2.Autogenous bone Graft of the tibial Bone Defect in Total Knee Replacement

In case of the large bone defect due to severe varus or valgus deformity in total knee replacement, there are many different methods for reconstruction of the bone defect for insertion of the tibial component after resection of the proximal tibia. Total knee replacement using the autogenous bone graft were performed in 27 knees (18 patients) with tibial bone defect. Authors report the methods and results with an average 23 months follow-up. We used the bone removed from distal femur and calculated the size of the proximal tibia. We used the screws, if necessary, for rigid fixation of the grafted bone. l. According to the type of bone defect, central defect type were 3 knees, peripheral defect type were 13 knees and combined type were 11 knees. 2. In A-P view of x-ray, the size of bone defect of the tibial plateau ranged 5mm to 25mm (average 11.1mm) in height. 3. Screws were used for fixation of grafted bone in 14 knees and the average size of the screws was 27.8mm. 4. Bony union of the grafted bone achieved from 3 months to 12 months (average 5.7months). 5. Average knee ROM was 105 degrees and improved post-operatively 121 degrees, and average flexion contracture was 22.4 degrees and improved post-operatively 7.5 degrees. 6. Post-operative tibiofemoral angle was average 7.5 degrees valgus. 7. Average knee rating score was 54.5 and improved post-operatively 89.2. 8. The comlication of bone graft was partial resorption in 1 case and the loosening or displacement due to screw were abscent.

3.Clinical Analysis for Prognostic Factors of Intertrochanteric Fractures

There are many problems in treatment of intertrochanteric fractures due to osteoporosis and unstable pattern of fractures in old ages and they are different from those in young ages, for example, type of fracture, degree of osteoporosis, and general condition, ect. Anthors studied about the prognostic factors of 42 cases among 88 patients with intertrochanteric fractures, who were treated and followed up from Jan. 1982 to Dec. 1987. 1. The patients under 50 years old were mainly males(94.4%) and its chief cause of injury was traffic accident or fall down(72.2%). The cause of injury over 50 years old was slip down on ground(62.9%). 2. The osteoporosis was increased according to increased age of patients and then the unstable type of fracture was increased. In yaung patients, the unstable intertrochanteric fracture was rare despite of high violent injury. 3. The bone union of the follow-up patients was gained at average 13.8 weeks. 4. The sliding degree of lag screw was increased with unstable fracture and the incidence of metal protrusion into joint cavity was increased in the patients of eccentric fixation of lag screw into femoral head. 5. The all cases of complication were occured in osteoporotic patients over 50 years old and their causes were inadequate fixation of unstable fracture. 6. The most important factors in prognosis of intertrochanteric fractures are initial pattern of fractures, degree of osteoporosis and adequacy of reduction state(postop.).

4.Clinical Experience of the Tibial Plateau Fracture

The intra-articular tibial plateau fracture can induce many complications of the knee joint after conservative treatment or operative treatment. Recent experience could decrease the complications with accurate anatomical reduction, stable internal fixation and early knee ROM exercise, but still unsatisfactory results were reported from 20% to 40%. The authors analyzed the 30 cases that could be follow-up from 1 year to 6 years and were treated with operative methods, and the following results were obtained. 1. The most common cause of injury was traffic accident (14 cases-46.4%). 2. The most common fracture type by Hohls classification was type VI (13 cases-43.1%). 3. According to Blokker's criteria, 26 cases (86.7%) among 30 cases had satisfactory results. 4. The one of the most important factor influencing end results was thught to be the anatomical reduction of the articular margin.

5.Manipulation under Anesthesia for Stiffness after Total Knee Arthroplasty

PURPOSE: This study evaluated the incidence of manipulation under anesthesia (MUA) for stiffness after total knee arthroplasty (TKA) and the degree of joint motion recovery after MUA. MATERIALS AND METHODS: A total of 4,449 TKAs (2,973 patients) were performed between March 2000 and August 2014. Cases that underwent MUA for stiffness after TKA were reviewed. TKAs were performed using the conventional procedure in 329 cases and using the minimally invasive procedure in 4,120 cases. The preoperative range of joint motion, timing of manipulation, diagnosis and the range of joint motion before and after MUA were retrospectively investigated. RESULTS: MUA was carried out in 22 cases (16 patients), resulting in the incidence of 0.5%. The incidence after the conventional procedure was 1.2% and 0.4% after the minimally invasive procedure. In the manipulated knees, the preoperative range of motion (ROM) was 102.5degrees+/-26.7degrees, and the preoperative diagnosis was osteoarthritis in 19 cases, rheumatoid arthritis in two, and infection sequela in one. MUA was performed 4.7+/-3.0 weeks after TKA. The average ROM was 64.5degrees+/-13.5degrees before manipulation. At an average of 64.3+/-41.3 months after manipulation, the ROM was recovered to 113.4degrees+/-31.2degrees, which was an additional 49.9degrees improvement in flexion. CONCLUSIONS: The satisfactory recovery of joint movement was achieved when MUA for stiffness was performed relatively early after TKA.

6.Vitamin C Improves The Age-Related Impairment of Endothelium-Dependent Vasodilation in Elderly.

BACKGROUND: Aging is associated with increased cardiovascular risk and firmly established as a risk factor for the development of atherosclerosis. However the exact mechanism of age-related damage to the arterial wall and its relation to the atherosclerotic process are not well known.The endothelium plays an important role for the regulation of vascular tone and the endothelial function is impaired in the presence of risk factors early in the process of atherosclerosis. Assessment of endothelial function appears to be a valuable tool for the diagnosing and therapeutic monitoring of coronary artery disease. Anti-oxidants are known to improve endothelial dysfunction in atherosclerosis patients. The aim of this study was, (1) to evaluate the endothelial function in elderly, (2) to investigate whether vitamin C administration has benefit on the endothelial function in elderly. METHODS: The endothelial function was estimated using venous occlusion plethysmography(VOP) in 7 elderly and 7 young healthy volunteers. The strain guage was connected to plethysmograph to record the forearm volume change. A rapid cuff inflator was used to inflate the arm cuff to 40 mmHg instantaneuosly thus occluding venous return from the forearm. The measurement of forearm volume change was repeated for 7 times each stage. The change of the forearm blood flow(FBF) was measured with the acetylcholine infusion through brachial artery and also with intra-arterial vitamin C. RESULTS: Endothelium-dependent vasodilatation was significantly impaired in the elderly group compared to the young group(321 +/-17% in elderly group vs 509 +/-81%, mean+/-SEM) Forearm blood flow response to acetylcholine was significantly enhanced with inraarterial infusion of vitamin C in elderly group(321+/-17% in elderly group vs 78% in vitamin C) Coinfusion of L-NMMA, an inhibitor of nitric oxide synthase, blunted forearm blood flow response to acetylcholine. CONCLUSIONS: Even though the mechanisms leading to drpressed endothelial function in elderly remains to be elucidated, our study shows that vitamin C result in demonstrable improvement by a mechanism that is probably related to antioxidant activity.
