3.Sepsis in Patients Receiving Immunosuppressive Drugs in Korea: Analysis of the National Insurance Database from 2009 to 2013.

BACKGROUND: The aim of this study is to evaluate the influence of immunosuppressants on in-hospital mortality from sepsis. METHODS: Using data of the Health Insurance Review & Assessment Service, we collected data from patients who were admitted to the hospital due to sepsis from 2009 to 2013. Based on drugs commonly used for immunosuppression caused by various diseases, patients were divided into three groups; immunosuppressant group, steroid-only group, and control group. Patients with no history of immunosuppressants or steroids were assigned to the control group. To identify risk factors of in-hospital mortality in sepsis, we compared differences in patient characteristics, comorbidities, intensive care unit (ICU) care requirements, and immunodeficiency profiles. Subgroup analysis according to age was also performed. RESULTS: Of the 185,671 included patients, 13,935 (7.5%) were in the steroid-only group and 2,771 patients (1.5%) were in the immunosuppressant group. The overall in-hospital mortality was 38.9% and showed an increasing trend with age. The steroid-only group showed the lowest in-hospital mortality among the three groups except the patients younger than 30 years. The steroid-only group and immunosuppressant group received ICU treatment more frequently (p < 0.001), stayed longer in the hospital (p < 0.001), and showed higher medical expenditure (p < 0.001) compared to the normal group. Univariate and multivariate analyses revealed that age, male gender, comorbidities (especially malignancy), and ICU treatment had a significant effect on in-hospital mortality. CONCLUSIONS: Despite longer hospital length of stay and more frequent need for ICU care, the in-hospital mortality was lower in patients taking immunosuppressive drugs than in patients not taking immunosuppressive drugs.

4.HLA Antigens, T and B Lymphocytes and other Associated Laboratory Findings in Patients with Vitiligo.

Although vitiligo is a common disorder with well-known clinical features, its etiology is still vague. It has been suggested that immunologic factors are important in the pathophysiology of vitiligo. And the association of HLA antigens with vitiligo has been reported. Authors examined HLA phenotypes in thirty-nine patients, T lymphocytes in thirty- three patients, B lymphocytes in thirty-one patients, and quantitation of IgG, IgA, IgM and IEP(immunoelectrophoresis) in twenty-five patients with vitiligo in this study. T3 and T4 were investigated in twelve patients, CBC in thirty-nine and urinalysis in thirty-five patients with vitiligo for screening of associated diseases ; thyroid disease, anemia and diabetes mellitus. The results are as follows; The frequency of HLA A9, Bw15 and Bw22 was significantly decreased in vitiligo patients compared to control group(p=0. 003, 0. 02, 0. OR) 2. There were no significant differences in the results of total and active lymphocytes and B lymphocytes in vitiligo patients compared to the control group. The results of quantitation of IgG, IgA, IgM and IEP showed within normal lirnits except one, an increased IgG, 2,200rng/100ml (normal: 500 1,500) and polyclonal gammopathy. 4. Among twelve vitiligo patients, one showed increased TR, 355ng/100ml (normal: 80 220) and T4, 17. 3pg/100ml (normal: 5 13). 5. CBC and urinalysis were within normal limits in all patients.

5.A Case of Localized Cold Urticaria.

Cold urticaria is a group of distinct clinical entities with different clinical manifestations that share the common feature of an urticarial response to cold exposure. In the localized cold urticaria, it is confined to portions of the body that have contact with the cold. In this article, we report an unusual patient with cold urticaria in which cold exposure causes hives in localized region of the face and the ice-cube test was positive only in that area.

Transepidermal elimination is a mechanism whereby foreign or altered constituents can be removed from the dermis. The phenomenon of transepidermal elimination may occur as a primary process characterizing disorders such as elastosis perforans serpiginosa, reactive perforating collagenosis and chondrodermatitis nodularis chronica helicis; as well as a secondary process characterizing disorders such as granuloma annulare, necrobiosis lipoidica diabeticorum, calcinosis cutis, pseudoxanthoma elasticum, spitz nevi, pimented nevi, porokeratosis plantaris discreta and metastatic tumors. A 17-year-old man presented with a 2X2 cm sized well-circumscribed slightly erythematous plaque with some black colored papules on the left dorsum of his hand. The histopathology of the lesion indicated angiokeratoma circumscriptum with transepidermal elimination.

7.A Case of Squamous Cell Carcinoma Arising in Nevus Sebaceus.

Various types of appendage tumors develop secondarily within lesions of nevus sebaceus. A syringocystadenoma papilliferum is the most common benign tumor while basal cell carcinoma is the most common malignant tumor developed within a nevus sebaceus. However, a squamous cell carcinoma rarely develops within a nevus sebaceus, although it may be aggressive. We report a case of a 44 year-old man who had a fingertip-sized hemorrhagic crusted mass in a 3x4cm sized nevus sebaceus which had developed since birth on the right occipital scalp. This mass has rapidly enlarged into 1x2cm sized growth in the past 4 months. Histopathologic examination of the lesion showed that infiltrative squamous cell carcinoma with good or moderate differentiation indicatied the lesion of nevus sebaceus. Wide excision of squamous cell carcinoma including the whole lesion of nevus sebaceus was performed. Three months later, squamous cell carcinoma recurred in the same part of the scalp. Another wide excision of the recurrence was performed, but recurred again in the same part with invasion into the muscle layer. The recurrence is currently being treated by radiotherapy.

9.Analysis of methods for the generation of dendritic cells from human peripheral blood monocytes.

Dendritic cells (DC) are highly efficient antigen-presenting cells that initiate the primary immune response. Several laboratories have developed culture systems for human DC from peripheral blood monocytes. Most of these studies have used fetal calf serum (FCS) containing culture conditions that are inappropriate for human application. GM-CSF and IL-4 were used to make immature DC. The monocyte-conditioned medium (MCM) was used to induce the final maturation of DC. Using the previously described methods, the quality of MCM has unpredictable variations. Therefore using a defined cocktail of growth factors for the generation of mature DC would be advantageous for experimental as well as clinical purposes. In this study, it is suggested that combinations of both GM-CSF/IL-4 or GM-CSF/IL-13 could be used as the first-step culture to produce immature DC, and that cytokine cocktail (GM-CSF, IL-4, IL-1beta, TNF-alpha, IL-6, PGE2) was as efficient as MCM for the second step-culture to produce fully maturated DC. Here, we have generated an easily reproducible culture system for DC that allows for the generation of large amounts of immature and mature DC, and we also now have established the method in a FCS-free system that is suitable for clinical use.
