1.Acute cerebral infarction following aconitine ingestion

Aconitine is a main component of Aconitum carmichaeli, a Chinese herb known to be effective for
arthritis and neuralgia. Many senior citizens consume the herb as a folk remedy. Aconitine-associated
cardiogenic toxicity has been observed but aconitine-induced cerebrovascular event has not been
reported so far. We report two elderly patients who became unconscious and showed neurological
dysfunction soon after ingesting aconitine for pain control. We speculate that the aconitine induced
cardiac arrhythmia. This resulted in cardiogenic emboli formation with subsequent large territory
cerebral infarction.

3.The change of visual acuity and visual field by diminished illumination in eyes with multifocal intraocular lens.

To investigate the effect of illumination on visual acuity and visual field in eyes with multifocal intraocular lens (IOL), the near and far visual acuity of the eyes with multifocal implants were compared to those of the eyes with monofocal implants in the diminished illumination, and the visual fields of two groups were also compared by the different target size and intensity. The near and far visual acuity of the eyes with multifocal implants were significantly decreased as compared to those of the eyes with monofocal implants under diminished illumination, and the visual field of the multifocal implanted eyes also significantly decreased as compared to that of the monofocal implanted eyes by the reduction of spot size and intensity. From the above results, it is assumed that the bifocal separation of incident light to two simultaneous focal points in the multifocal implants causes more decreased visual acuity and narrower visual field in the diminished illumination as compared to that of the eyes with monofocal implants.

4.Vascular risk factors for stroke among urban community dwelling adults in Ansan city, Korea

Background & Objective: The number of ischemic stroke is increasing steadily. The recent stroke
prevention strategies have targeted risk factors for preventing ischemic stroke. Community-based data
that quantify the prevalence of these risk factors are needed to develop effective stroke prevention
strategies. This study was performed aiming to establish customized prevention strategies by investigating
vascular risk factors of ischemic stroke among the healthy adults in an urban population in Korea.
Methods: The Prevention of Stroke and Dementia (PRESENT) survey collected data associated with
stroke risk factors between 2007 and 2009 in Ansan city, Korea. Of 46,537 people, 2,843 were selected
by random sampling, and final population included 358 men and 422 women over 50 years old. We
checked modifiable risk factors, such as hypertension, diabetes mellitus, smoking, hyperlipidemia,
atrial fibrillation, cardiovascular disease, obesity, and physical inactivity. Results: The mean age of
study subjects was 60.1±8.2 years; 55.5% had hypertension, 18.6% had diabetes mellitus, 17.7% were
current smoker, 44.9% had hyperlipidemia, 1.7% had atrial fibrillation, and 4.5% had cardiovascular
disease. Men had more risk factors for stroke, and higher rates of diabetes mellitus and current
smoking. However, women had higher cholesterol and obesity rates and lower physical activity. The
proportion of individuals with >1 risk factor for stroke increased with age. Hypertension, diabetes
mellitus, and cardiovascular diseases also increased with age, but low-density lipoprotein levels and
current smoking decreased.
Conclusions: This study provides valuable information to develop customized strategic policies for
primary and secondary stroke prevention.

5.Bronchioloalveolar Cell Carcinoma in Solitary Pulmonary Nodule(SPN) with Cavitary Lesion.

Lung cancer is the most common fatal malignant lesion in both sexes. Detection of the solitary pulmonary nodule is important because surgical series up to a third of solitary pulmonary nodules are bronchogenic carcinoma. Bronchioloalveolar cell carcinoma is a rare primary lung cancer and surgery is treatment of choice in brochioloalveolar cell carcinoma. We experienced a case of broschioloalveolar cell carcinoma in solitary pulmonary nodule with cavitary lesion in chest CT scan, which is an uncommon finding in brochioloalveolar cell carcinoma.

6.The Relationship between the Capillary Nonperfusion Area and the Retinal Nerve Fiber Layer Defects in Branch Retinal Vein Occlusion Patients.

PURPOSE: To determine the relationship between the capillary nonperfusion area and the retinal nerve fiber layer (RNFL) defects in branch retinal vein occlusion (BRVO) patients. METHODS: We compared the difference between the RNFL thickness of both eyes in unilateral BRVO patients using the scanning laser polarimeter (GDx R ). The mean RNFL thickness was calculated for each quadrant. Fluorescein angiography was done to determine the retinal capillary nonperfusion area and we evaluated the relationship between the retinal capillary nonperfusion area and the right-left difference of RNFL thickness. RESULTS: In left superior temporal BRVO group, the Average of RNFL was thinner (p=0.015) and the Integral was smaller (p=0.045) in the left BRVO eye than in the right normal eye in the superior temporal sector. In patients with the other sector BRVO, there was no significant difference of RNFL thickness between both eyes. As the retinal capillary nonperfusion area increased, the right-left difference of Average increased in the superior temporal sector in patients with left superior temporal BRVO (p=0.042). CONCLUSIONS: The scanning laser polarimeter allowed the quantification of RNFL defect in patients with BRVO, and it may help to evaluate the quantification of retinal ischemia.

7.Evaluation of in-Hospital Cardiac Arrest According to the in-Hospital Utstein Style.

BACKGROUND: The in-hospital 'Utstein style' is international recommended guidelines for reporting outcome data from in-hospital resuscitation events. This study was designed to evaluate the current status of in-hospital cardiopulmonary resuscitation(CPR), and to provide basic data far the unified reporting guideline of resuscitation in Korea. METHODS: The patients who had received CPR in a university hospital were searched during one year period from March 1997. The variables according to the Utstein style were evaluated with review of the medical records. RESULTS: During the period, 428 patients had received CPR. Excluding the patients of out-of-hospital arrest and less than 8 years, 242 were enrolled. Their initial ECG rhythms were 55 of asystole, 148 of pulseless electrical activity, and 39 of ventricular fibrillation, The spontaneous circulation was returned in 118(49%, and 48(20% were maintained more than 24 hours. Twenty-two patients(9%) were discharged with spontaneous respiration. Among the survivors, 17 had the Cerebral Performance Category of 1. CONCLUSION: We recommend the Utstein style to report the outcome of in-hospital CPR.

8.MR Imaging of Anterior Cruciate Ligament Injury: Associated Findings.

PURPOSE: Authors investigated the associated findings and their value in the diagnosis of anterior cruciate ligament(ACL) injury in MR image. MATERIAL AND METHODS: The knee MR images of 47 patients with ACL injury(complete; 24, partial; 23) and 61 patients with normal ACL confirmed by the knee arthroscopy or operation were reviewed retrospectively. The degree of anterior translocation of tibia and the degree of posterior cruciate ligament(PCL) buckling were evaluated. The prevalence and pattern of associated adjacent bone, ligament and meniscus injuries were studied. RESULTS: The means( +/- 2standard errors) of anterior translocation were different significantly in statistical analysis(p<0.001, student t-test) between injury group(7.51 +/- 1.16mm) and normal group(-0.56 +/- 0.92mm). In the level of 5mm of anterior translocation for the criteria of ACL injury, the sensitivity, specificity, accuracy were 78.7%, 89.5%, 84.3% for each. The means of PCL buckling ratio were also different statistically b. etween injury group(0.23 +/- 0.02) and normal group(0.17 +/- 0.01)(p<0.001). In the level of 0.20 for diagnostic criteria of ACL injury, the sensitivity, specificity, accuracy were 71.4%, 83.6%, 78.4% for each. Thirty one medial meniscus tear(66%), thirteen lateral meniscus tear(28%), ten medial collateral ligament injury(28%), one PCL injury (2%) were associated with ACL injury. The twenty nine bone marrow changes were found in twenty patients {43%) which included acutely injured seven patients. In acute cases, the bone marrow changes were depicted as diffuse or focal high signal intensity lesions in lateral femoral or tibial condyles in contrast to the changes in chronic cases depicted as focal low signal intensity lesions in variable location. Lateral femoral condylar notch depression were found in nine patients(19%) and avulsion fractures of anterior tibial spine in four patients(9%). CONCLUSION: The associated findings with ACL injury (anterior translocation, buckling of PCL, associated bone, ligament and meniscus injuries) are considered to be helpful in the diagnosis of ACL injury on Knee MR images, when the findings of anterior cruciate ligament itself are not confirmative.
