1.Diagnosis of Ventricular Septal Defect by Pulsed Doppler Echocardiography.

This study was performed to assess the accuracy of pulsed Doppler echocardiographic(PDE) diagnosis of ventricular septal defect(VSD). 119 children with congenital heart disease, aged 9 months to 17 years, admitted to our hospital for cardiac surgery, were studied. These children had VSD(56), ASD(18), PDA(43), PS(1) and Cortriatrium(1). A specific PDE diagnosis of VSD can be made by following the turbulent VSD jet(TVJ) through the septum, and a PDE diagnosis of VSD was made in 53 out of 56 patients(95%) proven by catheterization and operation. There were two false positive PDE diagnosis of VSD by PDE, were proven to be perimembranous VSD at operation, and 3 children who were classified into subarterial infundibular VSD by PDE, were proven to be perimembranous VSD at operation. PDE seems to be a very useful tool in preoperative evaluation of VSD.

2.The Effect of 6 Month Alphacalcidol Treatment or Nutrition Education on the Nutrient Intakes, Bone Mineral Density and Bone Markers in Continuous Ambulatory Peritoneal Dialysis Patients.

This study was performed to estimate the effect of alphacalcidol supplementation or nutrition education on the nutrient intakes, bone mineral density and bone markers in continuous ambulatory peritoneal dialysis (CAPD) patients. The 90 CAPD subjects were randomly assigned to 3 groups (alphacalcidol group: AG, nutrition education group: NG, and control group: CG). Alphacalcidol supplementation(0.5 microgram/day) was carried out for 8 months. Nutrition counseling was performed according to the patient's individual question for the first 6 months and scheduled nutrition education with individual counseling was carried out for the last 2 months. In baseline data, there were no significant differences in age, sex, family number, education years and monthly income except the NG showed significantly less duration of CAPD (p <0.05) compared to other two groups. After intervention all three groups showed tendency of lower intakes. NG revealed less decrease in protein, especially in animal protein, calcium from Ca-P binder, dietary calcium, dietary iron and niacin. NG showed significantly more increase in dry weight (p <0.05) and AG in waist circumference (p <0.001) after intervention. The groups did not show significant differences in the changes of biochemical indices related to bone metabolism. NG revealed more increase in trochanter BMD (p <0.05) compared to other two groups. It seems that nutrition education is more effective in preventing deterioration or improving the bone and general nutrition status.

3.A STUDY ON THE STAIN RESISTANCE OF DENTAL CERAMICS FOR ALL CERAMIC CROWN.

Simulation of naturally appearing enamel in fixed prosthodontics could be achieved with all ceramics. The purpose of this study was to investigate the stain resistance of 4 different all ceramics materials (In-ceram, IPS Empress, Celay, Vintage). Forty specimens were prepared using manifacture's instruction and the samples were divided into two groups. The specimens for one group were glazing and those for other group wee ground to remove the glazing layer. All specimens were immersed in methylene-blue for 24hours. The color values for each specimen was measured with colorimeter (Mode Tc-6Fx, Tokyo Denshoku Co.) prior to and after immersion in methylene-blue. The following conclusions were drawn from this study. 1. The stain resistance of glazing ceramics was higher than that of unglazing ceramics. 2. The changes of E* and L* values were revealed in Vintage and unglazing Celay. 3. The changes of a* values were revealed in unglazing vintage, Celay, In-cream. 4. The changes of b* value were revealed in Vintage, IPS Empress and unglazing Celay.

4.CT Findings of Bronchioloalveolar Carcinoma.

PURPOSE: We studied CT findings of bronchioloalveolar carcinoma that manifestated varied clinical and radiologic characteristics. MATERIALS & METHODS: We studied presenting radiographic patterns and their characteristics and secondary findings of histologically proven 30 cases of bronchioloalveolar carcinomas on chest CT scan. RESULTS: Bronchioloalveolar carcinoma appeared radiologically as a solitary nodule(19 cases), consolidation (7 cases), and multinodules(4 cases). A solitary nodular type shows irregular or spiculated borders, peripheral or subpleural location, heterogenous density, pleural tags, pseudocavitation, and sometimes rim-enhancement of mass. A consolidation type shows air-brochogram, pseudocavitation and CT anglogram within homogenous low attenuated consolidated lung. Hilar and mediastinal lymphadenopathy as secondary findings of bronchioloalveolar carcioma appeared frequently(43.3%). Even in a solitary nodular form, hilar & mediastinal lymphadenopathy was noted in 36.8% and follow-up study in 6 cases showed lung-to-lung metastasis with 14.7 months in mean metastasis duration. CONCLUSION: It is difficulty in diagnosis of bronchioloalveolar carcioma with clinical or plain radiographic features alone because of theis variability. We found that CT scan can help the diagnosis of this tumor. We also found out that a solitary form of bronchioloalveolar carcioma as well as diffuse form does not have a good prognosis.

5.Delayed Post-traumatic Collapse of the Vertebral Body (K mmell disease): Report of a Case with Serial Radiographs.

K mmell disease, the delayed post-traumatic collapse of the vertebral body, and its numerous postulated causes are discussed. A case is presented in which serial radiographs and magnetic resonance images demonstrate the presence of fracture and osteonecrosis.

6.A Case of Hereditary Trichilemmal Cyst with Proliferating Trichilemmal Tumor.

We report a case of hereditary trichilemmal cyst with proliferating trichilemmal tumor in a 64-year-oid man, who has generalized 157, variable sized firm cutaneous nodules for 40 years. The cystic nodules are chiefly located on the scalp and trunk which showed characteristic histopathologieal findings of trichilemmal cyst and proliferating trichilemmal tumors. His son also has multiple trichilemmal cysts and his mother seems to have had clinically similar lesions. The patient and his son were treated with surgical excision and therapeutic trial of oral isotretinoin for 2 months. Isotretionoin was ineffective in the patient, but the lesions of his son were decreased in size.

7.Expression of p53 and Rb Proteins in Invasive Ductal Carcinoma of the Breast.

Inactivation of tumor suppressor genes may play an important role in many human cancers including breast. This study was done to determine the relationship between the expression of p53 and Rb protein and prognostic factors such as histopathologic differentiation, tumor size, and lymph node metastasis. In 57 cases of breast invasive ductal carcinomas, the immunohistochemical staining with p53 and Rb protein gave the following results: p53 protein was detected in 45.6% (26/57) of cases. Tumors with large size, poor differentiation or lymph node metastases tended to show increased expression of p53 protein. However, p53 protein expression did not show any significant correlation with prognostic factors such as tumor size (p value 0.25), histologic grade (p value 0.75), and positive lymph node status (p value 0.26). Rb protein was detected in 57.9% (33/57) of cases. Rb protein also did not show any significant correlation with prognostic factors such as tumor size (p value 0.56), histologic grade (p value 0.71), and positive lymph node status (p value 0.98). There was no significant correlation between p53 expression and Rb protein expression (p value 0.80).

9.A Study on Serum IgE Levels in Various Dermatoses.

The serum value of immunoglobulin E (IgE) in 47 cases of atopic dermatitis and 43 cases of urticaria were determined by Phadebas PRIST(R) method in department of dermatology at National Medical Center, from Jan., 1982 to Sep., 1982. The results were as follow; 1. The mea,n IgE value in atopic dermatitis was significantly higher (408+/-l43 u/ml) than that of normal subjects (p>0.01). 2. The mean value of IgE in urticaria (310+/-299 u/ml) was not significantly higher than tha,t of normal subjects (p>0.05). 3. The rnean value of IgE in severely expanded atopic dermatitis (664+/-404 u/ml) was significantly increased than that of normal subjects (p>0.01) 4. The mean value of IgE in 7 cases of atopic dermatitis with allergic respiratory diseases (817+/-233 u/ml) was significantly elevated (p>0.01). 5. No close correlation was seen between IgE value and eosinophilia. 6. IgE in 2 cases of the urticaria infested with PW, CS were extended over 1000u/ml.

10.Clinical and Histopathological Observation on Pityriasis Rosea.

The present study was performed to evaluate clinical manifestations and histopatological findings of 95 patients with pityriasis roses who visited the Department of Dermatology in National Medical Center from October of 1979 to September of 1982. The results are as follows, 1. Clinical Manifestations: 1. The number of patients with pityriasis roses were 95, which occupied 0. 98/o of new out-patients in the department of dermatology in the same period. 2. In sex distribution, males were 36 and females were 59, with male to female ratio of I: l. 6. R. Age of onset varied from 13 months to 58 years with a mean of 26. 0 years and 68% of cases belonged to 15 years to 84 years. 4. Seasonal incidence was 32% in winter, 27% in summer, 22% in autumn and l9% in spring. 5. iMild prodromal symptoms developed in 8 cases(%) and itching was noted in 58 cases (61%), which was mild to moderate exeept for severe itching in 2 cases. 6. Herald patch was found in 42 cases (44%) and the predilection sites were the trunk and the thighs. 7. Daughter lesions could be classified into macular type 69 cases (73g), pa.pular type 22 cases (23%), pustular type 2 cases (2%), urticarial type 1 case (I,3 and purpuric type 1 case (1%) They appeared on the back (79%), the chest (74%), the abdomen(73%), the thighs (47%), the upper arms (40%), the lower legs (24%),the forearrns (22%), the neck (16%), the face (7 %), dorsa of the hands (4%) and dorsa of the feet(2%).
