1.An analysis of the electrophysiological characteristics of entrapment neuropathy

Objective To investigate the electrophysiological characteristics of carpal tunnel syndrome (CTS) and CTS-associated cervical spondylotic radiculopathy (CSR),and to examine the relationship between CTS and CSR. Methods The clinical characteristics and electrodiagnostic features of 81 patients with CTS and 20 patients with both CTS and CSR (the double crush,DC) were analyzed and compared.The data were analyzed according to the severity of the deficit in median nerve conduction using electromyography. Results The 81 patients with CTS had 123 median nerves with abnormal conduction (39 cases with unilateral abnormalities and 42 cases with bilateral abnormalities).The 20 patients with DC had 31 median nerves with abnormal conduction (9 cases with unilateral abnormalities and 11 cases with bilateral abnormalities).The rate of abnormal sensory nerve conduction velocity (SCV) was 100% in C6 and C7 level radiculopathies.The rate of abnormal distal motor latency (DML) was 92.31% in C5 level radiculopathies.There was a statistically significant difference between CTS and DC in the rate of abnormal SCV from the middle finger to the wrist.The other electrodiagnostic data were not significantly different between the CTS and DC patients.Neurophysiological tests were used to grade CTS into categories according to the American Association of Electrodiagnostic Medicine's criteria,but there was no statistically significant difference between CTS and DC. Conclusions CSR lesions on a proximal nerve root may cause the nerve to be more susceptible to distal injury and increase the risk of CTS.The findings support the DC hypothesis,but DC on a median nerve did not result in more severe injury than a single crush.
2.Normal CT manifestations of hypopharynx at the level of cricoid cartilage in adults

Objective To approach the normal manifestations of the hypopharynx of the adults at the level of cricoid cartilage as seen on CT images, and to evaluate their clinical value. Methods Eighty-four CT images of the normal hypopharynx were reviewed. The hypopharynx was divided into three regions:(1) the piriform sinus, (2) the retropharynx area, and (3) the postcricoid region. The postcricoid region of hypopharynx was subdivided into three levels as follows : (1) the upper margin slice of the cricoid cartilage at the cricoarytenoid joint level, (2) the middle portion slice of the cricoid cartilage, and (3)the inferior margin slice of the cricoid cartilage. The anteroposterior and transverse diameters, and anterior and posterior wall thicknesses in the postcricoid region were measured. Depiction of the layers of the musculature and adjacent fat planes was evaluated. Statistical comparisons of measured results were made by using ttest and x2 test. Results The posterior wall tended to be (0. 9±0. 4) mm thicker than the anterior wall. The average transverse extension of the postcricoid musculature was (4. 5±0. 3)mm shorter in female than that in male at CT images. There were statistically significant differences related to sex (at the upper margin level of the cricoid cartilage : the transverse extension was (38. 6±3. 3)mm in male, (34. d±2. 5) mm in female, t = 6. 26,P < 0. 05 ; at the middle portion level of the crieoid cartilage: (33.6±3. 6) mm in male,(28.9±2.8) mm in female t =6.36, P <0.01;at the inferior margin level of the cricoid cartilage:(28.6 ±3. 1) nun in male, (24. 0 ±2. 1) mm in female, t = 7.52, P <0. 01). The transverse diameter tended to taper (10. 1±2. 4) mm from the upper cricoid slice level to the lower cricoid slice levelDemonstration of the intramural fat planes of the postcricoid region decreased from the upper [ 81.0%(68/84)] to the lower region [23.8% (20/84)] of the cricoid cartilage. In fat planes around the postcricoid region at all levels, the posterior fat plane was seen least frequently, and the left-sided fat plane was seen most frequently. There was a statistically significant difference at every level(at the upper margin level of the cricoid cartilage, the visibilities of the left-sided, the right-sided, and the posterior fat plane around the postcricoid region were 77.4% (65/84), 72. 6% (61/84), and 28. 6% (24/84) ,x<'2> =24. 64,P <0.01 ; at the middle portion level of the cricoid cartilage, the visibilities were 89.3% (75/84), 75.0%(63/84), and 34.5% (29/84) ,x<'2> =24. 76, P <0. 01 ; at the inferior margin level of the cricoid cartilage:the visibilities were 95.2% (80/84), 88. 1% (74/84), and 52. 4% (44/84), x<'2> = 13.59, P < 0. 01.Conclusion Knowledge of the normal appearances and variations of the hypopharynx at the level of cricoid cartilage is essential in detecting abnormalities in this area.
3.Effect of different dentin cleaning agents on the bond strength of self-adhesive resin cement to dentin.

OBJECTIVEThis study aims to evaluate the bond strength of a self-adhesive resin cement to dentin by ethylene diamine tetraacetic acid (EDTA) and NaClO.
METHODSTwenty-seven freshly extracted non-carious human premolars were prepared to expose the buccal dentin and randomly divided into three groups: control group (A group), EDTA group (B group) and NaClO group (C group). All teeth were bonded to dentin using a self-adhesive resin cement after the teeth in the A group were processed with distilled water. The B and C group were processed with 3%EDTA and 1%NaClO, respectively. After 24 hours at 37 °C water, the shear bond strengths of the twenty-four specimens were measured. All statistical analysis was performed using SPSS 17.0 software package. Each fractured specimen was examined under dental microscope. Three new specimens were cut, and the morphologies of the cement-dentin interface were observed under scanning electron microscope (SEM).
RESULTSThe shear bond strength in the A group, B group and C group was (8.55±0.63), (8.47±0.56) and (12.97± 0.59) MPa, respectively. The difference between A group and B group was no statistically significant (P>0.05), whereas the difference between C group and B group (or A group) was statistically significant (P<0.05). SEM observation of the cement-dentin interface in the C group showed good adaptation, but resin tags were not observed. The other two groups showed poor bonding interface. Most of the fractured adhesive dentin surfaces exhibited cohesive failure in the A group and B group. All the fractured adhesive dentin surfaces exhibited cohesive failure in the C group.
CONCLUSION1% NaClO can increase the bond strength of self-adhesive resin cement to dentin, but 3%EDTA has no effect.

5.Yushi Huayu Zhixue prescription combined with levonorgestrel-releasing intrauterine system in treating adenomyosis:a prospective,randomized,double-blind,placebo-controlled trial

Objective To investigate the efficacy and safety of Yushi Huayu Zhixue prescription (Huayu prescription) combined with levonorgestrel-releasing intrauterine system (LNG-IUS) in the treatment of adenomyosis (AM). Methods A prospective,randomized,double-blind,placebo-controlled trial was designed and 102 patients with AM admitted to the outpatient department of International Peace Maternal & Child Health Hospital and The First Affiliated Hospital of Naval Medical University (Second Military Medical University) from Dec. 2019 to Dec. 2022 were enrolled. The patients were randomly divided into Huayu prescription group and placebo group for double-blind clinical trial. The Huayu prescription group was treated with Huayu prescription 1 month after LNG-IUS placement,while the placebo group was treated with placebo 1 month after LNG-IUS placement,and both Huayu prescription and placebo were taken for 3 months. A total of 95 patients completed the follow-up,including 47 in the Huayu prescription group and 48 in the placebo group;and 7 were shed,with a shedding rate of 6.86%. The pictorial blood loss assessment chart (PBAC) score,uterine spotting days,visual analogue scale (VAS) score,uterine volume,serum carbohydrate antigen 125 (CA125) level and traditional Chinese medicine (TCM) syndrome quantitative score were compared between the 2 groups,and the safety was evaluated. Results After 3 months of treatment,compared with the placebo group,the PBAC score,spotting days,uterine volume,serum CA125 level and TCM syndrome quantitative score of patients in the Huayu prescription group were all decreased (all P<0.05),but there was no significant difference in the VAS score of dysmenorrhea (P>0.05). During the follow-up,no patients in the Huayu prescription group but 2 patients in the placebo group received surgical treatment (including 1 case of laparoscopic hysterectomy and 1 case of laparoscopic adenomyomectomy),and there was no significant difference in the surgical rate between the 2 groups (P>0.05). Meanwhile,no obvious adverse reactions were found in both groups. Conclusion Huayu prescription can significantly improve the spotting of patients with AM,promote uterine volume reduction,reduce serum CA125 level,and significantly improve the TCM syndrome of AM. It is suggest that AM patients should take Huayu prescription after placing LNG-IUS,so as to reduce the adverse reactions of LNG-IUS.
