To assess the effectiveness of Magnetic Resonance Imaging (MRI) with endorectal surface coil in the ataging of carcinoma of the uterine cervix with emphasis on parametrial involvement. Thirty women with clinically and radiographically proven carcinoma of the uterine cervix were initially included for this study, but thirteen patients were excluded since the stages of tumors wore beyond stage IIa. CT and MR findings of the remaining seventeen patients were performed at Cheil General Hospital and compared a1ong with clinical findings with the special emphasis on the parametrial involvement by the tumor. Staging was assessed by CT and MRI, and the results were compared with the pathologic staging. Radiea1 abdominal hysterectomy with the pelvic and paraaortic lymphnode dissection was done to all sewenteen patients. The staging made primarily by CT and MRI was either stsge 1 or Ila, but MR images with endorectal surface coil was superior to CT in the visualization of depth of tumor infiltration, especially parametrial involvement. The determination of the depth of the tumor made by MR images showed statistically significant correlation with histologic evaluation(R =0.768, p<0.01). The accuracy rate for the evaluation of the parametrial invo1vement was 82.3% far CT and 94.1% for MRI with endorectal surface coil. The overall accuracy rate for tumor staging was 70.5% for clinical, 58.8% for CT and 82.3% for MR evaluation. The accurecy rate in evaluation of the pelvic and paraaortic lymphnode was 88.2% for CT, but the evaluation done by MRl was not adequate due to small FOV(field of view). In assessment of The steging of careinoma of the uterine cervlx, MR images with endorectal surface coil was superior to CT, especially in the evaluation of the parametrial involvement.

The cilio-retinal artery appears on the temporal side of the optic disc margin and is concerned with the supply of the portion between the optic disc and the macula. So, the cilio-retinal artery preserves vision when occlusion of the central retinal artery occurs. The aberrant macular artery is very rare and in most of the cases have been derived from the inferior temporal arterial or venous trunks. When this artery is appeared, the corrected vision is normal but sometimes diminished. The authors have experienced two aberrant retinal vessels of a large cilio-retinal artery and aberrant macular artery combined with the Stargardt's maculopathies. It was reviewed clinically with the literature.

OBJECTIVE: It has been suggested that the occurrence of persistent pain signal during the early postnatal period may alter an individual's response to pain later in life. The aim of this study is to assess whether neonatal nerve injury resulted in long-lasting consequences on nociceptive system in the rat. METHODS: We examined whether neuropathic pain behaviors and the changes of spinal neuropeptides (SP, CGRP, VIP and VIP) induced by peripheral nerve injury within 1 day after birth (Neonate group) were different from those at 8 weeks after birth (Mature group). RESULTS: The Neonate group showed more robust and long-lasting pain behaviors than the Mature group. Immunohistochemical findings demonstrated that spinal SP- & CGRP-immunoreactivities(ir) of the ipsilateral to the contralateral side increased in the Neonate group, whereas those decreased in the Mature group. In addition, increase in spinal VIP- & NPY-ir of the ipsilateral to the contralateral side was more robust in the Mature group than in the Neonate group. CONCLUSION: These results suggest that peripheral nerve injury in the early postnatal period may result in long-lasting and potentially detrimental alterations in nociceptive pathways.

An endodermal sinus tumor is a malignant germ cell tumor that usually arises in the gonads, but on rare occasion occurs in extragonadal locations. Our case was that of a 3 year old girl who complained of a rapid growing orbital mass. On histologic examination it revealed the typical picture of an endodermal sinus tumor and it also disclosed a positive reaction for alphafetoprotein using an immunoperoxidase technique. An orbital exenteration was performed followed by chemotheraphy, but the patient died 5 months after the onset of the disease.

To evaluate the expressions of cyclooxygenase-2 (COX-2) and inducible nitric oxide synthase (iNOS) in thyroid neoplasms in a Korean population, we studied a total of 154 cases: papillary carcinoma of classical type (PTC), 86; follicular adenoma (FA), 21; follicular carcinoma (FC), 35; medullary carcinoma (MC), 3; undifferentiated carcinoma (UC), 5; and Hurthle cell neoplasm (HN), 4. Using immunohistochemical staining, COX-2 expression was detected in 62 (72.1%) PTC specimens, 5 (23.8%) FA specimens, 10 (28.6%) FC specimens, 0 (0.0%) MC specimens, 1 (20.0%) UC specimen, and 3 (75%) HN specimens. iNOS expression was detected in 66 (76.7%) PTC specimens, 4 (19.0%) FA specimens, 13 (37.1%) FC specimens, 0 (0.0%) MC specimens, 3 (60.0%) UC specimens, and 4 (100%) HN specimens. The results showed that COX-2 and iNOS were frequently expressed in the PTC and HN specimens, and iNOS was more frequently overexpressed in the FC specimens than in the FA specimens. In PTC, COX-2 and iNOS were significantly overexpressed in patients over 45 yr of age (p=0.029, p=0.041), and iNOS expression was increased in patients with a large primary tumor (p=0.028). These results suggest that the upregulation of COX-2 and iNOS may contribute to the tumor progression of thyroid gland, particularly in PTC and HN, and iNOS may play an adjuvant role during the tumor progression of FC.

Serum levels of T3, T4, TSH, 24-h RAIU and Thyroid antibodies were examined in 13 cases of chronic lymphocytic thyroiditis who were diagnosed at the pediatric department of Yonsei University hospital during the period from 1978 to 1980. The results were as follows: 1. The age distribution was dispersed between 8 and 15 years of age and all the cases were female. 2. Common symptoms were Goiter, Poor Weight gain, fatigness, cold intolerance, lethargy, constipation and cold sweating. But in 5 cases(38%), there were no subjective symptoms. 3. The patients were classifed by their thyroid function state, as 1 case of hyperthroid group, 10 cases of euthyroid group, and 2 cases of hypothyroid group. 4. In the Euthyroid patients, Serum T3 concentration was 115-230ng/100ml)mean 167.4ng/100ml), T4 concentration 4.8-18.6microgram/100ml(mean 7.3 microgram/100ml) T3/T4 ratio 0.020-0.028(mean 0.024) and TSH was 37-60microU/ml(mean 49.5microU/ml). In the hyperthyroid patient, serum T3 concentration was 310ng/100ml. T4 concentration 14.9microgram/100ml, T3/T4 ratio 0.020 and TSH was 1.35microU/ml. 5. In 24-h RAIU, Euthyroid patients were in 2 cases of decreased state(15%), 2 cases of normal range (15~45%), and 6 cases of increased state(45%). All Hypothyroid patients were in normal range and hyperthyroid patient was in increased state. 6. Thyroglobulin antibody was positive in all the patients except 2 cases and microsomal antibody was positive in all the cases. 7.1 year of follow up study was performed in 4 cases of euthyroid patients, 1 case of hyperthroid patient, and 2 cases of hypothroid patients. There was no euthyroid patient who became hypothyroid state in the follow up study.
