1.Multiple Trichoepithelioma Occurring in a Son and his Mother.

A 23-year-old man had multiple small, skin-colored firm papules on the both sides of the nasolabial folds since the age of approximately 17. His mother had a similar skin lesions on her face. The histopathological findings of skin biopsy specimens from patient(son) showed horn cysts consist of a fully keratinized center surrounded by basophilic cells and tumor islands composed of basophilic cells arranged in a lacelike or adenoid network and occasional solid aggregates and there are also a foreign body giant cell reaction in the vicinity of the ruptured horn cysts.

3.A clinical and radiological study on spontaneous pneumothorax

A clinical and radiological study was done on 96 cases of spontaneous pneumothorax, encoutered in the Dept. ofRadiology, Busan National University Hospital during last 3 years from March. 1979 to March 1982. The results weresummarized as follows; 1. In the age distribution, the ages between 20 and 39 years were most highest, as 54 cases(56.3%). In the sex distribution, the ratio of male to female was 5:1 in male predominance. 2. The underlyingpathology of the total 96 cases of spontaneous pneumothorax were of tuberculous origin in 33.3% andnon-tuberculous origin in 66.7%. And below 20 years, most were of non-tuberculous origin. 3. In the cases of lungcollapse over 2/3, non-tuberculous origin was more than tuberculous origin and had characteristics of significantmediastinal shifting, in contrast to lower percentage of fluid level by chest radiography. 4. The rupture of blebsor bullae was the main immediate causes of spontaneous pneumothorax, independent of the underlying pathology. 5.In only 27 cases (28.1%) among total 96 cases, bullae or blebs could be detected on the chest radiography. 6. Intreatment of spontaneous pneumothorax, the closed thoracotomy with under water seal drainage is accepted to be thegeneral method of treatment. But open thoracotomy is considered as the best useful therapeutic procedure toprevent the recurrence, whenever bullae of blebs are found on the chest radiography. 7. In the cases of closedthoracotomy, the recurrent rate was 25.0% and most cases were found at the ipsilateral side of the first attack.8. Within a week, the collpased lung were well expanded in most cases of total 96 cases, after closed thoracotomy.

4.A Case of Progressive Idiopathic Atrophoderma.

Progressive idiopathic atrophoderma of Pasini and Pierini is an asymptomatic atrophic disorder of the skin characterized by sharply demarcated, slightly depressed and slate-gray to brownish patches. The affected skin may be thin but of normal consistency. We herein described a 32-year-old female who showed on the trunk and extremities well-defined, brownish and atrophic areas without induration, but the central portion of the abdominal lesion was slightly thickened. Histopathologic findings showed slight thinning of the epidermis and dermis with mild perivascular infiltrate, however, the central portion of the abdominal lesion showed a focal, slightly sclerotic change of the collagen fibers.

5.A Case of Mycosis Fungoides.

We present a case of mycosis fungoides of an 52-year-old woman who showed multiple pruritic erythematous papules, plaques with ulcers on the trunk, extremities and face. Her skin biopsy specimens were studied irnmunohistochemically with, monoclonal antibodies against T cells(80%, Leu-4, OKT11), T-cell subsets (60% Leu-3a, OKT4a, 40% OKT8) and Langerhans cell(OKT6). We think that this case should be considered in the differential diagnosis of adult T-cell leukemia/lymphoma. The proliferating cells were negative for anti-Tac.

6.Treatment of the Fracture of the Middle Third of Clavicle by Intramedullary Threaded Steinmann Pin Fixation

Clavicle fractures are frequently seen with the recent increase in traffic and industrial accidents. Recently, in adults, open reduction and internal fixation techniques are commonly performed for the patient to return earlier to their jobs. But internsl fixation technique for clavicle fractures has been a subject of controversy. We operated on 78 cases of fractures of the middle third of clavicle by the technique of intramedullary threaded Steinmann pin fixation from March 1985 to Nov. 1988. The following results were obtained. 1. The funtional results were excellent; 32 cases(41%), good; 41 cases(53%), fair ; 5 cases (6%). 2. The complications include motion limitation ; 5 cases(6%), pin tract infection ; 5 cases (6%), delayed union ; 1 case(1%) 3. The advantages of intramedullary threaded Steinmann pim fixation are as follows. 1) Early active range of motion is possible. 2) The operation procedure is simple to perform. 3) Removal of the pin is easy without anesthesia. Therefore, intramedullary threaded Steinmann pin fixation is thought to be a good operative technique for the clavicle fracture.

7.MRI Evaluation of the Vascular Pattern in Intracranial Meningioma.

PURPOSE: The purpose of this study was to assess the usefulness of MRI in the evaluation of vascular changes of the meningioma. MATERIALS AND METHODS: We retrospectively studied the MRI findings in 20 cases of intracranial meningiomas. The images were assessed for vascular hilum, vascular rim, sinus invasion, and vascular encasement. Cerebral angiograms were obtained in 15 cases and compared with MRI findings. RESULTS: MRI was as accurate as angiography in detection of vascular hilum(MRl=8/20, Anglogram=9/15), vascular rim(MRl=14/20, Angiogram=9/15) and dural sinus invasion(MRl=4/6, Angiogram=5/6). MRI was superior to angiography in detection of vascular encasement(MRl=2/3, Angiogram=0/2) and cavernous sinus invasion(MRI=3/3, Angiogram=0/2). CONCLUSION: MRI is an excellent noninvasive imaging modality in evaluating vascular changes of omas.

8.Trends in the Quality of Primary Care and Acute Care in Korea From 2008 to 2020: A Cross-sectional Study

Objectives:
Measuring the quality of care is paramount to inform policies for healthcare services. Nevertheless, little is known about the quality of primary care and acute care provided in Korea. This study investigated trends in the quality of primary care and acute care.
Methods:
Case-fatality rates and avoidable hospitalization rates were used as performance indicators to assess the quality of primary care and acute care. Admission data for the period 2008 to 2020 were extracted from the National Health Insurance Claims Database. Case-fatality rates and avoidable hospitalization rates were standardized by age and sex to adjust for patients’ characteristics over time, and significant changes in the rates were identified by joinpoint regression.
Results:
The average annual percent change in age-/sex-standardized case-fatality rates for acute myocardial infarction was -2.3% (95% confidence interval, -4.6 to 0.0). For hemorrhagic and ischemic stroke, the age-/sex-standardized case-fatality rates were 21.8% and 5.9%, respectively in 2020; these rates decreased since 2008 (27.1 and 8.7%, respectively). The average annual percent change in age-/sex-standardized avoidable hospitalization rates ranged from -9.4% to -3.0%, with statistically significant changes between 2008 and 2020. In 2020, the avoidable hospitalization rates decreased considerably compared with the 2019 rate because of the coronavirus disease 2019 pandemic.
Conclusions
The avoidable hospitalization rates and case-fatality rates decreased overall during the past decade, but they were relatively high compared with other countries. Strengthening primary care is an essential requirement to improve patient health outcomes in the rapidly aging Korean population.
9.Korean Early Childhood Education and Care Panel Study: Data Utilization Strategies for Policy and Practice

The Korean Early Childhood Education and Care Panel Study (K-ECEC panel study), commencing in 2021, aimed to collect data related to early life growth and development. In particular, based on Bronfenbrenner’s ecological model, the study aims to collect intrinsic and extrinsic parental factors, educational and caregiving experiences in institutions, and environmental factors from the prenatal stage through infancy, toddlerhood, and early childhood. This is important for understanding child development with varied dimensional aspects. The K-ECEC panel study uses a stratified sampling strategy in order to recruit pregnant women based on regions and numbers of deliveries to individual hospitals. This resulted in the participation of 3,372 women, 95 of whom were from low-income families and 72 from multicultural families. The K-ECEC panel study is based on a household survey collecting information related to pregnancy, birth, and childrearing. The survey included an annual follow-up for individual households of newborns from 2022, and the second round of the survey was conducted in 2023. A total of 2,775 children participated in the first round of the survey, 1,423 of whom were boys. As there are few birth cohort studies considering policy-oriented aspects in Korea, the K-ECEC panel study is expected to provide data to understand childhood trajectories for growth and development. Furthermore, in terms of childcare and education, it is expected to be utilized to evaluate the policy effects of early life and parenting support.
