1.Effects of Trastuzumab Combined with Neoadjuvant Chemotherapy on Clinical Efficacy and Related Index-es of Breast Cancer Patients after Surgery

OBJECTIVE:To investigate the effects of trastuzumab combined with neoadjuvant chemotherapy on clinical effica-cy of breast cancer patients,serum angiogenic factors and apoptosis factors of breast tissue. METHODS:A total of 116 breast can-cer in patients were selected from our hospital during Jan. 2012-Dec. 2014 as research object,and then divided into control group and observation group according to random number table,with 58 cases in each group. Control group was given Carboplatin for in-jection 100 mg(added into 500 mL 5% Glucose injection after diluted into 10 mg/mL),ivgtt,200-400 mg/m2 on the first day of each chemotherapy cycle;Docetaxel injection ivgtt,75 mg/m2 on the first day of each chemotherapy cycle. On the basis of control group,observation group was additionally given Trastuzumab for injection,4 mg/kg in the first week,2 mg/kg in the 2nd-8th week,once a week,ivgtt. A treatment course lasted for 3 weeks,and both groups received 6 courses of treatment. Both groups re-ceived modified radical mastectomy 2 weeks after treatment. The levels of serum angiogenic factors and apoptosis factors of breast tissue were observed in 2 groups before and after treatment. Clinical efficacies and the occurrence of ADR were compared between 2 groups 1 year after treatment. RESULTS:Before treatment,there was no statistical significance in the levels of serum angiogenic factors and apoptosis factors of breast tissue between 2 groups (P>0.05). After treatment,the levels of serum angiogenic factors and apoptosis factors of breast tissue were decreased significantly in 2 groups,and the observation group was significantly lower than the control group,with statistical significance(P<0.05). Clinical response rate of observation group was 82.76%,which was significantly higher than 56.90% of control group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Trastuzumab combined with neoadjuvant chemotherapy is helpful to improve the therapeutic effect with breast cancer, prevent recnrence,and reduce the expression of serum angiogenic fac-tors and apoptosis factors of breast tissue with good safety.

3.Evaluation of ultrasonic technology for monitoring children's cardiac output

Objective To explore the accuracy of the ultrasonic cardiac output monitor(USCOM) device for estimation of children's cardiac output.Methods We conducted a prospective study in Beijing Children's Hospital.Sixty healthy children were enrolled in this study.We evaluated the accuracy of USCOM monitor device in the measurement of cardiac output by comparing with the measurements obtained by the M-Teichholz formula and the Doppler aortic blood flow method.Results The data from the USCOM monitor device had better consistency with those from the M-Teichholz formula (r=0.88,P<0.05),and there was no significant difference (P>0.05) between the two methods.The similar results were found between the USCOM method and the Doppler aortic blood flow method (r=0.83,P<0.05).Conclusion This study shows that cardiac output monitoring by USCOM is accurate,and USCOM is a promising technique in cardiac output monitoring.
4.Research advance on exosome-based cancer immunotherapy

Exosomes are small membrane vesicles secreted into the extracellular medium by a broad array of cells in culture.Exosome proteic and lipid composition is unique and might shed some light into exosome biogenesis and functions.Exosomes secreted from professional antigen-presenting cells(i.e.dendritic cells) and tumor cells are enriched in MHC class Ⅰ and Ⅱ complexes,costimulatory molecules,Hsp70-90 chaperones and tumor antigen,and have therefore been more extensively studied for their immunomodulatory capacities in vitro and in vivo.In this review,we will describle the definition and composition of exsomes and the main biological features pertaining to tumor or DC-derived exosomes,and will also present the early clinical results and limitation of their implementation in cancer immunotherapy.
5.Surgical treatment of secondary hyperparathyroidism in patients with chronic renal failure

Secondary hyperparathyroidism (SHPT) is a common complication in patients with chronic renal failure (CRF).Medical treatment may be the first choice for most SHPT patients,but parathyroidectomy (PTX) is neededwhen medical treatment doesn't work in patients with refractory SHPT.However,there is no strong evidence to support which surgical procedure is the most effective one for patients with SHPT.An interdisciplinary discussion between nephrologists and surgeons is needed when choosing a preferred surgical method for refractory SHPT patients.The factors must be discussed including:the patient's age,the underlying kidney disease,clinical symptoms,the level of intact parathyroid hormones (iPTH) and blood calcium,the patient's ability to obtain and comply with medication treatment and the estimated duration of dialysis before kidney transplantation.Refractory SHPT patients will benefit from the surgical operation treatment and reoperation of recurrent SHPT is still an ideal treatment method.
6.Optimization of Water Extraction Technology of Medicinal Material in Lian-wei Anacyclus Formulation

OBJECTIVE:To optimize the water extraction process of medicine in lian-wei anacyclus prescription. METHODS:Composite score of polysaccharide content and raw polysaccharide were set as investigation index,the different adding water amount,extracting times and extraction duration were set as investigation factors,and L9(34)orthogonal test were conducted to op-timize the water extraction technology of medicine in lian-wei anacyclus prescription and verification test was carried out. RE-SULTS:Extracting times had the most significant effect on the investigation index;the most optimized process was as follows as 12 times of adding water amount,2.0 h of extracting times,3 times of extraction. The average content of polysaccharide was 36.34μg/ml(RSD=0.24%,n=3)in the verification test,and the average content of raw polysaccharide was 10.18%(RSD=0.65%, n=3). CONCLUSIONS:The optimized technology is reasonable,feasible and reproducible,and can provide reference for the dos-age form transformation.
7.A study on acid-base disorders during cardiac surgery with cardiopulmonary bypass

1727 arterial blood gas samples of 341 cases of acyanotic congenitla heart diseases,79 cases of cyanotic congenital heart diseases and 88 cases of acquired heart diseases during cardio-pulmonary bypass surgery were analyzed.It was found that there were various degrees of acid-base disorders right before the bypass was on.During the speration,acid-base imbalance was aggravated because of anesthesia,fluid infusion,hypothermia,surgical manipulation,etc.Respiratory alkalosis was most common to occur,and metabolic acidosis was the secodn.After the cessation of bypass,acid-base disorders could be significantly alleviated or imporved.So it is belived that it is very important to monitor the changes of blood gases and acid-base balance and to manage timely the complications during cardiopulmonary bypass.Fluid infusion,administration of agents to improve the microcirculation.and replacement of alkaline electrolytes are helpful to maintain acid-base balance.
8.THE SIGNIFICANCE OF URINARY ?_1-MICROGLOBULIN AFTER EXPOSURE TO ACCELERATION OF DIFFERENT SPEED

Objective To observe the changes in contents of ? 1 Microglobulin (? 1 M) in urine after exposure to acceleration of different speeds(+Gz) in order to provide scientific indications for efficient protection for pilots under high +Gz acceleration and efficient flight health service support. Methods Six healthy men were subjected to the following experiments: ①pressurized anti G suit (KH X); ②pressure breathing for +Gz (PBG); ③pressurized anti G suit and PBG and AGSM (up to +9Gz for 10 s). Urine samples were collected before, immediately after, 30 minutes and 24 hour after the exposures for the determination of ? 1 microgolbulin (? 1 M). 9.Diagnosis and Treatment of Intestinal Duplication in Children by Laparoscopy

Objective To evaluate the clinical value of laparoscopy in the diagnosis and treatment of intestinal duplication for children. Methods From June 2002 to May 2007, 15 children (aged 27 days to 10 years) with intestinal duplication were treated by laparoscopy in our hospital. The lesions were explored by laparoscopy, and then pulled out through the umbilical hole or the trocar hole at the right lower abdomen. The anastomosis was performed outside the abdominal cavity. Results The operation was successfully accomplished in all the patients. No serious postoperative complications occurred. Among the cases, end-to-end anastomosis was performed on 12 patients, resection of the duplicated intestine on 2, and demucosation on 1. Simultaneously, appendectomy was performed on 2 cases, and ring suturing for inguinal hernia on 1. The patients were followed up for 3-18 months (mean, 12 months), during which no patients had symptoms of intestinal adhesion (abdominal distension, vomiting), hemorrhage, or incisional hernia. Conclusions Laparoscopy is effective and reliable with a high diagnostic rate and low complication rate in the diagnosis and treatment of intestinal duplication for children.
