography revealed pelvic AVM fed by tortuous uterine and vaginal arteries with a dilated draining vein

6.Characterization of Principal Component Cell of DMBA induced Rat Malignant Fibrous Histiocytoma With Cell Culture and Cloning.

This experiment was performed to elucidate the cytologic origin of chemically induced MFH in Wistar rats. The tumor was produced by injections of DMBA(9,10-dimethyl-1,2-benzanthracene). With the produced MFH, cell culture and cloning were performed, followed by establishment of a cell strain, which was investigated by immunohistochemical and electron microscopic studies. The results were as follows. A) By immunohistochemistry of the tumor tissue, fibroblastic cells were positive for MEP-1(specific antibody for fibroblastlike cell of MFH, Takeya, 1993) and Anti-hPH(beta)(Anti-prolyl 4-hydroxylase beta), but negative for TRPM-3 and F4/80. Histiocytelike cells were positive for TRPM-3 and F4/80, but negative for MEP-1 and Anti-hPH(beta). In immunoelectron microscopy, normal spleen macrophage showed linear reactivity in cell membrane for TRPM-3, whereas histiocytelike cells of the tumor disclosed negative reaction. B) At 5 weeks of the primary tumor cell culture, the cells exhibited typical storiform pattern of MFH. C) The established cell strain revealed immunoreactivity for MEP-1 and Anti-hPH(beta), but negative for TRPM-3. The cloned tumor cells showed morphologic characteristics of undifferentiated fibroblastic cell. Latex particle (0.80 micrometer size) phagocytosis was negative in the cloned cell strain. The results of the current study support the concept that principal component cells of MFH is of fibroblastic cell origin.

8.The usefulness of thin-section rapid infusion CT for detection of cerebral aneurysm.

With 2mm-thick CT scanning during the rapid infusion of contrast material(TICT), cerebral aneurysms arising from the circle of Willis and adjacent vessels can be directly visualized. Twenty-five patients who had cerebral aneurysm confirmed by surgery were examined with TIGT and digital subtraction angiography. The authors examined TIGT prospectively to assess the detection rate of the cerebal aneurysms and to evaluate the clinical usefulness of TIGT. The detection rates of aneurysms by TIGT and digital subtraction angiography were 68% and 84%, respectively. TIGT is a rapid, safe and reliable method in the evaluation of patients with suspected cerebral aneurysm, permitting direct visualization of the aneurysm.

9.Evaluation of Enzyme Immunoassay for the Diagnosis of pulmonary Tuberculosis.

BACKGROUND: The diagnosis of tuberculosis has been based on the detection of tubercle bacilli by acid-fast stain of smear or cultures, and recently the serologic diagnosis of tuberculosis has been provided a means of sensitive and specific detection of Mycobacterium tuberculosis. We evaluated the utility of enzyme immunoassay using determiner Tuberculosis Glicolipids(TBGL) antibody kit(Kyowa Medex Co. Ltd, Japan) to detect anti-TBGL antibody for diagnosis of pulmonary tuberculosis. METHODS: Anti-TBGL antibody assay was performed to the form 44 patients with active pulmonary tuberculosis(17 patients with smear positive, 7 patients with only culture positive, 20 patients with clinically active tuberculosis) and 80 controls (30 healthy controls, 24 patients with non-tuberculous respiratory diseases, 26 patients with inactive tuberculosis). We compared the sensitivity and specificity of anti-TBGL antibody with culture and AFB stain. RESULTS: Anti-TBGL antibodies were detected in 16 of 17(94%) smear positive patients, 4 of 7 patients with only culture positive and 16 of 20(80%) smear negative patients who had been clinically diagnosed as active pulmonary tuberculosis. Nine(35%) out of 26 patients with inactive tuberculosis, one(4%) out of 24 patients with non-tuberculous respiratory diseases and no one of healthy control had a positive antibody response. Overall sensitivity, specificity of the anti-TBGL antibody assay were 82%, 88%, respectively and sensitivities and specificities of culture and AFB smear 64%, 97%, and 49%, 100%, respectively. Anti-TBGL antibody titers in patients with active tuberculosis were significantly higher than control grup(P<0.05). Conclusions : The anti-TBGL antibody assay was sensitive, rapid and convenient. This assay will be useful as a tool for the diagnosis of tuberculosis in combination with other conventional methods.
