1.The clinical study on the relationship between silent cerebral ischemia and cognitive impairment in inpa-tiens older than sixty with nonvalvula atrial fibrillation

Objectives The aim of this study was to study the relationship between silent cerebral ischemia (SCI) and mild cognitive impairment(MCI)in inpatients with nonvalvula atrial fibrillation (NAF) and those with sinus rhythm (SR) older than sixty. Methods Ninety-eight inpatients were enrolled from November 2014 to November 2015 in Hui?zhou Municipal Central Hospital: 45 patients with NAF and 53 with SR. All general clinical data was collected. The Montreal Cognitive Assessment (MoCA) and brain MRI were used to assess the cognitive performance of patients within 1 weeks of admission. Results The MoCA scores were lower in patients with NAF than in those with SR[16(10,20) vs. 20 (15,23),P=0.006]. At least one foci of SCI was present in patients 80%with NAF and 52.8%with SR (NAF vs. SR, P=0.018).The rates of SCIs in the cortex/subcortex were higher in the NAF group than in the SR group (25.2%vs. 12.4%, P=0.017). Multivariate analysis showed that MoCA score were related to the degree of education in NAF patients and were related to age and education level in SR patients. Conclusions NAF is an independent risk factor for cognitive impair?ment in NAF patient and associated with the SCI present. Educational level is a factor affecting the cognitive function with elderly patients.
2.Effect of clonidine on the proinflammtory cytokine and haemodynamics in hypoxia and trauma rats

OBJECTIVE To observe the effect of clonidine on the proinflammtory cytokine and haemodynamics in hypoxia and trauma rats. METHODS Forty male SD rats were randomly divided into 5 groups: (A) hypoxia control group (n=8), (B) trauma control group (n=8), (C) hypoxia and trauma group(n=8), (D) hypoxia trauma clonidine precondition group (n=8), (E) hypoxia trauma colindine postcondition group (n=8). Except B group, the rats of each hypoxia group were placed in a sealed hypoxia chamber which O2 concentrate were maintained at (10 ?0.5)% for 6 days, 7h per day. Except A group, the rats of each trauma group received pharyngeal trauma after anesthesia. The rats in D and E group received clonidine 30 ?g/kg before 10 minutes or right after the pharyngeal trauma respectively. The mean artery pressure (MAP) and heart rate (HR) on the several moment around the trauma were recorded. The serum level of TNF-?, IL-6 was measured 1h after trauma. 4.General paralysis of insane: two case reports

Objective To explore the clinical manifestation,laboratory tests,imaging and treatment of general paralysis of insane(GPI).Methods The clinical data of 2 patients with GPI analyzed retrospectively.Results 2 sufferers were both male,and mainly presented with memory decline and mental disorder.One case presented with visceral crisis.Both rapid plasma reagin test and treponema gelatin agglutination test in serum and CSF were positive.Brain MRI scanning showed ventriculomegaly.After antisyphilitic treatments,2 patients obviously improved.Conclusion GPI are often seen in middle-aged males and its misdiagnosis rate is high.The diagnosis of GPI should be considered in young patients with dementia without clear reason.Early diagnosis and active treatments are important for the prognosis of this disease.

6.Meta-analysis of the level of adiponectin in chronic obstructive pulmonary disease

Objective To investigate the clinical significance of the level of adiponectin (APN) in patients with chronic obstructive pulmonary disease(COPD).Methods The case-control study involving APN in OSAHS were searched and identified from Cochrane Library,PubMed,Embase,China Academic Journals Full-text database,VIP Database,and Full text database of China' s.According to inclusion and exclusion criteria,evaluated the quality of choice experiment,and then extracted the valid data for meta-analysis.Results Seven articles were selected,including 2 in SCI,5 in Chinese.Meta analysis showed that APN in acute exacerbation COPD group was significantly higher than in COPD group (standardized mean difference (SMD) =2.32,95%CI=0.62-4.02,P<0.05).Compared with normal control group,APN in COPD was higher (SMD=1.39,95%CI=1.00-1.78,P<0.01).Determined publication bias by Begg's and Egger's test,the results did not show any bias (P> 0.05).Conclusion The serum APN of COPD patients are significantly elevated,the APN testing in some extent reflect the severity of COPD.

8.Development of Reciprocating Sports Intermittent Hypoxia Chamber

Objective To research and develop the model of external reciprocating sports intermittent hypoxia box for rats. Methods Two animal cabins (28 cm?20 cm?15 cm) and one hypoxia chamber (80 cm?80 cm?60 cm) which covered each cabin in turn were developed. 4 male SD rats were placed in each animal cabin. The hypoxia chamber in which low O2 concentration was maintained with filling high-pure Nitrogen continuously, moved to and fro between two animal cabins, and driven by a servomotors. Two animal cabins were covered in turn by the hypoxia chamber and get the same low O2 concentration as in the hypoxia chamber. The goal volume fraction of oxygen in hypoxia chamber was (10?0.5)% and the time of unilateral movement from one animal cabin to another was 4 s, the cycle time of intermittent hypoxia was 180 s, the retention time was 86 s (including the hypoxia and normoxia time was 90 s respectively). The hypoxia chamber runs 8 h per day. The numerical value on artery blood gas of rats in which hypoxia chamber inside and outside was measured Results The reciprocating sports intermittent hypoxia chamber can be operated safely and reliably. The lowest PaO2 is (4.766? 0.536) kPa(or (35.75 ?4.02) mmHg) and the lowest SaO2 is 0.69 ?0.08 when the animal cabin in hypoxia chamber. Conclusion The chronic intermittent hypoxia model for rats is established by self-developed external reciprocating sports intermittent hypoxia chamber.
9.Effects of additive Foshousan on the serum levels of estradiol and progesterone in rats with primary dysmenorrhea

Objective To investigate the effects of Additive Foshousan(AFSS) on the serum levels of estradiol and progesterone in rats with primary dysmenorrhea. Methods SD rats were divided into six groups, each 10. Group 1 was normal control , groups 2,3 and 4 were treated with low, middle and high dose of AFSS(0.75, 1.5 and 3.0 g/kg) respectively, group 5 as positive controls was treated with Yuanhuzhitong tablets (1 mg/kg)and group 6 served as model control. Except the normal control group, all rats were injected diethylstilbestrol and oxytocin to establish primary dysmenorrhea model. The levels of estradiol(E2)and progesterone(P) in rats serum was determined by ELISA method and the ratio of E2/P was calculated. Results In middle and high dose of AFSS groups, the level of E2(48.27±6.42)pg/L,(47.51±7.03)pg/L respectively were lower than that in model group(54.47±9.12)pg/L and the difference was statistically significant(P<0.05). In low dose of AFSS group, the level of E2 was(50.83±6.26)pg/L and the difference was no statistically significant compared with model group. In all doses of AFSS groups,The content of P(687.41±21.14)ng/L, (720.47±41.03)ng/L, (719.78±32.01)ng/L respectively were higher than that in the model control group (667.32±46.51)ng/L and the difference was statistically significant(P<0.05 or<0.01). In middle and high dose of AFSS groups, the content of P were higher than that of Yuanhu-Zhitong tablets group(699.31±36.31)and the difference was statistically significant(P<0.05). Conclusion To reduce the content of E2, increase of P the content and decrease ratio of E2/P is one of the mechanism for AFSS to treat primary dysmenorrhea.
10.Factors Associated with Fungal Infection Following Pancreatoduodenectomy

Objective To investigate the relevant factors for fungal infection following pancreatoduodenectomy and offer the theoretical foundation for preventing the emergence of complications after operation. Methods Medical records from 562 consecutive patients who underwent pancreatoduodenectomy in this hospital from 1995 to 2005 were retrospectively reviewed by using single factor and non-condition Logistic regression analyse. Results ①Seventy-eight patients (13.9%) developed invasive fungal infection. The most frequently isolated fungal were Candida albicans accounted for 67.0%, and followed by Candida glabrata, Candida papasilosis and Candida tropicalis and gastrointestinal tract was the most common infection site, followed by respiratory tract, abdominal cavity. ②Fungal infection occurred significantly more often in patients with the length of time in parenteral nutrition, antibiotic use or abdominal cavity complications. Conclusion The most common infection site and isolated fungal associated with pancreatoduodenectomy were gastrointestinal tract and Candida albicans. Abdominal cavity complications such as pancreatic fistula, biliary fistula and abdominal infection and extended use parenteral nutrition and antibiotic are the most important factors leading to invasive fungal infection after pancreatoduodenctomy. Eliminating the various risk factors will decrease the incidence of fungal infection.
