1.Progress in the studies of colon cancer recurrence

Colon cancer is one of the most common cancers in China, with an increasing incidence these years. Currently, the combined therapy based on radical surgery has dramatically improved the survival rate and life quality of patients, however, a large percentage of patients still died of recurrence and metastasis. In recent years, the treatment strategies for the recurrence of colon cancer have been greatly advanced, especially the new targeted drugs and chemotherapeutic drugs, which lead to a significantly improved therapeutic efficacy and a better life quality. Therefore, how to choose and use these new treatments synthetically and reasonably become a great challenge to all surgeons, especially to digestive surgeons.
2.Effects of hydrogen sulfide on hepatic ischemia-reperfusion injury in rats

Objective To investigate effects of hydrogen sulfide on hepatic ischemia-reperfusion injury in rats.Methods Thirty healthy male SD rats weighing 220-250 g were randomly divided into sham operation group (S group),ischemia-reperfusion group(IR group) and different doses of hydrogen sulfide groups (H2S1~3 groups),every group contains six rats.In S group hepatic portal was only exposed but not ligated.In IR group the left and middle lobes of hepatic pedicle,portal vein and hepatic artery were clamped for 1 h and then reperfuse.H2 S1~3groups received intraperitoneal injection of NaHS 14,28,56 μmol/kg at 5 min before reperfusion.Blood samples were collected for measuring the activity of alanine transaminase (ALT) and aspartate transaminase (AST) in serum at 6 h after reperfusion.Liver tissue samples were collected for measurement the glutathione (GSH) content and pathologic examination.Results Compared with S group,the activity of ALT and AST in serum was increased,GSH content in liver tissue decreased in IR group ; Compared with IR group,the activity of ALT and AST in serum was decreased,GSH content in liver tissue increased in H2 S1~3 groups.Hepatic tissue pathologic injury was reduced in H2 S1~3 groups as compared with IR group.Conclusion Hydrogen sulfide can reduce hepatic ischemia-reperfusion injury in rats.
3.Clinical value of spiral CT in diagnosis of fracture of tibia plateau

Objective To investigate the clinical value of spiral CT in diagnosis of fracture of tibia plateau, to provide a reference for clinical treatment.Methods 50 cases of tibial plateau fracture were selected,they were diagnosed with X -ray and diagnostic multislice spiral CT three dimensional reconstruction scan respectively.The fracture type and so on of the two groups were compared.Results 50 patients were diagnosed as 14 cases of type Ⅰ, type Ⅱ 10 cases,Ⅲ type 6 cases,Ⅳ type 7 cases,Ⅴ type 5 cases,Ⅵ 8 cases,CT three -dimensional reconstruction diagnosis accuracy rate was 100.00%.X -ray diagnosis was accurate in 41 cases,diagnostic accuracy rate was 82.00%,the difference of two diagnostic ways was statistically significant (χ2 =8.657,P =0.000).All patients were not detected with significant artifacts,the line was good,the fracture line was blurred and visible fracture callus formation surgical outcome was good.Conclusion Spiral CT diagnosis in tibial plateau fracture has high accuracy,through clear and intuitive display morphological characteristics of the patients improves anatomical repair treatment,it is worthy of clinical application.

5.Application of dental operative microscope in detecting the mesiobuccal root canal orifice of the maxillary first permanent molar

Objective To observe the effect of the dental operative microscope(DOM) and the ultrasonic apparatus in detecting the mesiobuccal root canal orifice of the maxillary first permanent molar(MFPM). Methods According to traditional method,the pulp chamber was opened and the second mesiobuccal root canals(MB2) were explored in 76 MFPM teeth.If MB2 was not found,then we modified the shapes of the pulp chambers and detected the orifices.At last,MB2,not found under naked eyes,was explored with DOM and ultrasonic technique. Results The detection rate of the second mesiobuccal orifice of MFPM was 81.6%.The rate was only 31.6% when using traditional method,while after modifying the shapes of the pulp chambers there was an increase of 30.2% under naked eyes and then another 19.7% increase was obtained by using DOM and ultrasonic apparatus.The distance between two mesiobuccal root canal orifices was(1.42?0.51) mm. Conclusion It is helpful to find some invisible orifice of MB2 by using DOM and ultrasonic technique,and it would be beneficial to improve the root canal therapy.
6.Effects of chronic cigarette smoking on hypoxic pulmonary vasoconstriction in rats——role of prostaglandins and leukotrienes

Investigation of the roles of prostaglandins(PGs) and leukotrienes(LTs) inthe changes of hemodynamics and hypoxic pulmonary vasoconstriction(HPV) induced bychronic cigarette smoking in Wistar rats.After exposure to either smoke or room air, rats were anesthetized, the changes ofhemodynamics and hypoxic pulmonary vasoconstriction were measured. Blood samplesdrawn through the carotid arterial cannula were analyzed with radioimmunoassay(RIA)for PGs and bioassay for LTs in plasma. The results showed that cigarette smoking for onemonth did not change the pulmonary arterial pressure nor the pulmonary vascularresistance(PVR) in rats, while the pressure response of pulmonary vessels to hypoxiawas reduced significantly, which could be abolished by indomethacin. Cigarette smokingcould increase the concentration of 6-Keto-PGF_1?but not that of TXB_2, in arterial plasma,during hypoxia and it reduced the plasma level of LTs as well. These results suggestedthat chronic cigarette smoking might have reduced HPV by two ways: i. e. increasing theptoduction of vasodilative PGs, especially PGI_2, and reducing LTs production duringhypoxia.
7.CLINICAL ANALYSIS ON 16 CASES OF 21-HYDROXYLASE DEFICIENCY

To analyze the clinical characteristics of 21 hydroxylase deficiency and to improve its diagnosis and treatment, medical records of 16 in patients with 21 hydroxylase deficiency were carefully and retrospectively reviewed. The results showed that of the 16 cases, 1 was male with increased penile size, but no testicular enlargement. 15 were females with masculinized external genitalia to varying degrees even chordee in some cases, pigmentation (66 7%), hirsutism (60%), and ovarian dysfunction(40%). Laboratory tests showed elevated levels of serum ACTH, 24 hour urinary 17 KS, 17 KGS, and the results of medium dose dexamethasone suppression test were all positive. Fifteen cases received glucocorticoid replacement therapy and 12 female cases received corrective surgery. It is suggested that 21 hydroxylase deficiency, at present, still remains to be an underdiagnosed and undertreated disorder, therefore, more attention should be paid to it for improving the accuracy of diagnosis and adequacy of treatment.
