1.Microendoscopy in posterior lumbar intervertebral stenosis

Objective To evaluate the clinical outcome of lumbar spinal canal stenosis treated by microen-descopic discectomy. Methods The rear guard intervertebral discoscope was used to carry on unilateral or-bilateral "windowing" to relieve the dura mater spinalis and the nerve root oppression thoroughly. The "C" ann machine or laferal side photography position X was used for localization. Approximately 1.5 cm incision was made under local anesthesia or under the shallow epidural anaesthesia at posterior waist, the pathway tube was implanted after progressive expansion, part of lamina of vertebra was removed by drill, endoscope was inserted under the television surveillance to reveal removed lamina of verfebra, the proliferated cohesed articular process, the plump yellow ligament flava and the bulging intervertebral disc nucleus pulposus tis-sues, the nerve root canal was depressed, to relieve thoroughly its oppression, dura mater and nerve root. Results The follow-up continued from 5 months to 36 months. The evaluation with Nakai scale revealed ex-cellent in 168 cases, good in 9 cases, fair in 6 cases. The rate of good results was 96.7%. Conclusions Totally 183 cases were treated with this procedure posterior microendoscropie discectomy shows minimal inva-sion, less blood loss and quick recovery. The stability of lumbar spine can be reserved. It is safe and effec-tive for treating lumbar disc herniation complicated with spinal stenesis.
2.RNAi silencing MTA1 gene inhibits invasion and migration of esophageal carcinoma 9706 cells

Objective To observe the effect of RNAi that silences MTA1 gene on invasion and migration of esophageal carcinoma 9706 cells. Methods The siRNA expression vector that silences MTA1 gene was transfected into EC9706 cells by liposome. MTA1 mRNA and protein expressions were detected through quantitative RT-PCR and Western blot, respectively. The invasion and migration of EC9706 cells were evaluated by scrape wound healing assay and cell invasion assay in vitro. Results MTA1 gene expression significantly decreased. Abstract Gram-negative ( G-) bacteria, such as denitrifying bacteria and anaerobic ammonia oxidation bacteria, are highly social organisms capable of sophisticated cooperative behavior mediated via quorum sensing. As signal molecules of the chemical communication, N-acyl-homoserine lactones ( AHLs ) can mediate the quorum sensing of the functional microbial population and regulate the population density. To understand the growth of functional microbial population and the mechanism for biological nitrogen removal in upflow microaerobic sludge reactors ( UMSRs ) treating organic wastewater with low ratio of chemical oxygen demand to total nitrogen, a method was established to simultaneously detect AHLs in the microaerobic processes. Water-sludge mixtures sampled from the UMSRs were pretreated in sequence by liquid-liquid extraction using ethyl acetate, rotary evaporation, constant volume with methanol, separation by C18 column. Gradient elution was carried out using 5 mmol/L ammonium acetate ( containing 0 . 1% formic acid ) and methanol as mobile phases. On the base of multiple reaction monitoring analysis, a triple quadrupole mass spectrometer with an electrospray ionization was introduced to detect the target compounds. Nine kinds of AHLs were used to evaluate the established method and the results showed that the detection limits were 0 . 01-0 . 5 μg/L and all of the AHLs presented excellent linearity with the concentration ranging from 0 . 5 to 100 μg/L. The recovery and relative standard deviation ranged from 62. 5% to 118. 1% and 2. 9% to 12. 1%, respectively. The analysis could be finished within 6. 5 min. The rapid, accurate and precise method for detecting AHLs provided a new insight into the growth and metabolic activity of functional microbial population in the activated sludge processes to understand the mechanism of biological nitrogen removal, suggesting a good application in regulation and operation of wastewater biological treatment processes.
4.Expression of thymidine phosphorylase in human metastatic liver cancer tissues from colorectal cancer

Objective To detect the tbymidine pbospborylnse (TP) expression in metastatic liver cancer tissues from human colorectal cancer by immunohistochemistry, and analyze the correlation between TP ex-pression and the tumor-associated macrophages (TAM), and the prognosis of patients. Methods Twenty-eight metastatic liver cancer specimens resected from patients with colorectsl cancer, were immunohistochem-ically stained by 654-1, an anti-TP monoclonal antibody, IC6-203, another anti-TP monoclonal antibody, PG-M1, anti-macrophage marker CD68 monoclonal antibody. Morphometrical analysis and positive cell counting were performed, and the correlation of TP expression with the patient's prognosis was evaluated. Results In normal liver tissues, the hepatic cells apart from cancer nests were weakly positive for 654-1 as well as for 1C6-203. The most TP-positive cells were distributed mainly along the invasive margin of cancer or around the cancer nests. In the corresponding areas, CD68-positive macrophages were also increased. The distribution patterns of CD68-positive cells were similar to those of TP-pesitive cells. The numbers of the TP-positive cells stained by 654-1 were significantly correlated with numbers of 1C6-203 positive cells (r=0.697, P<0.01), also correlated with the numbors of CD68-positive cells (r=0.703, P<0.01). While the numbers of 1C6-203 positive cells had no significant differences with the numbers of CD68-positive cells (r=0.359, P>0.05). The TP-pesitive cancer cells both for 654-1 and for 1C6-203 were detected only in 2 of 28 specimens. Both the number of TP-pesitive cells for 654-1 and 1C6-203, and the number of CD68-positive cells had no correlation with the survival period of patients. Conclusions In the metastatic liver cancer tissues of human colorectsl cancer, the TP-expreasinn stained by 654-1 was coincidence with 1C6-203, and the most important source of TP-expreasion is the TAM in stromal tissues around cancer nests, while the cancer cells are little expressed. The numbers of TP-positive cells stained by 654-1 are significantly related with CD68-pesitive macrophages, but not with the post-operation survival period of patients.
5.Perioperative efficacy on radical resection of rectal carcinoma: a comparafive analysis of laparoscopy with open approach

Objective To compare the efficacy of the radical resection by laparoscopy versus open approach in perioperative period on the patients with rectal carcinoma,and investigate the feasibility,safety and oncological clearance of the laparoscopy.Methods The clinical data of 44 patients who underwent radical resection of rectal carcinoma by laparoscopy in our hospital were reviewed and compared with another 53patients who underwent an open approach in the same period.The surgery-related data,postoperative recovery status,tumor radical resection index,and postoperative complications by laparoscopy were analyzed by statistics,and compared with those by open approach,and evaluated the deference of too kinds of operation.Results This study showed a longer surgical time (260.45 ± 67.46) min vs ( 179.25 ± 40.92) min,P <0.05,a less intra-operative blood loss( 125.20 ±61.80) mL vs ( 198.02 ± 131.24) mL,P <0.05,in laparoscopic group compared with open approach.Meanwhile,it also showed an earlier recovery of bowel functions for discharge gas from anus,taking in food,and out-of-bed activity (4.34 ± 1.55) d vs(5.45 ± 1.55) d,P <0.05,in the laparoscopic group compared with open approach.There was no statistical difference of incidence of post-operative complications (5 cases vs 11 cases,P >0.05) between the two groups and the laparoscopic approach was also equal to the open approach as regard to post-operative stay (15.34 ±6.62) d vs (16.82±5.73) d,P >0.05,and demand of intra-operative blood transfusion (4 case vs 8 cases,P>0.05 ).Conclusions Compared with open surgery,the radical resection of rectal carcinoma by laparoscopy has shown obvious advantages in smaller incision,less blood loss,less pain,earlier recovery of bowel and bladder functions,and earlier out-of-bed activity.And it is also possible by laparoscopy approach to decrease the post-operative complications and post-operative stay.Meanwhile,there is no significant deference on oncological clearance for laparoscopy compared with open approach during perioperative period,while the long term follow-up data is still needed to support the results.
6.Improvement of Reusable Textiles Management in Infectious Disease Hospital

OBJECTIVE To improve capability of nosocomial infection(NI) prevention and control in reusable textiles management.METHODS In an investigation to previous reused textiles handled procedure,care to keep from creation of potential infectious aerosols should not be taken in contaminated textiles collection,transportation of clean and soiled textile was done in undistinguished uncovered vehicles,sorting room shared a common space with dressing room and washing room,and personal protection was not performed in handling contaminated textiles.RESULTS A series of NI management as follows effectively were adopted in hospital laundry.Appropriate bags were adopted for collecting used textiles or fabrics at the point of generation and subsequently closed securely for transportation of them to the laundry.An independent sorting area with exclusive ventilation system was separated from other parts of the laundry.Hand hygiene facilities and products and appropriate personal protective equipments were available.Transportation and storage of clean from soiled textiles were separated.CONCLUSIONS Appropriate handling and processing textiles in infectious disease hospital plays key role in NI management.

Objective To study the effect of gene engineering adenovirus H101 for treating esophagus carcinoma EC9706 induced by CEA gene specific silencing and to explore internal influential factor of H101 sensitivity. Methods To construct the siRNA expression vector of CEA and to inhibit the expression of CEA through RNA interference and gene transfection in EC9706 cell,stable CEA gene silencing system was set up,compared with empty vector group and non-transfected EC9706 cell,the model of athymic mouse subcutaneous transplantation tumor of human esophagus carcinoma EC9706 cell was established followed by injection with H101. The mRNA and protein expressions of CEA were detected by real time PCR and immunohistochemistry,the tumor size was measured. Results Silencing CEA gene by applying RNAi can inhibit CEA mRNA and protein expression in nude mice model with transplanted human esophageal cancer cells,there was no evident influence on tumor growth and mass oftumor. 8.Value of MR placental protusion sign in predicting postpartum hemorrhage in patients with placenta previa

Objective To explore the MRI findings of placental protusion sign in predicting postpartum hemorrhage in patients with placenta previa.Methods Totally 354 placenta previa patients with whole clinical data underwent MR scaning 2 weeks before operation.Association of postpartum hemorrhage and placental protusion sign was analyzed.Results Among 354 patients with placental previa,the age of the pregnant women (x2 =4.34,P=0.04),gestational age at delivery (x2 =5.19,P=0.02) and the number of cesarean sections (x2 =44.85,P<0.01) had associated with postpartum hemorrhage.Eight cases had placental protusion sign in MRI,while 6 cases occurred postpartum hemorrhage.The incidence of postpartum hemorrhage was 75.00% (6/8) and 12.72% (44/346) in patients with placenta accreta and with placental abruption,respectively (x2 =20.14,P<0.01).The sensitivity,specificity,odds ratio (95% confidence interval) and positive likelihood ratio of predicting postpartum hemorrhage was 12.00% (6/50),99.34% (302/304),20.59 (4.03,105.23) and 15.68,respectively.Conclusion MRI placental protrusion sign has important clinical reference value in predicting postpartum hemorrhage.
